PODIATRISTS IN THE NEWS
Self-Care Remedies May Help But Won't Correct Bunions: NY Podiatrist

Over-the-counter “bunion correctors and splints may reduce shoe irritation, but these products are unlikely to reduce the actual deformity,” explains Michael J. Trepal, DPM, a podiatrist and a professor of surgery at the New York College of Podiatric Medicine of Touro University. “There’s no good research that these will routinely correct a bunion.”

Dr. Michael Trepal

“You can get some bunion pain relief with over-the-counter pain reducing creams or perhaps even one with cannabis compounds,” says Dr. Trepal. Topical cannabis oil or creams may have anti-inflammatory and analgesic abilities, he adds. Orthotics can ease pain and may help prevent a bunion from getting worse. “Custom-made orthotics, following a comprehensive biomechanical evaluation by a podiatrist, is generally the best route,” says Dr. Trepal.

ON THE INTERNATIONAL LECTURE CIRCUIT
CA Podiatrist Lectures at Podiatry Seminar in Brazil

Last weekend, Kevin Kirby, DPM lectured at a clinical workshop and 2-day conference, the Congresso Internacional De Atenção Aos Pés (CIAP), in the largest city in Brazil, Sao Paulo. On the first day, Dr. Kirby held a 2-hour clinical workshop for a group of 45 podiatrists on foot anatomy, biomechanics, and gait examination.

Over the next two days, Dr. Kirby lectured on "The Biomechanics and Orthosis Treatment of Pediatric Flatfoot Deformity" and on "The Longitudinal Arch Load Sharing System of the Foot." Over 800 podiatrists from the countries of Brazil, Mexico, Argentina, and Peru attended the conference. The CIAP is the largest annual podiatry conference held in Brazil, with 20 other podiatrists, physical therapists, biomechanists, and physicians also lecturing at this very successful podiatry conference, organized by Brazilian podiatrist, Bruno Gallart.

PRACTICE MANAGEMENT TIP OF THE DAY
5 Ways AI is Reshaping Private Practice

From ambient transcription to smart scheduling, AI is helping doctors cut costs, save time, and improve care. Here’s where it’s making the biggest difference — and what to consider before you invest.

5. What to ask before you buy

With AI tools handling sensitive data and influencing clinical decisions, practices need to ask tough questions before adopting new software.
  • Is it HIPAA-compliant?
  • How is the model trained?
  • Is it vulnerable to bias?
  • Can this model integrate with my existing EHR?
These questions help ensure patient safety and protect your practice from compliance risks.

FINANCIAL TIP OF THE DAY
7 Essential Investing Rules We All Should Know

Here are seven essential rules every investor should know. They may not turn you into a billionaire, but they just might help you build the future you want.

5. Know your "Why"

A number doesn't define success in investing. It's defined by your ability to achieve "why" you're investing in the first place.

When you start with a specific goal, you might realize you don't have to take on maximum risk to reach it. Instead, you can work backward to build a plan that fits. Whether it's buying a house in five years or retiring in 20, your timeline and priorities should drive your strategy.

CODING & BILLING Q&As FROM THE DOCTORLINE.COM
Query: Eccrine Disorder

What is the recommended treatment code for L74.8 (eccrine disorder) with debridement of the lesion. My first impression would be callus debridement (1105-7), but it is not really a callus. The treatment is not destruction of a benign lesion.

Jeff Leibovitz, DPM, Indianapolis, IN

Response from thedoctorline.com: Let's start with the definition of CPT 11055-11057, defined as Paring or cutting of benign hyperkeratotic lesion (e.g., corn or callus);11055= single lesion, 11056= 2-4 and 11057=5 or more. You are correct; you stated it is not a callus so it would not be appropriate. You biopsied the growth and determined it was a "L74.8 Other eccrine sweat disorders" or was it a "L85.8 Porokeratosis, acquired" or was it a "D23.7- Porokeratosis (other benign tumor)"

We often find the incorrect diagnosis is assigned to the abnormal skin growths and it is important to know that is the correct diagnosis as to what you are treating. Using the correct terminology is key. If you are removing tissue,

RESPONSES/COMMENTS (NON-CLINICAL)
RE: DPM is NOT a Dead-End Degree; It Is Just a Starting Point
From: Paul Kesselman, DPM

Over the last few years, there has been an increase in the "bashing" of the DPM degree by many of my colleagues. Fortunately, there have been many of my colleagues who have called for this eternal bashing to stop. The reasons for those who are not happy with their current lot vary as do those who are satisfied with their careers. But I suspect there are many in the MD/DO, JD, accounting, and other fields who are also not happy with their current career path.

I am not an occupational sociologist or psychologist and cannot get into the mindset of those who constantly bash this profession or specialty (that too seems to be debatable). The purpose of this letter is to demonstrate that there are many individuals who have taken their DPM and used it as a springboard to go...

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION)
From: Elliot Udell, DPM

If what Dr. Tomczak says is true, that there is an annual and lifetime cap on loans to medical students, we might not have to worry. Such a ruling will affect medical, osteopathic, dental, optometry, and physical therapy doctoral programs. The old saying is "misery loves company."

This would give all the professional societies a chance to pool their funds and work together to petition for this to be overturned. Together we could hire the biggest and best lobbyists. I am sure that if this is true, all of the societies are preparing to deal with it.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Lawrence Rubin, DPM

In a recent poll, many practicing podiatrists were unclear about the requirements of the Stark Law.  So, I asked AI for its take on the Stark Law essentials podiatrists should know. Here is AI's reply:

"The Stark Law, also known as the Physician Self-Referral Law, is a complex federal law that impacts physicians, including podiatrists, who refer patients for Designated Health Services (DHS) payable by Medicare or Medicaid to entities with which they (or their immediate family members) have a financial relationship. The law aims to prevent potential conflicts of interest that could lead to unnecessary healthcare services and increased costs to the Medicare program.

Here are some key essentials podiatrists should understand about the Stark Law:

*Prohibition on self-referrals: Podiatrists cannot refer patients for DHS to an entity where they (or a family member) hold a financial interest, unless a specific...
