INTERNATIONAL PODIATRISTS IN THE NEWS
Irish Sisters Follow in Podiatrist Father's Footsteps

Two sisters from Ballymote have followed in their father’s footsteps by graduating from Queen Margaret University (QMU) Edinburgh. In July this year, Rachel Thorpe was awarded Master of Podiatry (MPod), while Jessica Thorpe achieved a BSc Podiatry from the same institution where their father qualified as a podiatrist over 30 years ago.

(L-R) Jessica and Rachel Thorpe

The girls were inspired by their father Wilson Thorpe to pursue a degree and a career in podiatry. Wilson graduated from Queen Margaret University in 1992 and has been working as a podiatrist in Ireland ever since. He runs the Wilson Thorpe Podiatry practice in Sligo.

MEDICARE NEWS
CMS Proposes 3.6% Pay Bump for Docs

The Trump administration is proposing a pay bump for physicians in 2026, alongside new reforms that align with the agency's Make America Healthy Again positioning. The Centers for Medicare & Medicaid Services released the proposed physician fee schedule which would set the conversion factor, or the amount that Medicare pays per work relative value unit, at $33.42, an increase of 3.62% over the 2025 rate of $32.35.

That increase accounts for a 2.5% payment adjustment required by the Big Beautiful Bill Act, and a bump of 0.55% to account for changes to some RVUs, per a fact sheet on the rule. The agency also said it intends to set the conversion rate in qualifying alternative payment models at $33.59, an increase of 3.83%.

RESPONSES/COMMENTS (CME)
RE: Rules Governing CMEs
From: Jeffrey Kass, DPM

I have attended  many online webinars lately and have been baffled by the rules governing CMEs. The past 2 seminars were about a product and a drug respectively. Both had "catchy" names. I found it ridiculous that the speakers did not state the name of the product or drug during the presentations as "this is not allowed". I'm sure many, if not all, attendees knew both the product and the drug. That being the case - call it what it is. Why play games? It's bad enough insurance companies play games; I don't need to play more games when viewing webinars, whether in person or online.

In the first webinar - the one with the product, - after the hour was up, they went to "part 2"  of the program where you found out how to purchase and dispense the product. Talk about a "bait and switch". We were not told about a part 2 until part 1 ended.

As long as there is full disclosure, I would have been happier to hear the name of the product and where to get it during the presentation. Let's teach how this drug can help your patient population. We are all grown-ups and can make decisions on our own. Regarding the drug that was being discussed - describing everything about it, but not the name of it - seems rather dumb.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: STAND+ Shoes (Jack Ressler, DPM)
From: H. David Gottlieb, DPM, Robert Scott Steinberg, DPM

First, I have no relationship with STAND+ Shoes. I received a pair of these shoes for evaluation after a meeting in DC last year. I was told they were meant for standing in all day, hence the name. Also, they were heat moldable/adjustable with a hair dryer, as well as non-absorbent and machine washable. They sat in the box for a couple of months before I tried them on.

They are now my favorite shoes and all of the claims made by the company are true. I highly recommend them for anyone. I am not associated with STAND+ nor do l have any financial inducement to recommend their shoes.

H. David Gottlieb, DPM, Baltimore,  MD

How does the APMA determine who is eligible to receive its Seal of Approval? Do the companies whose products are endorsed by the APMA advertise with the APMA?

Robert Scott Steinberg, DPM, Schaumburg, IL

Editor’s comment: The APMA Seal of Acceptance and Seal of Approval are granted by the APMA to products that promote good foot health. The Seal of Acceptance is awarded to footwear, insoles, and materials, while the Seal of Approval is given to therapeutic products like lotions and medicines. These seals indicate that a product has been reviewed by podiatrists and found to be beneficial for foot health, safety, and quality. Source: APMA

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1A
From: Amol Saxena, DPM, MPH

I have only met Dr. Tomczak once (I believe at a meeting in Canada in 2003) and view him as a podiatrist dedicated to helping the profession advance. As he and others have related, the truth is hard for many to accept. So what if MDs read this? He is suggesting ways to improve the profession going forward and this may be unpleasant for many to hear as well as change. I have motivated over 30 students to become DPMs but have noticed various things have decreased the interest in the profession including limited license, difficulty in achieving board certification, and paying off student loans. Is it a rewarding profession: yes. Can you make a good living now, i.e. pay off your loans (look at the Big Beautiful Bill restrictions on educational loans and would podiatry even count?), buy a house, provide for a family including their education, and retire comfortably: It depends, but it is much more difficult for the current graduates.

I continue to mentor younger DPMs and help them publish valuable research as they don't seem to get the support to do this...

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1B
From: Bret M. Ribotsky, DPM

I wanted to take a moment to express my sincere congratulations on Dr. Block’s outstanding work with PM News these 8,000 issues. His dedication to journalistic excellence has not gone unnoticed, and I felt compelled to acknowledge the remarkable integrity he brings to our profession.

In an era where media landscapes are often polarized and one-dimensional, Dr. Block’s commitment to presenting multiple perspectives on complex issues stands as a beacon of responsible journalism. His willingness to explore various sides of the same story demonstrates a level of intellectual honesty that is both refreshing and essential for informed podiatric discourse. This balanced approach allows PM News’ audience to form their own well-rounded opinions rather than being fed a singular narrative.

Dr. Block’s moral standards and unwavering integrity shine through in every piece he...

RESPONSES/COMMENTS (MEDICARE FRAUD)
From: Paul Kesselman, DPM

Coming from a family deep seated in law enforcement, David Secord's recent letter definitely provides the unfortunate reality of why certain criminal actions are not taken against specific individuals. However, what his accurate description of the costs involved does not address is why it takes so long to build a case or at least initiate pre-payment review of certain individuals suspected for fraudulent or criminal investigations.

Audits can be approved by CMS and instituted by the Medicare carrier prior to payment and based on some accepted mathematical formula. This is way before the FBI or OIG get involved. And way before any money is paid out and lost to the wind. I am not calling for the abuse of pre-payment authorizations as perpetuated by many in the insurance industry, who now instead become the criminals and deny...
