Common Pitfalls in a Doctor's First Year in Practice
Congratulations for making it through medical school and residency, and even perhaps a fellowship, and now you are ready for your first or second job out of school. Here are some areas that trip up new doctors the most.
2A. Pay attention to the fraud and abuse statutes.
You go into practice so well trained about clinical medicine, but often have no clue what business relationships can get you into trouble or what to avoid. Remember this: If it’s too good to be true, IT IS. If someone is giving you money for not much work, if something smells funny, if there is an underlying motive behind it, be cautious. As a physician, you are an especially attractive target for kickback schemes because you can be a source of referrals for fellow physicians or other healthcare providers and suppliers. Patients trust you. You can at times help your patients decide what drugs they use, which specialists they see, and what services and supplies they get. If you tell them to use a certain lab or certain drug, they do. Companies want your patients and their business and will incentivize you however they can to get business thrown their way.

Query: Pediatric Flatfoot
I see kids but certainly don’t specialize in it. A 7 year old came in with a progressively worsening flat foot. He has a pronounced genu valgum gait and his medial arch, which is flexible, drops all the way to the ground. I suspect an internal hip or knee issue may be contributory. I’m sending him to a pediatric orthopedist for an evaluation further up the kinetic chain. His Achilles does not appear tight as he has plenty of dorsiflexion.

I’ve attached an x-ray of his foot and ankle. There is no evidence of medial malleolus ossification and his talus looks extremely malpositioned. His parents and I had a general discussion on shoes and inserts to prevent worsening of the deformity. I was hoping to get thoughts and suggestions from my colleagues who are more experienced with pediatrics.

Software tools such as ChatGPT are 'not ready for prime time' when it comes to healthcare due to the reasons that are outlined in the article. I am optimistic about the future of IA as a tool for the future of medicine. Note: I use the term IA - Intelligence Augmented as opposed to artificial intelligence. These are tools to assist us in clinical decision-making, not tools to make decisions for us.
