MI Podiatrist Discusses Consequences of Common Bunion
According to Conway McLean, DPM, "Sufferers of hallux valgus may first experience pain on the side of the second toe, not even technically part of the bunion. But the big toe is, indeed, bigger and can exert significant force on the neighboring digit. Repetitive pressure has many effects on skin, and many are not good, especially in the presence of vascular or neurologic incompetency (e.g., diabetic nerve problems and arterial disease)."

"A reliable byproduct of this bony migration is altered joint motion. Any time a joint moves chronically out of position, there will be increased wear on the cartilage, with inflammation and bone spur formation also generally resulting. This latter issue leads to bone growth on the side of the head of the metatarsal bone, adding to the size of the prominence," says Dr. McLean.

There seems to be a bit of naivety within this question, and thus it is best to assume the provider asking this question is relatively new in practice. The following is not to unduly alarm anyone. Instead, it will hopefully be an attempt to provide a clear understanding of the differences between risk adjustment and post-payment audits.
Ciox and other risk adjustment audits are audits on whether or not the carrier, typically a Medicare Part C plan, can mine enough data from the provider's chart. This data is used to determine if the carrier can attempt to recoup more money from CMS. In plain English, the carrier is attempting to use your data to make more money from the government. The carrier is not auditing your claims to see if they should take money back from you. On these audits, you can and should charge for the data. Presently, to my knowledge, no provider has been booted from a plan for not providing this data.
This is a far cry from what you are experiencing with Noridian. As either your local MAC or the DME MAC (A/D), or CERT contractor, Noridian is asking to review your charts, to see if they paid you correctly. That is...

I disagree with your daughter's school decision. If the state where her school is located includes podiatry in the definition as a physician, then I think you or your daughter can argue/refute the administration's decision. I once had an MD/DO point out to me that I shouldn't be parking in a physician designated parking spot. I politely told him I am considered a physician in the eyes of the Ohio medical board and left my car where I parked it. Nothing came of it and nothing more was said.

I am sorry to hear that your daughter was denied the privilege of your being able to put the white coat on her future White Coat Ceremony at her present school of osteopathy. In my opinion, this sends the wrong message to the new students. Recently, I attended my son's WCC at a medical school and was allowed to put the white coat on his shoulders without any controversy.
As I understand, the WCC was designed by The Arnold P. Gold Foundation. I would contact this foundation and let them know your situation. Maybe they have a position that you can present to the osteopathic school your daughter is attending and hopefully help change their thoughts about all this. Regardless of the outcome of this situation, your daughter should be very proud of you for being a DPM.

I find it interesting that the DO school which Dr. Michael’s daughter is attending won’t let him do the white coat ceremony for his daughter. I was allowed to do this for my daughter at the DO school in Michigan and also help the Dean give her diploma to her after her four years at the graduation ceremony. After being in practice 30+ years in the state, to be with our daughter, we moved to North Carolina
She was the first DO doctor to do the palliative fellowship at Vanderbilt and she was, at times, talked down to because of her DO degree. Professional degrees status varies in different states and around the world. I believe that Michigan’s DPM status is due to the fact we had the first residency in the country at Civic Hospital in the early 60s. I am hoping that the number of residencies continues to grow in the states where we lag behind in them, and through interaction with not only our colleagues, but other health professionals, i.e. MDs, DOs, and other recognized providers.

Thank you, Dr. Moore, for giving us your perspective on what we as podiatrists can do to help our at-risk patients prevent falls. Having lost a grandparent to a fall as well as patients, fall prevention has always been a high priority for me. This past year, former vice presidential candidate and senator, Joseph Lieberman, died after sustaining a fall.
It is a pleasure to read that Dr. Moore along with Jason Kraus have developed a multifaceted approach to the identification and treatment of at-risk patients. Management is not just the "Moore Balance Brace" but physical therapy and other products that we can recommend. Fall prevention should be an ongoing effort on the part of all of us.
