PODIATRISTS IN THE NEWS

Podiatrists Discuss Plantar Fasciitis and Pregnancy

If you’re pregnant, you probably can’t wait to get home and put your feet up. If your feet hurt, you might have plantar fasciitis, a condition that affects about 10 percent of the population - and probably occurs more frequently in pregnant women, according to Phil Vasyli, podiatrist and founder of Orthaheel, and Dr. Andrew Weil of Integrative Footwear. "The result is a sharp pain in the heel, especially after you’ve been sitting for a while, or when you wake up in the morning,” said Dr. Alan Berman, a podiatrist at Somers Orthopaedic Surgery and Sports Medicine Group in Carmel, New York.

You might think a shoe insert for your heel would help, but Berman said, “If you don’t support the arch, it’s not as efficient.” Any drugstore shoe insert will do, according to Berman. Also, nix the flimsy flip-flop and flat sandal and look for a shoe that has good arch support. If the pain doesn’t get better, consider seeing a podiatrist or an orthopedist. Plantar fasciitis can linger for weeks, months and even years after pregnancy. It all depends on your body, Berman said.

PODIATRISTS AND DIABETES

TN Podiatrist Discusses Diabetes and Podiatry

Dr. Ira Kraus has seen first-hand how diabetes can affect people. “My father’s father passed away at the age of 33 due to complications of kidney malfunction secondary to diabetes,” he said. “My father developed late-onset diabetes. His sister and mother also had diabetes, so it is very prevalent on my father’s side of the family. Both my brother and I feel blessed we have not developed diabetes at this point in our lives, but we’re definitely affected by it.”

Dr. Ira Kraus

When he started practicing podiatry, there was not a lot of focus on diabetic foot care. “I felt that diabetes wasn’t really being treated as well as it should be,” he said. “We started seeing that there was more of a need for diabetic foot care. I felt we would be able to help the patient by offering more specialized services. I have found that practicing diabetic foot care is very rewarding. The patients are very thankful because many of them suffer from different types of ailments and pains, and we’re able to help make them feel better.”

OBITUARIES

Dr. Wallace Gordon Yadon, age 90 of Delavan, WI, passed away peacefully Friday evening June 21, 2013. Yadon was a man of many hats: He was a retired postmaster and podiatrist, veteran of two wars, local historian, newspaper columnist, active member of the local Legion Post, volunteer director of Delavan's arboretum, and great-grandfather.

QUERIES (NON-CLINCAL)

Query: MLS Laser

Is the MLS laser as effective as the sales team claims? Are patients willing to pay for it? Do the results justify the cost? Is it as profitable as they say?

RESPONSES / COMMENTS - (CLINICAL)

RE: Tibial Torsion and Femoral Anteversion
From: Stanley Beekman, DPM

I would like to add a few things that I have picked up during my years in practice. I find that “tibial torsion” is a positional deformity at the knee. Not only can the tibia flex relative to the femur, but it can also rotate relative to the femur. The internal rotation of the tibia is accompanied by an inversion (going into varus). Denis-Browne splints work well up to the age of three. I still bend them as was taught by Herman Tax, DPM to prevent pronation. At age three, I find that the children can...

RE: Will Podiatric Residencies Accept MDs? (Ivar Roth, DPM, MPH)
From: David Gottlieb, DPM

While it’s a nice thought for the times when there are more programs than applicants, MDs cannot be podiatry residents. One of the required prerequisites to be accepted into a CPME-approved residency program is that the applicant has graduated from a CPME-approved podiatry school/college.

A better option would be for your son to graduate medical school, then go to podiatry school for 2 years. At that time, he would be an MD/DPM and could decide which residency track and specialty he wanted [MD or DPM].

RE: Will Podiatric Residencies Accept MDs? (Ivar Roth, DPM, MPH)
From: Allen Guehl, DPM

This is an excerpt from the Ohio medical board licensure requirements. It does not appear that in Ohio, a podiatry residency would lead to MD licensure.

Graduate medical education means: Internships and residencies accredited by the Accreditation Council for Graduate Medical Education (ACGME) or the American Osteopathic Association (AOA); a clinical fellowship conducted at an institution that has an ACGME or AOA accredited residency program in the same or a related field; Canadian internships accredited by the National Joint Committee for Approval of Pre-Registration Physicians Training Program; and Canadian residencies accredited by the Royal College of Physicians and Surgeons of Canada or the College of Family Physicians.

RE: The Bell Tolls for Us
From:  Mark S. Davids, DPM

Today, I received a letter from my alarm monitoring company and our pest control company telling me that, due to rising business costs (incurred with supply costs, employee healthcare, regulatory compliance, employee training, etc.), they will be implementing price increases. Of course, they want me to continue to use them to service my office; and I have no reason to change because everyone is likely handling the financial matters the same way….except for us.

Faced with the escalating costs of maintaining a practice and...

RE: Scholl Dean Praises School's Co-Founder (Hartley Miltchin, DPM)
From: Robert Scott Steinberg, DPM

Thank you Dr. Miltchin, for saying, what I believe is how many of us Illinois College of Podiatric Medicine graduates feel. The school is dangerously close to becoming what it was before the strike. I have a newspaper photo from back then. It was one of our strike signs, that said, Give the Dean the Foot.

RESPONSES / COMMENTS (RESIDENCY SHORTAGE CRISIS)

RE: Unmatched Residency Placements Currently Stand at 92 (Steven J. Kaniadakis, DPM)
From: Michael M. Rosenblatt, DPM

Dr. Kaniadakis is entirely correct in his suggestion that there should be different "goals and practice styles for podiatrists." But at the present time, CPME excludes these possibilities. In order for this model to occur, a DPM graduate should be allowed and accepted as a "Training Associate," which includes hospital and various externship rotations." This is not unheard of in the MD/DO community, in the form of official fellowships.

CPME would have to accept these (well designed and ordered programs) for the ability to obtain a license, which would make them equal to an actual residency. Another factor completely missed by CPME/APMA is that if 1/5th of graduate podiatrists never become licensed, they will never pay back their college loans. This threatens the continued existence of various Federal loan programs to the profession.

This is one of the main reasons why I am calling for the resignation of the two main administrators of CPME. They have the ability to destroy the profession. Their power to do this must be stopped.
