FL Podiatrist Comments on Botched Ankle Fusion
 
When General Hospital star Kin Shriner underwent foot surgery last year, he had no idea it was going to leave him unable to walk for six months. “A year ago tomorrow, I had some arrogant surgeon give me a subtalar fusion,” the actor explained. “And he botched it." The botched surgery Shriner experienced was subtalar fusion surgery, or a surgery to fuse the two bones of the subtalar joint located in the heel. Although Shriner, did not reveal the reason for this surgery, it was most likely to manage severe arthritis pain, explains Jodi Schoenhaus, DPM, a podiatrist at the Foot, Ankle & Leg Vein Center in Boca Raton, FL.

“Subtalar fusion is recommended as the final measure when the joint pain becomes painful and arthritic,” Dr. Schoenhaus explains. “Patients who have decided to undergo this surgical reconstruction have already failed various forms of conservative care, including braces, injections, physical therapy, and orthotics, and they still are in pain daily. “The largest risk that any fusion procedure presents is a non-union, which occurs in up to 10 to 20 percent of cases,” Dr. Schoenhaus explains. “Other problems may be hardware complications, infection, irritation of nerves, or mal-alignment when the bones fail to fuse. It’s not common, but possible.”

An old Wall Street adage goes: There's no such thing as a free lunch. Every investment comes with costs, and those costs come straight out of your return.
 
The average mutual fund fee is around 0.4%, but some funds charge over 1%, according to the Investment Company Institute. That might not sound like much, but over time, even small fees can compound into thousands of dollars lost. Legendary investor John Bogle put it plainly: "In investing, you get what you don't pay for. Costs matter."

I’m very surprised that someone hasn’t questioned the validity of these so-called “custom” orthoses being sold for forty to forty-five dollars per pair, including delivery, prior to this thedoctorline.com inquiry. Also, I’m disappointed that the concern expressed is related to coding liability as opposed to the possibility that the practitioner and or the patient may be a victim of fraud. In addition, one should question the efficacy of such devices. Dr. Kesselman did an excellent job of explaining that in order to determine if a custom orthosis meets the HCPCS definition of custom fabricated, one would have to know the exact manufacturing process. I can assure you that it is not economically feasible for a company to make a profit and therefore stay in business selling custom orthoses for forty-five dollars a pair; and comply with the HCPCS definition of custom (i.e., an L3000 device) at the same time.
 
I am aware that some distributors who sell raw materials and pre-fabricated shells to podiatrists and labs alike, sell pre-fabricated orthotic shells to some of these companies who sell “custom” orthoses at these unbelievably low-price prices. Part of the problem is that the seller’s definition of custom may be different than the HCPCS's definition of custom. For example, if a doctor sends a cast or a scan to a lab and the lab uses a pre-fabricated shell and customizes it by adding a blue top cover as opposed to a red one, and maybe a met pad and a heel lift, is that a custom made device? One could argue it is because it was made-to-order and was customized to the doctor’s specifications, but it certainly wouldn’t meet the HCPCS definition of custom. In the interest of practitioner and patient protection, this issue needs much closer inspection.

Dr. DiResta gets it! It's not because he mentions me or my ideas about the DO path to an unrestricted license but because of his observations concerning the DPM degree NOT being a starting point to move forward. It is a starting point to move laterally. You can have an MHA, MPH, or a PhD in healthcare, but try to aspirate a knee and see how far a master's degree gets you.
 
Get the DO degree, then enough ACGME-approved residency time to get an unrestricted license, followed by an APMA fellowship in podiatry from Temple, Touro, LECOM, DMU, or UTRGV. Now you can move forward from your starting point.

I too enjoy the posts in PM News. Whether one agrees with the authors or not, doesn't detract from their insightfulness. I appreciate the dialogue presented by Drs. Kesselman and DiResta and others on this topic. Considering how things are evolving for podiatrists with diminishing numbers of qualified (academically prepared) applicants, likelihood of schools (some or all) closing and increasing number of MD/DO schools on the horizon. It would seem to me a likely scenario might be that future applicants matriculate to MD/DO schools (domestic/foreign), complete orthopedic residencies and foot and ankle fellowships (or others), then train physician extenders (LVN, RN, NP, PA, etc.) in C&C, nail care, and primary podiatry. I see this as a more plausible pathway than watching our decline and obsolescence.

I’m happy to hear government funded loans are being regulated. I frankly feel they should be discontinued. Nowhere else can someone get a loan for almost any amount with bad credit, no credit history, or for educational tracks that historically do not create enough income to repay the loan. Government loans are not low interest loans as they are touted. They are not subject to bankruptcy. If one becomes unable to pay, the interest is compounded and the repayment exponentially increases. If one has to claim bankruptcy, the loan is not discharged and continues accruing compounded interest, becoming a noose around that person’s financial neck.
 
Educational institutions have figured out, no matter how much of an increase they place on the cost of education, the government will finance it, hence the high cost of education. These institutions have even added degrees that lead nowhere, since the institution bears no liability in the future success of the student.
 
How does one receive an education without government loans? The institutions themselves can and should back the loan and have some skin in the game. There are private sector loans and scholarships available. Let’s face it, not everyone should attend college. We have a serious deficit in trade school applications, and the reason why plumbers and electricians are making more than doctors. One does not even need to attend school to have a successful business or career. Our education system is so flawed, it’s no wonder a person ends up in their late twenties yet to generate income, all the while creating debt.
