MI Podiatrist Discusses Diabetes and Depression
 
According to Conway McLean, DPM, "The latest findings reveal a new condition associated with diabetes, a higher incidence of depression, one of the most common mental illnesses. Adults with diabetes are more likely to report depression than those without. Research seems to indicate it may be up to three times more common in the presence of type 1 diabetes, and twice as likely with type 2."

"Generally, depression seems to be the result of a complex interaction of many factors, including psychological, biological, and social. The latest data shows depression isn’t necessarily a problem with brain chemistry but may be due to an individual’s nerve cell connections. Recent research has better defined the role of dietary sugar and our long-term psychological health. Depression is apparently one of the consequences of diabetes and requires a wholly different approach to treatment. Will better control of your blood glucose levels result in a better state of mind? It’s too soon to say; the jury is still out," says Dr. McLean.

Beginning July 1, 2024, Central Carolina Foot & Ankle Associates will join the Duke Health Integrated Practice. Durham providers Melissa S. Hill, DPM, Gary Liao, DPM, and Alan Sotelo, DPM will continue to treat a full range of services including plantar fasciitis, ankle sprains, hammertoes, nail disorders, and acquired foot deformities at their North Duke Street office.

QUERIES (MEDICAL/LEGAL)
Query: Inappropriate Billing Practices
 
I am going through a billing company transition and have uncovered inappropriate billing practices. I do not believe they intentionally committed fraud but their mistakes are fraudulent. For example- they submitted the same claims two and sometimes even three times. Another example - they separated E&Ms from CPT codes for the same date of service. Luckily, they only did this 10 or so times. 
 
Now, I am having to submit recoupments to the insurance companies. I am concerned this will trigger an audit. I also paid the billing company their commission on payments received for these double billed claims. Some of these double billed claims were for skin substitution products, which reimbursed well. That increased my overall collection last year, which I paid taxes on. What recourse do I have?

1. Podiatric medicine mandates knowledge beyond the foot. Podiatrists who specialize in podiatric medicine act as gatekeepers similar to internists so long as physical evaluations are in conjunction with the provision of podiatric treatment. Both an internist and podiatrist can treat a foot condition, find the underlying cause of this foot condition and make a referral to the correct specialist to treat the underlying systemic cause.
 
2. The way medicine is practiced today is not in the best interest of patients. Podiatrists should have knowledge beyond their own...

Dr. Santi, when I quit accepting insurance 24 years ago, the reimbursement fees, which I thought were terrible and getting worse, are now about 75% lower than when I went to a direct-pay model. This does not support your argument that APMA has done so much to put money back in the pockets of podiatrists. In fact, over the past 24 years, private practice has become a dying model because of the untenable expenses imposed on doctors by the sheer fact that without seeing a high volume of patients every day, no one can survive in the insurance-dependency model. Larger offices, larger staffs, more supplies, etc. eat away at the profitability of a private practice. Stress and burnout have replaced professional satisfaction. 
 
So young podiatrists look for employment, where they are controlled by a private equity corporation and have productivity demands, lack of job security, nasty bosses, and no autonomy at all. I'm not sure that this is something to take pride in.
