NY Podiatrist Recommends Barefoot Therapy at Home

No matter who walks (or limps) into her office at Midtown Podiatry with a foot injury, Dr. Emily Splichal offers the same prescription: barefoot therapy. “When I send patients to physical therapy, on [the prescription] I’m saying, ‘Must be barefoot.’ It’s the foundation of every patient’s rehab,” Splichal said.

For patients who don’t require physical therapy, Splichal has them brush their teeth while barefoot and balancing on one leg. “When people think about barefoot, they think about running,” she said. “But I don’t want them to — I want them to think about training, rehab, performance.” Splichal recommends strengthening the foot and ankle via a series of at-home exercises — done barefoot, of course.

From boyhood until his death at age 85, Scholl (1882-1968) was committed to curing corns, bunions and falling arches. That passion for feet paid off handsomely because he was not only zealous, but also a brilliant innovator, salesman, and marketer. The result was his co-founding, in 1912, of the Dr. William M. Scholl College of Podiatric Medicine in Chicago.

Runners Need to be Mindful of Early Signs of Injury: NY Podiatrist

Dr. Robert Russo, President of the New York State Podiatric Medical Association, suggests that even the most experienced and well prepared runners and walkers need to be mindful of signals and symptoms that indicate that foot pain requires medical attention.

“A visit to the podiatrist should happen at the first sign of pain or discomfort,” said Dr. Russo. “Many people will tolerate pain thinking it is normal and will go away, only to find they have a fracture or other serious injury that requires medical treatment or even surgery,” he added.

OH Podiatrist Rides for Medical Supply Charity

Logging hundreds of miles on winding country roads in Ohio is the way more than a dozen Rotarians are raising money to ship medical supplies to Third World countries. By the time members of the Elyria Sunrise Rotary, Elyria Noon Rotary and others return to the town’s center sometime Thursday afternoon, they should have ridden more than 350 miles. “We’re in Willard now,” said Dr. George Costaras, president of the Elyria Sunrise Rotary, speaking by phone Tuesday afternoon when the group stopped for lunch. “We were in Bucyrus Monday night and Tuesday morning. I think today will be a 70-mile day.”

“We are constantly collecting the equipment all year round,” said Costaras, a podiatrist with the Elyria Foot Clinic. “It’s things as simple as wheelchairs, hospital beds to pacemakers and x-ray machines. It’s a lot of things we take for granted.”

AMA House of Delegates Rejects ICD-10 Report

An American Medical Association board of trustees report that recommended against a strategy of bypassing the ICD-10 set of diagnostic and procedural codes and skipping straight to ICD-11 was rejected by the AMA House of Delegates on Wednesday morning.

The report was originally discussed Sunday before one of eight reference committees hearing debates over proposed reports and resolutions. The reference committee, chaired by Dr. Marta Van Beek, a dermatologist from Iowa City, Iowa, recommended that the report be accepted. It noted this report “achieves the goal of discussing the feasibility of moving directly from ICD-9 to ICD-11,” which is what it was intended to do.

Query: Tibial Torsion and Femoral Anteversion

My patient is a 2 year old white male. He is constantly tripping when he runs in shoes or sneakers.  Many of the older treatments, e.g. serial casting and night splints, are no longer considered acceptable.  What is the current treatment protocol? Would a gait plate/orthotic aid in reducing tripping and falling?

Dr. Michaels' unfortunate experience demonstrates the specific reason why I would never participate in a meaningful use attestation. No amount of government incentive or stimulus money is worth that degree of scrutiny and harassment by an agent of the CMS. Their solitary mission is finding abuse and recovering money which the government doesn't have, and shouldn't have spent in the first place.

Maybe that's why my certification has been renamed twice and my lifetime status has been redefined.

We had a chain called Good Feet here in Northern California using a similar approach. Patients would bring their $300 "custom orthotics" to me and ask why they didn't work. The answer would be immediately obvious--poor fit, minimal heel cup, no skive where needed, etc.; they were a joke. This business has left the area after 7 or 8 years but they raised awareness of orthotics, I'll give them that much--celebrity endorsers in their TV ads helped. They didn't give refunds for orthotics that didn't work, but you could apply that amount to shoes they sold.

The Dr. Scholl's kiosks are popular here and they allow refunds and cost much less. The bottom line is that the Internet and advertising, and big business will continue to provide all manner of choices for heel pain sufferers.  Some people with heel pain will get better if they stick a piece of cardboard in their shoes--if they don't, they will eventually find a podiatrist. Those are the patients you want in your office. They will truly appreciate you for your skill in fitting them to the right orthotic and they will tell others.

RE: ASPS Holds Inaugural Surgical Conference

I'd like to congratulate ASPS for what appears to have been a successful conference. What caught my attention was their dedication to education, evidenced by the fact that "All conference attendees received a flash-drive with the conference presentations and materials on them at no additional cost." It reminded me of the day when NYSPMA recorded the NY Clinical Conference and provided those lectures on CD for any member who wanted to buy them. To this day, I still watch some of the lectures, even though this was many years ago.

We, as a profession, neee d to bserious about education. I suggest that the APMA, as well as any state or local organization that sponsors educational clinical conferences, adopt the model set forth by ASPS so that members don't have to sit in the lecture hall and try to take notes feverishly. How insulting! I understand there is a cost for this service, and I am not adverse to paying it.

Unfortunately, I also understand that there was little interest for this material by a majority of conference attendees. This also speaks volumes as to the seriousness in which podiatrists want to be educated and would rather mingle on the floor and meet with their friends for lunch, or get on line to be scanned 1-2 hours before the scheduled time. And if lecturers don't want their lectures to be available to their colleagues, then I suggest we get others to lecture. Others, like Dr. Jacobs, willingly provides this material to those who ask. Some of the younger practitioners would love the honorarium. We all talk about parity, or dual degrees or being taken more seriously. It all starts with education.

RE: Scholl Professor Sues Associate Dean for Defamation

As a Scholl graduate, I found both litigants (Drs. Bareither and Hrywnak) to be valuable in my education. It was also easily apparent to me, even as a relatively naïve student in 1992, that Scholl College of Podiatric Medicine had a very political framework to its administration and faculty-hiring that appeared to have a negative effect on its students. While there were wonderful, highly qualified professors (including Dr. Bareither and Dr. Hrywnak), many other professors were hired who were sub-optimal… in my time there I can cite biochemistry and pharmacology…. 2 important topics, and I have no personal grievances here….

Both of those professors were “let go” after a year, which supports my position that each of those professors should never had been hired to begin with. Each of those hires could have been chalked up to a simple mistake, but during my same time of schooling, several valuable part-time excellent podiatrists who had long-standing positions to teach in the school-led clinics were let go. Those part-time professors were exceptional, and were let go for reasons unbeknownst to students. My overall impression was that the internal politics at Scholl was all about maintaining the political careers of the powers-that-be, and firing anyone who was not part of the political machine. It seems that, even with alignment with Rosalind Franklin University and 15 years later, nothing has changed.
