Block Heels Provide Better Weight Distribution Than Stilettos: CA Podiatrist
According to Tea Nguyen, DPM, “The shorter the heel, the closer you are to the ground, which reduces pressure at the ball of the foot.” The 2-inch heel keeps the shoes walkable—because who really has time for car-to-door shoes? There are other factors that contribute to comfort and walkability, too. Dr. Nguyen notes that a block heel will provide better weight distribution than a stiletto, and that a cork footbed will absorb more shock than plastic.

Dr. Tea Nguyen |
“An analogy I like to use is picturing a slide on the playground,” she says. “The steeper the slide is, the more gravity is exerted downwards. [This] is the pressure the ball of the foot experiences. Less steep is better for pressure reduction, and flat on the ground is best (zero drop).”

PRACTICE MANAGEMENT TIP OF THE DAY |
Ways to Boost Collections at Your Practice
Boost your medical practice collections with nine proven revenue-cycle tactics that cut denials, speed payments, and keep cash flowing with no extra staff needed.
4. Set up empathetic payment plans
High deductible health plans can blindside even diligent patients. Structured payment plans — automated, interest free and clearly documented — keep revenue flowing without souring relationships. For nuts-and-bolts advice, see Medical Economics’ take on making payment plans work.

QUERIES (CLINICAL) |
Query: The Use of Injectable Hyluronidase Gel for Arthritis of the Feet
For decades, orthopedists and other medical specialists have been injecting hyluronidase gel into knees. There have been a few papers published suggesting that this can be helpful with arthritis of the foot and ankle. Question: How many of you have used this for foot and ankle arthritic conditions and does insurance cover it for foot and ankle osteoarthritic conditions?

RESPONSES/COMMENTS (WOUND CARE) |
From: Joseph Borreggine, DPM
It looks like wound care is going to be facing much needed upgrade since it has recently been discovered that in 2019 the cost of wound care to Medicare was $250 million and as of 2024 that cost skyrocketed to $10 billion a year, which is more than what they spent in this country on cancer treatment in that same year. The number of patients during that time who received wound care only doubled, meanwhile the cost went up 40 times.
Based on this amount of money being spent by Medicare on skin substitutes, they decided to use this new program, which is considered “voluntary.” This will affect three MACs which include five jurisdictions within those MACs and will only be in six states. This program will be ongoing until 2031. Based on the results of this new voluntary policy, it may include the entire...

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1A |
From: H. David Gottlieb, DPM
The only workable solution to the DPM v. MD degree debate that I can see is based on what many of our colleagues have done. All 11 podiatry colleges should work out an agreement with an off-shore medical college such that all graduates can receive a DPM/MD/DO degree. If a graduate wants to practice as an MD/DO, they will sit for all the appropriate exams, and if they pass, apply for a medical residency; they can then receive a license in their state of choice as an MD/DO.
If they wish to practice as both an MD/DO and a DPM, well, they will do both and go through both processes to be licensed as both an MD/DO and as a DPM. Merely having an MD/DO or DPM degree does not mean that one can practice as one. You still need the appropriate license. Of course, the MD/DO license would allow one to practice foot care as a podiatrist does based on the DPM training as well as that upper extremity and treat warts, or whatever.
I believe that this model would allow for a greater pool of applicants. It would also end the incessant perennial debate that one must scroll through here just to find some new bit of opinion or tidbit of news.
H. David Gottlieb, DPM, Baltimore, MD

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1B |
From: David Secord, DPM
I have a niece, whose husband is a journeyman electrician. He is now 30 years of age, is the head of his crew of 15 and they do large, industrial wiring projects. The last time I talked to him, he let me know that he makes about $250,000 a year, after spending six months in school to obtain his certification. His crew has done projects around the world, with the accommodations picked up by the client.
I really like what I do for a living but have to admit that the preponderance of individuals spending four years in a University setting to obtain a degree in underwater basket weaving or some such other waste of time should really look at what plumbers, electricians, welders, pipe fitters and tool makers are earning. With that earnings potential, without massive school debt, and zero chance of malpractice actions, these occupations are also a threat to our professional longevity. Searching for warm bodies to occupy seats at our schools, as opposed to those with a drive and thirst to help people and serve the profession is not the way to go. Just my opinion.
David Secord, DPM, McAllen, TX
