Pitting Edema as a Sign of Serious Heart or Kidney Problems: CA Podiatrist
 
A health expert has issued a warning over a "nasty" leg symptom that could be a red flag for several deadly diseases - including heart and kidney conditions. Dana Brems, DPM, a podiatrist who shares her insights on TikTok, highlighted pitting edema as a "common" indicator of these serious health issues. In a video response to another user's post showcasing their own encounter with pitting edema, Dr. Brems, also known as Foot Doc Dana online, delved into the details of this alarming sign.

To self-examine for pitting edema, Dr. Brems advises pressing down on your legs and ankles. A lingering indent lasting more than a couple of seconds should raise concerns. Dr. Brems underscored that both heart failure and kidney disease are common culprits behind pitting edema.

MA Podiatrist Discusses Bunions and Hammertoes
 
Louis J. DeCaro, DPM, President of the American College of Foot & Ankle Pediatrics appeared on Mass Appeal to talk about bunions and hammertoes. Dr. DeCaro delivered the message that “Prevention begets correction.” He encouraged early intervention, and that understanding biomechanics  at an early age helps limit these conditions later in life. “There are 24 different foot types. Based on your foot type, you can be prone to certain orthopedic conditions throughout your life. Understanding which foot type you are is vital for understanding and prevention of these conditions."

Dr. DeCaro said, “As a pediatric specialist, I am able to explain most chief complaints using biomechanics as early as one year old. Common pediatric issues such as gross motor milestone delay, growing pains, Severs’ disease, and shin splints have a specific origin based on one’s foot type. Bunions and hammertoes are no different! Controlling those biomechanics through orthotics can easily solve these issues kids (and then later as adults) suffer from.”

In the past few decades, healthcare has focused on money as a metric of practice success. Yes, we can only have a comfortable livelihood with money. Still, some areas of life are far more important than the girth of your investment portfolio or the size of your bank account. 
 
6. Appreciate what you have.
 
As a physician, we can interact with many others from all walks of life. There are people with more material wealth than I have and many more with much less. I have noticed how many people focus on the material objects they need to improve rather than appreciate what they have. Unfortunately, our culture emphasizes what we don't have and what others have instead of enjoying what we do have. Consider making a list of what you do have and reviewing that list regularly.

QUERIES (CLINICAL)
Query: Acute Peroneal Longus Tear/Surgical Case 
 
A 63 year old well-controlled DM type 2 active male was playing golf and felt a "pop" in his lateral right ankle after finishing his swing. There was no inversion sprain and the patient did not fall before or after the injury. The pain was initially +8/10. He was unable to bear weight. He went to an ED. X-rays were negative. A modified Jones dressing was applied; crutches  rest, elevation, and ice relieved symptoms to a 90% improvement. An MRI was ordered by the ED that was done 5 days after the injury revealed a completely torn peroneal longus tendon retracted to the lateral calcaneus. Old AFTL and CFL tears were noted as well. The brevis was normal.
 
Physical exam revealed intact sensation and palpable pedal pulses. There was edema laterally with minimal to no ecchymosis, +4/5 eversion without pain. The patient states tha he has almost no pain and no trouble walking with normal ADLs. He wants to know if the operative repair can be delayed until the fall. I discussed muscle atrophy of the PL with delaying operative repair. I'd like to know the opinion of my colleagues.  I'd appreciate any input.

Query: Source to Affordably Send eRXs
 
I am a podiatrist in Louisville, Kentucky. We are required to electronically prescribe for controlled substances. I do not write prescriptions for controlled substances that often and my EMR will charge a large fee for that service. My office has already filed a temporary exemption for this prescribing mandate but my exemption is about to expire. Do any PM News readers know of any affordable services which allow me to electronically prescribe controlled substances?

Never go with “buy just the active charts,” even if the definition of “active” is very generous. Inactive charts don’t have much value anyway. Once they own your patient base (and you don’t), where’s the advantage to them to take over the practice and employ you? They move the patient base into a location under their umbrella, get a recent graduate (or a PA), and you’ve still got rent to pay and have to start from scratch. They’re not interested in your practice to be nice; it’s because they see a potential profit center. Sell the whole practice as a unit, one thing, one price.
 
Remember, they don’t own YOU, just the business. And tell them “no thanks” to the non-compete clause.

Vicki- if you’ll refer to the multitude of emails sent out from the NCFAS, this is widely discussed. The new Palmetto ruling absolutely affects podiatry. If patients are not seen by an MD or DO within 6 months of their RFC appointment, it is no longer a covered service. Most offices I have spoken to are either rescheduling these patients or having them sign an ABN for non-covered services at the time of their RFC appointment. There are letters explaining the situation for patients and providers both on APMA's website, and that have been sent by NCFAS.

Many of the responses on PM News regarding the APMA survey on the Revised Definition of Podiatry are off the mark in my opinion. The APMA's opinion on this is their opinion. It does not carry any legal weight in any state or territory. The 'role as a podiatrist' is the scope of practice granted to you by the legal entity of the state or territory which awarded you your license to practice podiatric surgery and medicine. However aspirational or inspirational the APMA's definition may be, it does not change one iota the legal scope of practice that you currently have.
 
If you want a broader scope, fight for it in your state. If you don't, then enjoy what you have.

I read with great interest the recent comments concerning the revised definition of podiatry, specifically as to the removal of the "treatment of the local manifestation of systemic disease". Elimination of this phrase is absolutely paramount for the survival of podiatry. Increasing our scope by inching our way proximal from the tibial tuberosity is not the answer.
 
If we have increased our level of education to that of a true single track medical school which provides us an equal foundation to our allopathic and osteopathic colleagues during our 1st and 2nd years curriculum in the anatomical sciences and body systems course of study, and we then provide a carefully planned and more focused series of clerkships and classes during our 3rd and 4th years that emphasizes the diagnosis and treatment of systemic illnesses in addition to surgery and...

Dr. Pribut brings us some excellent points. Each member of the practice should have their own login to practice software. Well-designed software has an audit log which can be reviewed to see "who did what and when". This is important should there be issues or discrepancies in the information in a EHR system or practice management system. You should not allow users to 'share' an account. In addition, each user should have their own login for the operating system.
 
With each user having their own account, when a member of your practice leaves, you then re-activate their login information to all practice software systems as well as their access to the actual computer. This is an integral part of...
