Foot Pronation Vital to Body Mechanics: MI Podiatrist
 
According to Conway McLean, DPM, "The most important joint in the foot is the subtalar joint (the STJ), which is immediately below the ankle, and is composed of the talus bone and the heel bone. This is not a simple structure, being composed of three different joint surfaces. The motion of the STJ moves the foot in three different directions relative to the leg which, taken as whole, constitute pronation and supination."

4 Strategies to Deal with Difficult Patients
 
Here are four ways that physicians can deal with “difficult” patients.
 
2. Praise the positive, not the negative
 
Patients almost expect bad news from their doctor, like they need to lose weight, stay on their medications, stop drinking as much, or exercise more. But sometimes, it’s great to showcase what someone is doing well, especially if you have a non-compliant patient who doesn’t always follow the treatment protocol. If a patient’s blood pressure was improved or a weight loss goal was met, issue praise and recognition! One doctor used to print out labs that showed improvement and wrote notes like “well done!” and had his nurse mail it to the patient. A personal token of recognition goes a long way toward improved compliance.

How are fracture care codes used? If a patient comes in to the clinic with a fracture that you plan on eventually operating on, can you bill a closed fracture care code and then bill the surgical code when the open reduction and internal fixation is performed in the operating room?

Response: That would be a no. You are entitled to bill for an evaluation office visit code when first assessing the patient. Then, after you perform surgery to repair the fracture, bill the appropriate open reduction and internal fixation code for the specific bone and the level of fracture repair as described by each fracture code.

I completely support Dr. Saxena's suggestion to settle the ABFAS and ABPM conflict in a just, all-encompassing, and reasonable manner that promotes the well-being of all podiatrists and the public. His proposal, which involves ABPM certifying DPMs for podiatric medicine and ABFAS overseeing the CAQ for foot and ankle surgery, appears to be a sensible and impartial solution that can be executed expeditiously. I urge all parties involved to consider its merits. It is imperative to take prompt action in resolving the issues between ABFAS and ABPM.

Dr. Kornfeld is completely on target when he points out that insurance companies are eating us up alive. That combined with the increased cost of supplies and other expenses, many of us are entering hard times. Congratulations to Dr. Kornfeld and others who were able to completely free themselves from the chains of the insurance world early on in their careers. The problem as enunciated by other colleagues is that many of us have made our beds and giving up insurance company payment would be taking a very big risk.
 
There might however be another solution and perhaps some of us could weigh in on the following scenario: I went for cataract surgery. My insurance company was willing to pay for my procedure; however, the ophthalmologist let me know that if I want a better lens or to have him use laser-assisted surgery which would be better in my case, I would have to pay out-of-pocket. I paid $1,500 out-of-pocket. Had I opted for other bells and whistles, I would have been laying out thousands. 
 
What I am getting at is this: Are there things podiatrists can charge for without losing out on being part of insurance reimbursement? Let's call it a hybrid experience. Perhaps some of us can give examples.

FULL-TIME CLINICAL FACULTY POSITION – NYCPM 
 
NYCPM seeks DPM for its Department of Orthopedics and Pediatrics. Must be board certified by the ABPM, have a NY license and a current DEA Certificate. This position teaches biomechanics and pathomechanics to podiatric medical students. Duties include patient care and student education, and live lectures in classroom and patient-care settings, including workshops, webinars and seminars. Candidate will participate in scholarly service activities within the College community and work collaboratively with other faculty, students, staff and administration. Prior teaching experience strongly recommended. Salary range: $125,000 to $140,000.
