MA Podiatrist Discusses Flexible Flatfoot in Children
 
“Prevention is better than correction,” said podopediatrician Dr. Louis DeCaro, the incoming president of the American College of Foot and Ankle Pediatrics, the APMA group which specializes in children’s foot health. “My main mission is to make sure that my colleagues and the public know that much of the correction that they are doing in their adult population is preventable.” The first step of prevention is to start evaluating the condition of a child’s foot as early as age four.

According to DeCaro, the most common condition that afflicts children is flexible flatfoot, a disorder caused by a foot that does not become rigid with age coupled with an increased eversion of the arch. Most children younger than four typically have a flat foot, but develop an arch later in childhood. Some key indicators a child zero to four years of age may develop flexible flatfoot are poor coordination, balance, posture, and strength.

Query: Livedoid Vasculopathy?  
 
This patient is a 29 year old, healthy appearing, white male. He first presented to me approximately a year ago with a "sore" on the top of his left foot, which he stated developed a few weeks prior, after he'd walked several miles home from work while wearing his work boots. He'd been applying antibiotic ointment to it and covering it with a Band-aid but it was not getting better.

In addition to this, he complained of "skin pain" and gradual discoloration of the skin over the past several years. He related this to Raynauds that he has "had forever." Patient's medical history was significant for...

Query: Legality of Physician-Owned Pharmacies 
 
Over the past two years, I have been approached by a dozen or more compounding pharmacies to write prescriptions. Recently, I have been contacted by two organizations setting up physician-owned pharmacies (40% physician owned) with the promise of big dividends! I would appreciate anyone's thoughts on whether this is legal or not?

The concern is that a patient used econazole nitrate cream and developed an allergic reaction. The patient could truly have an allergic reaction to econazole nitrate and/or the vehicle cream. She should remember the manufacturer of the product as she may be prescribed other medications that will be provided as a generic in that same vehicle cream. The patient should consult with an allergist.

Being an allergy sufferer as well as having had patients who developed contact dermatitis from things that we never would imagine would cause it, I take allergies seriously. The case presentation does sound allergic, however it was not made clear whether the patient had a reaction at the location where she applied the econazole or if the symptoms were only in other parts of her body. Contact dermatitis can spread all over (think poison ivy), however the site of application is always affected first. If this was not the case, look at possible other allergies or even other conditions which might be causing  the symptoms.

RE: Cancers Mimicking Fungal Infections

I think that the article "Cancers Mimicking Fungal Infections" is an incredibly educational treatise on a topic we encounter regularly (fungal infections) - which might actually be cancerous, cutaneous manifestations. This article made being a member of the APCWA worth the cost, and there is much more to the organization.
  
Abstract: Primary and metastatic malignancies may occasionally mimic or co-exist with cutaneous fungal infections. The authors report three cases of cancers that were initially presumed to be cutaneous fungal infections. Dermatologists should maintain a low threshold for skin biopsy in patients with persistent or refractory fungal infections.

RE: PADnet Now Charges to Transmit Reports

I own a PADnet machine from Biomedix. This is the non-invasive machine that takes segmental pressures with PVRs and ABIs, and then allows the podiatrist to transmit the test to a "reader" of their choice (usually vascular doctor or interventional radiologist). The equipment was expensive ($20,000 range). Biomedix apparently was sold. All of a sudden, my PADnet no longer will transmit the report to my "reader." My error message reads, "The subscription expiration date is 5/31/14 contact nnothrup@biomedix.com 651-233-5957.
 
I called and got a loud ringing noise. I emailed, and the email was bounced back. I called a colleague who informed me, "oh you didn't hear, you are now going to be charged to transmit the test." He was quoting me price ranges of $600-1200 per year depending on if I want hardware/software support as well. I am not a lawyer, hence I do not know the legality of this. From my perspective, this is "highway robbery" and NOT appreciated. I implore my colleagues if you are as outraged as I am, DO NOT PAY. Print a copy of your reports and fax them to the reader of your choice, and let the company know that what they are doing is not appreciated. (assuming you figure out how to contact them).
