Flip-Flops Problematic for Feet: Louisiana Podiatrist

Summer staples can also be the season's most fashionable health hazards. Flip-flops, strappy sandals, wedges, and the pedicures to go with them make this a busy time of year for podiatrists. Doctors say patients suffer from heel pain or arch pain. One of the top culprits: warm weather wear for the feet - starting with the beloved flip-flop. That's because they don't have any arch support. Those problems can add up over time, even lead to surgery.

Dr. Tyson Green

"If people are going to be walking a whole bunch, whether it's on a trip or in the summer, walking around, flip-flops usually cause a lot of problems," says Dr. Tyson Green, a podiatrist.

INTERNATIONAL PODIATRISTS IN THE NEWS

Canadian Podiatrist Discusses Prevention of Athlete's Foot

My feet are killing me! Who hasn’t uttered that at the end of an extra long day? But it doesn’t have to be that way — many common foot problems are preventable and treatable, says Toronto-based podiatrist Dr. Robert Chelin. Athlete's foot is caused by fungus, and is one of the better known foot maladies. Look for scaling and flaking, especially on the soles and in between the toes; itching; and areas that are inflamed.

Dr. Robert Chelin

“To avoid it, don’t walk barefoot,” Chelin says, pointing out it’s often picked up in communal areas such as change rooms. “Dry your feet properly, always change your socks and give your shoes a chance to dry out if perspiration is heavy.” The fungus needs a warm moist environment to incubate, and shoes are the perfect spot.

CT Podiatrist Pleads Guilty to Healthcare Fraud

A Guilford podiatrist has pleaded guilty to healthcare fraud for submitting false claims to Medicare. Richard Sokoloff, 70, of Guilford, claimed that he performed a treatment for ingrown toenails, which he had not done, according to acting U.S. Attorney Deirdre M. Daly’s office. Daly’s release said Sokoloff engaged in the fraud between July 2008 and February 2012, claiming $200,000 in Medicare reimbursements.

More Healthcare Employers Finding Reasons Not to Hire Candidates on Social Media

While your social media profile can be a great asset in your job search, a new CareerBuilder study shows it can also end up costing you the job. More than two in five (43 percent) healthcare hiring managers who currently research candidates via social media said they have found information that has caused them not to hire a candidate, up 5 percentage points from last year.

The nationwide survey, which was conducted online by Harris Interactive© on behalf of CareerBuilder from February 11 to March 6, 20l3, and included more than 200 healthcare hiring managers and human resource professionals, found that more than three in ten health care employers (33 percent) use social networking sites to research job candidates, up from 28 percent last year.

QUERIES (NON-CLINICAL)

Query: Resource for Safety Shoe Program

A good friend works in administration of a large and highly specialized long-term care facility for people with disabilities in Philadelphia. They are a wheelchair community. The heavy motorized wheelchairs have become the source of many employee foot-related injuries. They are looking for a local resource who can help develop a foot safety program at their facility, perhaps someone from TUSPM. In addition, they are looking for recommendations for safety shoes that they could provide to their employees - slip resistant soles, protective toe box, integral orthotic foot bed. Any help is appreciated.

RE: MIS For Bunionectomies? (Randall Brower, DPM)
From: Stefan Lorincz, DPM

Until recently, I was also quite skeptical of the use of MIS in foot and ankle surgery. I had not been trained in it, nor did I hear positive input from those that mentioned it. So, I did what any curious person would and attended the recent MIS seminar in New Orleans, LA.

After listening to the lectures and spending a full day in the lab, I can say that I am putting what I learned to use already. As with any surgical technique, not every patient is a candidate. There are...

RESPONSES/COMMENTS (EMR) - PART 1A

RE: TRAKnet Charges (Marge Portela, DPM)
From: Trevor Neal, DPM

I too have TRAKnet and can tell you from past experience that their fees are VERY reasonable. TRAKnet is our third EMR and is by far superior to the first two. Their new support staff is very knowledgeable, and the response time has been good, even with the transition to a new company. It wasn't unusual to have to wait 7-10 days for a response with the other EMRs but it hasn't been the case with TRAKnet. Our practice has five physicians and we have  been using TRAKnet since its inception. With its total focus on podiatry only, it blows the others away.

As far as the credit card issue, that's becoming an industry standard. I was at a hotel last weekend and wanted to purchase a bottle of water from the vending machine, and it ONLY took credit cards, not cash! I have the same credit card arrangement with two other maintenance contracts.

RESPONSES/COMMENTS (EMR) - PART 1B

RE: TRAKnet Charges (Marge Portela, DPM)
From: Elliot Udell, DPM

The problems that people are having with TRAKnet were well predicted as soon as meaningful use caused hundreds of new EHR programs to come out of the woodwork. Many will go out of business leaving doctors with expensive leases that will still have to be paid for. That is why 150,000+ doctors went with Practicefusion.com. It is free. If they go out of business, you lost nothing.

The owners of TRAKnet are very honorable people and took action to save the company from total collapse. Unfortunately, they have to charge what seems like megadollars in order to finance their investment. The alternative was not rosy.

People considering obtaining a new EHR program need to take the problems TRAKnet customers are having into consideration, along with the specter that many other small companies will probably fold. The government wants us to have EHR, but will not guarantee the solvency of any of the companies that sell these products. This is unfortunate.

RESIDENCY SHORTAGE CRISIS - PART 1

RE: Unmatched Residency Placements Currently Stand at 92 (Michael Rosenblatt, DPM)
From: Jeffrey Kass, DPM

I have been following the "residency crisis" discussion and have periodically asked what, if anything, is being done. Dr. Rosenblatt called for the firing of individuals on the CPME board. Dr. Gottlieb writes in informing us it is not the job of CPME to create positions; rather, it is their job to regulate.

Assuming, this is true, there are still approximately 92 graduates without programs. So, is there any "governing body" within the profession that is supposed to "guard" residency positions? Does one need to be created?

RESIDENCY SHORTAGE CRISIS - PART 2

RE: Unmatched Residency Placements Currently Stand at 92
From: Eric J. Roberts, DPM

I may be missing something about this debate. We are asking for parity with MDs, but are all MDs guaranteed a residency placement out of internship or medical school? As far as I know, they are in the same situation or even worse. Correct me if I’m wrong in stating that not even close to all MD candidates get a residency. Many times, poor matching strategies, rather than poor candidates, are what lead many to not matching. But for medical school candidates, there is nowhere near the number of residencies available for all applicants.

I encourage everyone to read the NRMP (National Resident Matching Program) data on MD matching from 2009-2013. Here is an excerpt which demonstrates that in 2013, 25.9% of seniors of MD, DO students did not match. 74% matched. How many...
