OH Podiatrist Provides Tips to Keep Kids Injury-Free
"With the start of school comes the excitement of fall sports. It also brings a higher risk of injury for young athletes. Foot and ankle injuries are especially common, and the good news is many are preventable with the right preparation. Before the season kicks off, encourage your child to gradually build their activity level through safe, age-appropriate conditioning. This includes bodyweight strength training, agility drills, and balance work — especially focused on the core and lower legs — to support healthy movement patterns and reduce strain on the feet and ankles," says Kayla Schaefer, DPM of Balance Foot & Ankle Wellness Center.

"Always ease into new sports or higher levels of activity. A sudden spike in training is one of the leading causes of injuries in youth athletes, as it overwhelms muscles, tendons, and joints that haven’t had time to adapt. Dynamic warm-ups before practices and games are key. Focus on ankle mobility, calf flexibility, and balance to prepare the body and prevent injury. Post-activity stretching is equally important — it helps reduce tightness and supports recovery and long-term flexibility. Finally, teach kids to listen to their bodies. Pain is not just “part of the game” — it’s a signal that something may be wrong. When in doubt, early evaluation by a foot and ankle specialist can prevent small problems from becoming season-ending injuries," says Dr. Schaefer.

Podiatry and Wound Clinics Open in Coshocton Regional Medical Center
Ohio podiatrist John Smilo, DPM has partnered with Coshocton Regional Medical Center to continue his podiatry clinic while addressing what he saw as a critical need in the community. Dr. Smilo recently moved his practice into the west wing of the second floor of the hospital and became co-director of a new wound care clinic, along with Dr. Grant Lewis.

The wound care center provides personalized treatment for chronic and non-healing wounds, such as diabetic ulcers, surgical wounds, pressure sores, and soft-tissue injuries. It includes therapies like debridement and negative pressure wound therapy. Dr. Smilo said every county bordering Coshocton offers such wound care, but it wasn't available locally until now.

5 Things to Consider Before Rolling Your 401(k) into a Roth IRA
Converting at least some of an old 401(k) to a Roth IRA can offer long-term tax benefits and retirement flexibility, especially if you anticipate being in a higher tax bracket later or wish to leave a tax-free legacy.
1. What kind of 401(k) do you have? There are two main types of 401(k)s, and the kind you have will affect how a rollover to a Roth IRA is taxed. A traditional 401(k) is funded with pre-tax dollars, which means any amount you roll into a Roth IRA will be treated as taxable income in the year you convert it.
A Roth 401(k), on the other hand, is funded with after-tax dollars. That means you can roll it into a Roth IRA without triggering additional taxes. However, it must stay in a Roth account (you can't roll a Roth 401(k) into a traditional IRA) if you want to preserve its tax-free growth benefits.

Query: Facility Accreditation and Surety Bonding
I've recently heard that Medicare, in an attempt to combat fraud, is going to institute a yearly facility accreditation and they have also started cracking down on something I never heard of, surety bonding. What is this, and as a DPM, do I require this?
Response from thedoctorline.com: All physicians, including DPMs, are exempt from the Facility Accreditation (FA) and Surety Bond (SB) requirements. However, this exemption applies only as long as you are providing the DMEPOS to your own patients. Once you start filling Rxs from other doctors and that's all you are doing, you have crossed the line, and at that point you may be subject to...

I agree wholeheartedly with Jeff Root when he said, “I can assure you that it is not economically feasible for a company to make a profit and therefore stay in business selling custom orthoses for forty-five dollars a pair; and comply with the HCPCS definition of custom (i.e., an L3000 device) at the same time.”
Having been in private practice for over 27 years, and now serving as Medical Director for one of the premier custom orthotic laboratories in the world, I can say with confidence that all of us involved in the business of fabricating (truly) custom foot orthoses know what “custom” means and what it does not. Any company or vendor that is promoting what they offer/manufacture as truly custom orthoses knows what the HCPCS definition is for such a device (e.g., L3000).
Custom means the device is made from a plaster or foam or digital replica of the patient’s foot. Labs that use a library system are not creating actual patient-specific custom devices from a positive model of the specific patient’s foot. Taking a device from a library and ‘customizing’ it does not make it a truly custom orthosis either. Labs that take either of these manufacturing paths are choosing to not follow the universally-accepted definition, usually for their own economic gain and hopes of gaining market share.
Perhaps our podiatric organizations as well as publications that receive revenue from these dubious companies should have higher standards for what they will accept from vendors – rather than letting them say/sell whatever they want as long as they pay the advertising bill. We can do better as a profession and for our patients.

Dr. Purdy may be correct in stating that one does not need to attend college to make a living, and we are all aware of famous billionaires who achieved success without graduating from college. The problem we are dealing with in PM News is not the guy who forgoes college to become a plumber or electrician. We are facing a serious shortage of applicants to podiatric medical colleges. If the government does cut loans to these applicants, the problem could become far worse.
Expecting most of our podiatric medical colleges to fund loans to applicants is a pipe dream. Our institutions do not have the kinds of endowments that the Harvards and Yales of the world have. Perhaps some of the directors of our podiatric medical colleges could enter this discussion and let us know what percentage of their student population relies on government loans in order to foot their tuition.

Thank you, Dr. DiResta, for pointing out the fact that APMA discontinued their sponsorship of the Dartmouth MPH program. I was totally unaware of that, but the fact remains that when it was in place, it provided graduates who went onto non-clinical careers or it simply improved those individual's status in whatever clinical programs they were involved with. Now that Becker's has published the story I wrote about where both MD and DO programs are offering dual programs for medicine and MPH or MBA, perhaps it is time that APMA reassess its importance. Perhaps APMA, if it cannot afford to sponsor this program itself, can partner with other private enterprises which have profited from podiatry well over the years to offer such scholarships to individuals interested in one of these programs.
