UK Podiatrist Climbs to Everest Base Camp
 
Sue Hubbard, who works as a podiatrist, climbed to the base camp with her sister Sylvia and brother-in-law Buck. The climb started as a project by Buck, who had previously worked as a rescuer in Nepal after the earthquake there.

The climb helped Hubbard raise more than £1,800 for two charities: Shelter and the Midlands Air Ambulance, with donations coming in from across the Malvern Hills and further afield. Speaking after returning from the climb, she said, "You do have lots of trepidation about whether you will make it up there, but with the blood-oxygen testing, you actually feel more confident the further you get."

Here are some actions for practices to curb physician and medical staff burnout.
 
3. Review and revise policies so employees are not deterred from seeking care for their physical health, mental health, and substance use challenges.
 
Practices should examine questions on applications and renewal forms for jobs and hospital credentialing, and normalize conversation about the use of mental health and substance use care for staff.

One of the most rewarding parts of my practice is the use of TENS as an adjunct to local anesthesia. I utilize this device for every injection that I administer, and I would estimate that 80% of the injections are painless. There is nothing more rewarding than to do a nail avulsion on a 6-year-old and have them completely unaware that they even had an injection, let alone a procedure. My technique requires more time, but it is worth such a rewarding outcome. I have quelled the fears of so many people who have had painful injections in the past and hesitated at the thought of another painful experience. 
 
Years ago, I taught the technique to residents, but I do not believe that any of them have incorporated it into their practice. Perhaps, they are directed to see a certain number of patients regardless of how the patient will view the encounter. In 18 years of practice, I have not had a single patient request an injection without the TENS unit. I also am a solo practitioner and have the ability to do the injection the way I want to. I highly recommend use of TENS as an adjunct to local anesthetic. It is helpful for minor things like slantback debridement of ingrown toenails and drainage of a localized abscess/blister. There is no billable code that I am aware of for TENS for anesthesia.

RE: Study Shows Relationship of Diabetic Foot Ulcers to Vitamin D Deficiency

A retrospective study was done on 339 hospitalized patients with type 2 diabetes, aged 60-90 years (mean age 67, nearly two-thirds were men) . They were seen between January 2020 and March 2020, including 204 with and 135 without diabetic foot ulcers. Overall, 80.5% had vitamin D deficiency (defined as < 50 nmol/L or < 20 ng/mL). Factors that independently linked with an increased diabetic foot ulcer risk were prolonged diabetes duration and elevated systolic blood pressure. 
 
While the relationship between diabetic foot ulcers and vitamin D levels has been controversial, the study showed that among the people with a diabetic foot ulcer, only 3% had vitamin D levels that were sufficient (> 75 nmol/L or > 30 ng/mL), 24% had levels defined as insufficient (50-75 nmol/L or 20-30 ng/mL), and 73% were deficient. The differences between these rates and those among the people without a diabetic foot ulcer were significant. Elderly people with diabetes were recommended to have routine vitamin D screening or receive vitamin D supplementation to prevent the onset or improve the prognosis of diabetic foot ulcers. I agree that Rafael Nadal's grand slam performance has been incredible. I understand that his foot condition has been incredibly painful for a long time, and that must create problems for him both on and off the tennis court. He, however, gets local anesthetic injections prior to playing actual matches, so he is not playing through any pain while anesthetized. Ethically, I have some concerns about playing while numb as that seems to be an invitation for additional damage. I wish him the best of luck but...I am a huge Novak Djokovic fan.

I just saw the ad for the upcoming conference called Internal Medicine Review for Podiatric Physicians being done by the American College of Podiatric Physicians and Surgeons (ACPPS). I am so impressed by this effort.  How they are doing this for a $25 registration fee I don’t know, but I would encourage any podiatrist who can get to Chicago on Sept 30-Oct 2 to attend this course. What a great idea, to bolster our colleagues already good knowledge of general medicine with this intensive 3-day course. We all complain about the lack of “parity” and our being so deserving of it. IMHO, THIS is how we work toward parity…by getting educated and earning it. Kudos to these people!

Dr. Block’s mention of the shortage of podiatry applications is profound! I retired from private podiatry practice 3 years ago. I retired US Air Force in 1994, following my 21 years as a commissioned podiatrist in 1974. I struggled in the Air Force for recognition by the medical field for podiatry recognition as a doctor and surgeon. Unfortunately, podiatry was never accepted as a “doctor” by the Air Force medical specialists! We were placed into the Allied Health department along with physical therapists, lab officers, optometrists, and the like. Podiatrists were looked at as pariahs of the medical corps. We were placed under the supervision of MDs who wrote very poor evaluations, and thus the dilemma of promotions beyond Captain or Major. 
 
I have been convinced that unless podiatrists become specialists under the general medical field, we (podiatry) will become extinct. I believe podiatrists must obtain their medical degree alongside a podiatry degree in order for us to advance! The podiatry schools must offer dual degrees, medical and podiatry.

Of course, I defer to the learned colleagues, leaders, and giants of the profession who have imparted the audience with their thoughts and opinions. I sincerely thank them for their work and insight. I have attempted to lead an effort to research and create continuing education experiences centered on state mandated courses that I felt needed a podiatric approach and data: HIV-AIDs, medical errors, medical ethics, human trafficking, and opioid prescribing and monitoring. 
 
In the past, I felt our podiatric profession was neglected by allopathic/osteopathic professions, and nursing as state podiatry boards offered and approved programs from the above professions. My interpretation to this action is summed up in the word: “PARITY”.
 
Recently, I applauded all the work performed at the congressional level for Veterans Affairs parity. We as a profession travel the yellow brick road on our way to Oz and encounter travelers with views that differ from ours yet add to our collective consciousness; thus we learn and become better professionals for the betterment of our patients.

When I read these posts, there is unending frustration about the profession they have chosen and the blame is usually cast on their degree. Everyone needs to consider that prospective students and the medical world have access to all this online content. When choosing a profession, why would one choose a profession when all the dialogue they research on the Web are statements by podiatrists who self-proclaim themselves as second class medical citizens?
 
One thing I heard since entering this profession over forty years ago is how “podiatrists are their own enemies and that they eat their young”. In this case, they are eating their embryos. This is the part of the problem. The insecurities of podiatrists continues to be...
