Kristin Brown, DPM was recently presented with the Sole Supports Scholarship Award. Dr. Brown graduated from the 3-year Swedish Foot & Ankle Surgery and Medicine with Reconstructive Rearfoot/Ankle Surgery Residency Program, Seattle, WA on June 27, 2015.

NV Podiatrists Help Prevent Amputations in Diabetics 
 
An alliance of medical professionals is trying to create a new program to ensure that people have foot exams to head off treatable problems that can lead to severe infections, amputations, and amputation-related deaths. Doctors with Lower Extremity Amputation Prevention, or LEAP, met Monday with Nevada Lions Club members, medical executives, and healthcare professionals to launch the Comprehensive Foot Examination Assistant Program. The goal of the program is to provide primary care doctors with qualified assistants to provide diabetic and other at-risk patients with comprehensive foot examinations, said Dr. Larry Rubin, a retired podiatrist and founder of LEAP.

Dr. Kenneth Fatkin of Henderson, vice president of the Nevada Podiatric Medical Association, has said minor foot problems can become exacerbated over time, and preventing more intensive treatment procedures could result in significant savings in healthcare spending. LEAP members say their program would reduce the personal and economic burdens of diabetic lower extremity amputations through a collaboration of primary care physicians and trained foot exam assistants.

CMS Cutting-edge Technology Identifies & Prevents Improper Medicare Payments
 
After three years of operations, the Centers for Medicare & Medicaid Services (CMS) reported that the agency’s advanced analytics system, called the Fraud Prevention System, identified or prevented $820 million in inappropriate payments in the program’s first three years. The Fraud Prevention System uses predictive analytics to identify troublesome billing patterns and outlier claims for action, similar to systems used by credit card companies. The Fraud Prevention System identified or prevented $454 million in Calendar Year 2014 alone, a 10 to 1 return on investment.
 
The Fraud Prevention System helps to identify questionable billing patterns in real time and can review past patterns that may indicate fraud. In one case, one of the system’s predictive models identified a questionable billing pattern at a provider for podiatry services that resulted in Medicare revoking the provider’s payments and referring the findings to law enforcement.

Query: Medications to Consider for Neuropathy
 
I would appreciate some ideas on what type of medications would be advisable for the following patient. He has alcoholic neuropathy and neuropathy from several instances of frostbite. He reports swelling in his feet which aggravates the problem, however no edema was noted on examination. Pulses were 3/4 for the DP and PT bilaterally. Light touch and vibration sense were diminished. Blood work was negative for diabetes, thyroid disease, or Vit B 12 deficiency. He is a smoker who will most likely never quit. He is taking gabapentin 600 mg tid and is also on trazadone, sertraline, oxcarbazepine. Any suggestions?

RE: Fungal Cultures

In the past, I have always sent cultures in for plating by the microbiology department when considering placing patients on oral antifungals with potential side-effects. PAS stains (as well as the GMS stain) are histological techniques which are more specific when used to identify fungal infections. It has been published in the Journal of the American Academy of Dermatology: “PAS is the most sensitive method for the diagnosis of onychomycosis. It is also superior to the other methods in its negative predictive value. PAS is potentially the single method of choice for the evaluation of onychomycosis.”
 
So what I have been doing lately is sending two cultures: one to microbiology for culture, and the other to histology for staining (with the instruction that the micro department should not plate the specimen if the PAS stain is positive, but only if PAS is negative). This has caused a ton of administrative chaos. I was wondering how others are handling this situation.
 
Another question that I have is regarding the appropriate lab testing to follow the extended dosing of Lamasil. I have been dosing Lamasil in certain cases at 250mg/day x7 days, skip 3 months, and repeat for 1 year. The question is: Are baseline LFTs and CBCs required for this dosing regimen and should you take patients off statins for the single week that they are taking Lamasil?

I have great respect for Hal Ornstein, but the recent post about systemic candida infections being diagnosed by a naturopath makes no sense. Nothing in any medical literature hints that sugar in any form causes or affects systemic candida. The only place you will find this is in non-medical postings by pseudo-medical groups.
 
Secondly, systemic candidal infections are serious, life-threatening conditions which require potent antifungals, not sugar reduction (please see the article from the CDC). I am sure Hal felt better after removing sugar from his diet, and we should all do that, but to propagate the notion that a naturopath has the answer to a serious condition could lead to dire consequences.
