MA Podiatry Resident Has Unfinished Business on Golf Course
 
It has been six years since Eric Marchetti, DPM, the former North Attleboro High and University of Rhode Island (class of 2017) standout, teed it up in the AAGA City Open. Now Dr. Marchetti is in his second year of podiatry residency at St. Elizabeth’s Hospital in Brighton, although he still lives in North Attleboro. But Marchetti found a few hours from his schedule on Friday to play 18 holes at Heather Hill CC, hoping to regain his scoring touch. Marchetti was en route to the title in 2018, but took a nine on the No. 10 hole at Norton CC and fell into second place at 296, finishing one shot behind champion Kolby Simmons.

A year earlier, in 2017, Marchetti was the No. 4 finisher at 297, six shots behind titlist Scott Congdon. “I feel like I should have won. I still look back at that and feel like I have some unfinished business,” Marchetti said. On Friday, Marchetti finished with a 1-over-par medalist round of 71 on the Middle and South courses, collecting nine pars and four birdies.

Reviewing this dermatology website, there are about 14 different items that this could be. Without a detailed history, perhaps using dermoscopy, and yes a nail unit biopsy could give you a more definitive diagnosis.

My daughter is a 1st year student at a college of osteopathic medicine. Next month, I am going to her white coat ceremony. They informed my daughter that they will allow parents who are MDs or DOs to coat their kids, but not a DPM as we are not considered physicians. Also consider, the 3 other schools she was accepted to would have allowed a DPM to coat a student. My daughter is very upset as she told them I am a surgeon and board certified. Their answer was we just call them doctors for respect, but we do not consider them physicians as they have a limited license. Any thoughts?

Walking forward REQUIRES the ability to step over the weight-bearing foot. If the ankle is “stabilized” sagittally (read as locked), and the foot plate is full length blocking MTP joint motion, it becomes IMPOSSIBLE to do this. Adjusting the foot plate to end behind the metatarsal heads, and adding a 1st ray cutout to facilitate MTP joint dorsiflexion makes these braces immediately more comfortable.  
 
I’ve done this countless times, and patients are amazed at how much easier it is to walk and how their arch pain spontaneously resolves. Even if these braces are for drop foot, since they are ALWAYS worn in shoes, there is no issue in stabilizing their gait, despite the fear of the orthotists. And since gait speed and balance are directly proportional, this adjustment allows for a faster gait and therefore improved balance.

Dr. Kesselman asks why a podiatrist has never led a team of researchers or published research concerning the correlation between gait and general health. Dr. Jacobs very succinctly states the reason is because we don’t perform a true gait analysis anymore. Besides, it’s difficult to perform a true evidence-based examination in the office. Experience-based maybe, but real objective data is hard to glean from the equipment most podiatrists have on hand. Most reputable journals would scoff at an article submitted with the title, “Too Many Toes Sign and…” or “Wet Foot Impressions on the Floor in Diagnosing Pes Planus and Pes Cavus.”
 
But Dr. Kesselman’s question addressed why a podiatrist hasn’t authored the study. I posit there are a few reasons. One, a biomechanics paper is simply not as sexy as a supramalleolar osteotomies manuscript. Secondly, podiatry is primarily an action profession, not an...

I agree with Dr. Jacobs' assessment that commercial driven ideas are often passed off as vetted gait research. In addition to all the direct access “biomechanic  solutions” that are directly marketed to patients at online and brick and mortar retail outlets, other specialties have begun to fill the void that has been created by many but not all residency programs and other podiatry seminars. Recently, a friend sent me a popular podcast that featured a chiropractor talking about gait and a strong pitch for the benefits of minimalist shoes. There are physical therapy clinics that have specialty run clinics that also fill the void. Another colleague sent me a course called Gait Gurus that reviews cases and is organized by chiropractors. 
 
When I was consultant at NIH, we used to routinely have a biomechanics PhD, orthopedic surgeon, physical therapist, physiatrist, and me watch a patient walk and collectively make recommendations for treatment. I make it a point to try to video my patients’ gait as much as possible and review it with them. 
 
Hopefully, our profession is past the recognition point that we are being superseded by other commercial entities and specialties and is now making active course corrections to remain relevant in the area of gait.

When I was working, we never had a policy of charging a fee for a no-show. Between handing out appointment cards and confirming patients the day before, our no-show rate was very low. We were fortunate enough to have a busy practice so an occasional no-show was not worrisome. Conversely, I would think a practice with lower patient appointments would think twice about charging for no-shows for fear of losing a patient. As we all know, patients can be very fickle. Keeping patients happy seems like the way to go.
