When I read these posts there is unending
frustration about the profession they have chosen
and the blame is usually cast on their degree.
Everyone needs to consider that prospective
students and the medical world have access to all
this online content. When choosing a profession,
why would one choose a profession when all the
dialogue they research on the web are statements
by podiatrists who self-proclaim themselves as
second class medical citizens?

One thing I heard since entering this profession
over forty years ago is how “podiatrists are their
own enemies and that they eat their young”. In
this case, they are eating their embryos. This is
the part of the problem. The insecurities of
podiatrists continues to be worn on their sleeves.
Age has certain benefits. You can say what’s on
our minds and not care what people say. I would say
cut the crap!

This is not the same profession I started in. Our
profession has gained significant parity with
medicine. We are in allopathic medical
universities as staff. We are partners in
orthopedic practices. We are an integral part of
wound care. The list can go on and on. Amazingly,
this has all occurred without a degree change.
Imagine that.

I am very proud to be a podiatrist and I am fine
with the degree. I have been blessed to have been
a college Dean, faculty member in the orthopedic
department in a major University, and partner in a
very well-known orthopedic sports medicine group
for 27 years. My degree did not limit me in
obtaining these positions and I would not claim to
be smarter than anyone else in our profession and
anyone could have done the same. I am very glad
that I choose podiatry and turned down my
acceptance to allopathic medical school.

I know that the shrinking of our applicant pool
has other origins. Our professional organizations
needs to stop chasing unicorns. Fix the things
that can be fixed. Fix the reimbursement
discrepancies. That should be their most important
goal at this time. Prospective students are less
likely to choose a profession that gets paid less for
doing the same service. Other issues - why was
consent granted to open more podiatry schools when
there is a dwindling applicant pool? Who we take
into our schools now and the resultant quality of
its graduates are what our profession will be
judged by in the future.

YOU CAN'T MAKE THESE THINGS UP

RE: Mrs. Malaprops Returns

A patient comes in with toenails that have turned green. She asks me "Is it gangrene?"  No, I said, "It's wintergreen."

Perfect for St. Patrick's Day?

I had correctly deduced that she soaks her feet in wintergreen alcohol (having seen this before). The oddest part is that she didn't make the connection herself.

RESPONSES/COMMENTS (SCOPE OF PRACTICE)

RE: Podiatrists Treating Systemic Conditions Affecting the Foot

From: Daniel Chaskin, DPM

In different states, podiatrists are licensed to carry out physical exams that involve ordering tests and other examinations alongside foot care treatments. The APMA and local podiatry groups should advocate for the licensing of podiatrists to manage all systemic conditions linked to any foot condition. The podiatric medical colleges offer a solid base of studies to support this argument. I recall that NYSPMA offered classes in anatomy, neuroanatomy, biochemistry, histology, pathology, internal medicine, and emergency medicine, etc.

If nurse practitioners have the authority to treat the whole body, podiatrists should also have the authority to treat any systemic condition that causes foot symptoms. If nurse practitioners do not remove a patient's shoes or socks during an examination and a podiatrist does, that podiatrist should be the one authorized to treat systemic conditions related to foot symptoms, since the feet are likely to be examined during every follow-up appointment. Patients are protected because a podiatrist’s code of conduct includes not independently treating conditions they lack current competence to treat.

What happens if more than one person uses the same computer?

In my office, many assistants were cross-trained and the computer had multiple users on different days.

RESPONSES/COMMENTS (BOARD CERTIFICATION)

RE: Confusion About Board Certification

From: H. David Gottlieb, DPM

I don't understand the mischaracterization of the concept of one podiatry board for the podiatric profession. If enacted, this would only mean that ALL podiatrists would have the chance to be board certified. Those of us who are certified by APBM or ABFAS would then have TWO, not one, certification.

One podiatry board should not eliminate the desire to specialize if one wants. Nor should it eliminate the desire to seek certification in that specialty if one wants. Podiatrists should be able to be board certified podiatrists.

PRACTICE MANAGEMENT TIP OF THE DAY

10 Things More Important Than Money

In the past few decades, healthcare has focused on money as a metric of practice success. Yes, we can only have a comfortable livelihood with money. Still, some areas of life are far more important than the girth of your investment portfolio or the size of your bank account.

3. Your health - As doctors, we are surrounded by stress. We have a sedentary occupation, don't have a regular exercise program, and are frequently sleep-deprived. Being caught up in our careers and working long hours may lead to neglecting our health. Too many physicians don't find time to get an annual examination by a colleague or fail to participate in screening tests like mammograms, PSA testing, and regular colonoscopies.

CA Podiatrist Provides Summer Foot Health Tips

According to Howie Que, DPM, "As we eagerly engage in the activities of summer, it’s crucial to prioritize the care of our feet. From leisurely beach strolls to adventurous mountain hikes, our feet and ankles bear the brunt of various seasonal elements and activities, which can occasionally lead to injuries. These foot and ankle injuries can vary from sprains, strains, and fractures to skin injuries that can have serious consequences. What’s more, people with complex medical issues may be more susceptible to severe conditions that can lead to limb- and life-threatening situations."

Dr. Howie Que

"Don’t go barefoot — To minimize the risk of sunburn, infections, and injuries, avoid walking barefoot. Wear shoes or flip-flops around the pool, beach, or hotel room to protect yourself from injuries or wounds that can lead to bacterial infections. People with diabetes should never go barefoot and should keep their feet protected to reduce the risk of cuts and scrapes, which can lead to complications," says Dr. Que.

INTERNATIONAL PODIATRISTS AND OUTSIDE INTERESTS

Former UK Podiatrist Hosts First Solo Art Exhibition

Felt artist Noelle Ferris opens her latest exhibition this week in The Grey Heron in Bandon. "I am a self-taught felt artist, a lover of wool and art, but my journey here has not been very straightforward. When I left school, I did a degree in podiatry and worked as a podiatrist in the UK and Ireland for 13 years."

Noelle Ferris

"When I was a podiatrist, I used felt (albeit semi-compressed felt) to deflect pressure from troublesome areas in patients’ feet. However, now I create felt paintings out of sheep’s wool to take a load off people’s minds," says Ferris.
