Town Hall Meeting on Board Certification Held In Nashville
On July 14th, the American Board of Podiatric Medicine (ABPM) joined the American Board of Lower Extremity Surgery (ABLES) and the American Board of Multiple Specialties in Podiatry (ABMSP) at a town hall style meeting to discuss the future of podiatric board certification. Present at the meeting was Lee C. Rogers, DPM, ABPM Board President, Frank Lepore, DPM, President of ABMSP, and Doug Dockery, DPM, Board Member of ABLES, as panelists fielding discussion from attendees of diverse podiatric clinical practices, backgrounds, and board certification status.

Approximately 100 attendees joined online and in-person at the Gaylord Opryland Hotel & Resort in Nashville, Tennessee to provide perspective from their experiences in all areas touched by specialty board certification. This included commentary on privileging issues at hospitals around the country, the consequences of the patchwork of state laws regarding podiatric medical licensing that vary from one locale to another, impressions from students in podiatric medical school and residents in training, and beyond.

MN Podiatrist Uses Community Garden as Extension of His Practice
The Rochester Clinic and Lotus Health Foundation uses their FarmacyRx garden in northeast Rochester to promote health and wellness through volunteerism and harvesting local vegetables. Jengyu Lai, DPM, a podiatrist and family medicine provider at the Rochester Clinic, uses the community garden as an extension of his practice. He said he asks patients about their vegetable intake and what veggies they do and do not like. This practice, Dr. Lai said, increases the possibility that they will eat those greens. The other health benefits, Lai said, come from working in the garden.

"You put your exercise, a more functional exercise, your labor into something that you get to see the result, not only your body but also what you grow," Liu said. "And also that helps you develop a sense of purpose, because once you grow something, you always want to check (it) out."

APMA 2023 Annual Scientific Meeting Wows Attendees in Nashville
Last week, APMA welcomed well over 1,000 podiatric physicians and surgeons, sponsors, exhibitors, and guests to its Annual Scientific Meeting (The National) in Nashville, TN. The meeting was an outstanding success with excellent lectures, hands-on workshops, a booming exhibit hall, and the engaged community that is a signature of The National.

Ross Taubman, DPM, president and chief medical officer of PICA, offered an inspirational opening address. The meeting played host to the Team APMA 5K Run/Walk to benefit the APMA Educational Foundation, as well as the Sixth Podiatry School Student Quiz Bowl, which saw Treloara Harrisson from New York College of Podiatric Medicine emerge victorious after the first-ever all-female final round.

Ways to Successfully Gather Online Reviews for Your Practice
Patients often rely on Google searches to find a healthcare provider, and the number of positive reviews your practice has determines how well you’ll rank when searched. The more reviews you get, the more likely you are to bring in new patients. Here are some ways to win more reviews for your practice and attract more patients over time.
2. Send review request emails to previous patients.
Requesting reviews over email is a tried and tested method. It’s important to keep your message short and sweet, especially if you want valuable Google reviews that prospective patients will be reading. The timing is vital, and you should send out a review request when the experience is still fresh in their minds. Use the short window after an appointment to send your email.

Query: Medicaid Not Paying the Co-insurance and Deductible
We are having challenges with Medicaid not paying the co-insurance and deductible for qualified Medicare beneficiary (QMB) patients. Some QMB patients enroll in Medicaid plans. A state Medicaid program might refuse to pay the deductible and co-insurance. They delegate the responsibility for a podiatrist to bill the managed care plan directly which Medicaid was really supposed to pay. Some of these QMB patients enroll in a Medicare HMO plan that has a co-pay that is high. The Medicaid plan can pay for a large percentage of the co-payment. If the Medicaid managed plans do not pay, there are state health departments that oversee these managed care plans. Any thoughts on dealing with this?

Response: This is a common conundrum around the United States in many payer jurisdictions. Often, one plan reimburses less or more than the other, which leads them to allowing one coverage and not adding on as they see it going beyond what they will cover. Plan A may pay more than plan B. So Plan B is happy to not add on as it exceeds their approved fee payment amount. This is a common problem in states like Alabama where the Medicaid plan will not pick up after Medicare has paid as it exceeds their approved amount already. Reaching out to your CAC representative from APMA and/or insurance commissioner may be helpful, with the former being most likely to explain this better and open any dialogue needed with the payer.

The American Podiatric Medical Association (APMA) article indicates that there have been some improvements made in cuts for Medicare payments that were to take place in 2024 and 2025. It concludes, "While this outcome is better than recent proposals that decreased payment to podiatrists, APMA, along with other medical societies, will seek relief on the cut to the conversion factor, as has been the case in recent years."
The Academy of Physicians in Wound Healing's (APWH) recent newsletter reported that a number of medical specialist societies are supporting bipartisan bill HR 3674. The bill addresses medical care for various specialties for senior citizens and proposes to roll back payment cuts and restore reimbursement cuts that are to initiate in 2024 and 2025.
The bill will increase non-facility practice expense relative value units for procedures performed by office-based specialists. HR 3674 is bipartisan and sponsored by representatives Gus Bilirakis (R-FL) and Tony Cardenas (D-CA). If you want further information, you can contact your legislators. Thank you Dr. Rubin (who served on the Medicare HFCA CAC). You say that the reason for changes to the Medicare nail avulsion guidelines were mandated by The Medicare office Of the Inspector General. I find this quite surprising and shocking, since the “Inspectors General are empowered by Congress to identify and inform the legislative branch of incidence of waste, fraud, abuse, and mismanagement,” not formulate medical decisions or guidelines.
In reviewing the 21st Century Cures Act, The Social Security Act, and the Medicare Integrity Manual, there is no mention of The OIG having any role in developing LCD or treatment guidelines. MACs (Medicare Advisory Committee) have science committees, professional consultants, medical writers, scientists, researchers, advisory committees, statisticians, physicians,...
