Canadian Podiatrist Facing Questions Over His Credentials 
 
A Prince Edward Island podiatrist who calls himself "Dr. John Johnson, DP" and treats Islanders for various foot conditions is facing allegations he has no formal credentials. CBC News has asked John Johnson for proof of his credentials, but he hasn't provided it to them.
 
Although podiatry is regulated in most of Canada, it isn't in Prince Edward Island. "If there were regulations in place, this wouldn't or shouldn't happen, and there'd be a consequence to someone," said Howard Green, DPM, president of the Canadian Podiatric Medicine Association (CPMA). "But without the regulations in place, unfortunately it's a bit of a free-for-all."

Seven Communication Sins Every Practice Should Avoid
 
Avoiding seven common communication errors can boost the productivity of the average medical group by 80 percent. So claims Skip Weisman of Weisman Success Resources, a Poughkeepsie, NY-based consultancy.
 
1. Lack of Specificity
 
Weisman recommends avoiding phrases like "anytime," or "when convenient," in favor of precise details. Weisman gave the example of a manager who told him to call "anytime," but when Weisman tried to follow up, the manager was never available.

QUERIES (CLINICAL)
Query: Transient Discoloration of Fingernails  
 
A 46-year-old type two diabetic, BMI of 31, has been on testosterone therapy for a few years and has recently been taken off therapy. He has been having some generalized neuropathy complaints in his legs, however he has discoloration on the sides of his fingernails but nothing on his toes.

He states that this will appear for a few months and then go away and come back. It is not painful. He has tested positive for ANA with a pattern consistent with lupus a few times. His rheumatologist did not think it was worth treating. Any input or thoughts if you recognize what’s going on in his fingers that might be contributing?

Dr. Kass raises some valid questions regarding the efficacy of "balance braces" to actually prevent falls in the elderly population. In response to his first question, "Is there any proof at all in the medical literature that wearing of this brace prevents falls?" The answer is "No" as documented in this article I wrote in 2019: See: New Studies Refute Claims Of Fall Prevention With AFO Bracing
 
In his second question, Dr. Kass asks about the effects of reducing sagittal plane ankle motion with balance braces. Numerous studies show that static, non-articulated AFOs have negative effects on balance compared to articulated AFO braces. These studies are summarized in this article which I wrote in 2018: AFO Bracing And The Elderly: What The Literature Reveals.

Some years ago, tapentadol was released as an alternative for the treatment of post-operative pain. Bunionectomies were selected for the study. No podiatrists were authors. Dr. Kesselman, if you speak to students on outside rotations and particularly residents, you will find that patients seldom if ever are subjected to weight-bearing examinations. Of course ABPM and CPME will quickly raise the requirement for a limited number of so called “biomechanical examinations” as a requirement. However, this is an examination that should be ongoing daily for 3 years. This includes patients being evaluated and treated for conditions undergoing surgical decision-making.
 
The fact is that gait analysis is not a routinely observed part of the podiatry evaluation observed by many podiatry residents. Therefore, it is not surprising that an evaluation of the relationship between gait and general health did not include podiatry...

I wanted to add to my recent post as to why we charge for no-shows. First, all patients sign a document clearly stating our no-show policy. We have that policy online when they make an online appointment that they must read and accept. We call, text, and email every patient at least once or twice before every appointment. Just this overzealous communication concerning their appointments has drastically reduced the no-shows. However, for the few that are chronic or do not care, despite our Herculean efforts to notify them of their appointments, we do sometimes bill a no-show. The policy is hardly a money-grabbing situation. We work hard to make sure our patients are responsible like we are.
