Replace Running Shoes at Least Every 6 Months: AL Podiatrist

Area podiatrists warn that ill-fitting or worn-out shoes could cause soreness or injury to feet. With winter a seeming distant memory and summer officially launching Friday, people are trying to get firm, stay fit, lose weight, stop smoking, and eat less, said Dr. Jeffrey Schneider, a podiatrist in Florence. “These programs often involve jogging, walking, hiking, playing ball sports,” Schneider said. “In doing so, each of these activities should be accompanied by appropriate footwear.

Dr. Jeffrey Schneider

“The appropriate footwear protects your feet from becoming injured or can help from forming different foot deformities.” “I find, personally speaking, that one needs to change out running shoes probably every six months,” Schneider said. “By the time six months comes around, the shoe is worn and your foot has returned to the old, harmful biomechanics that create the problems.”

Human Foot Not Intended for High Heels: MD Podiatrist

Dr. Michael Liebow, a podiatrist in Bethesda, pulls out a wince-inducing photograph of a foot x-rayed in a high-heel shoe: It reveals the ball of the foot at a nearly 90 degree angle to the bones in the rest of the foot. It does not look good. The x-ray is a prop that Liebow says he shows to patients who “walk into the office in six-inch heels and say, ‘My feet are killing me! Why?’ ” He says he tells them, “That is not how your foot has evolved to walk.”

Dr. Michael Liebow

To sum up his brief and frequently futile plea for foot health: Humans are meant to walk heel-to-toe, with the leg at about a 90-degree angle to the foot and the ankle joint employing a 60-degree range of motion during normal daily activities. By wearing a high heel, Liebow explains, “you’re altering the position of the foot and how the foot is to function. Therefore, lots of bad things happen.”

PODIATRISTS AND THE LAW

Convicted CT Podiatrist Threatens 93 Year Old Witness

Hours after his conviction Friday on 28 healthcare fraud charges, a Brookfield podiatrist found himself back in trouble after he appeared at the home of a 93-year-old witness against him, police said. During the incident, Dr. Samir Zaky, 38, of High Ridge Road, Brookfield, asked the elderly man why he testified against him, police said, which led federal agents to take Zaky into custody Saturday on charges of violating conditions of his bond. The witness, who had open-heart surgery and carries an oxygen tank, is the grandfather of a federal law enforcement officer.

PRACTICE MANAGEMENT TIP OF THE DAY

Best Handshake

A solid, firm handshake is still one of your best bets for making a good first impression. Put your best hand forward by making eye contact, waiting until the other person extends his or her hand and then grasping firmly. Two to three shakes is sufficient. Important: Some men think it’s rude to shake a woman’s hand, but it is business-appropriate. Shake everyone’s hand the same way, regardless of the person’s gender.

RE: Persistent Interstitial Tearing of the Peroneus Longus Tendon
From: Dennis Shavelson, DPM

This case has a biomechanical component to its existence and eventual care. What is this patient’s foot type (biomechanical foot print)?  As the peroneus longus is a pronator of both the rearfoot and the forefoot, where is it being stressed enough to prevent healing in the BK walker? Only after making these determinations can we select a custom plan of care.

Until we have this information, in general, valgus forefoot wedges or 5th ray cut-outs may help when applied to a footbed. Proper vaulting is essential (STJ neutral may work, but not as well). Consider a cuboid pad and a strapping/Unna boot as well.

RE: HITECH EHR Meaningful Use Audit
From: Daniel D. Michaels, DPM

Dear Colleagues,

Our office would like to share the experience we encountered during an audit from Medicare for HITECH EHR Meaningful Use. Bottom line - use exact dates on the letter from your EMR company proving you have met meaningful use.

On January 11, 2013, we submitted our Attestation report for 2012. It was our understanding that once the submitted information was complete, there would be a 30-60 day window for the stimulus monies to be disbursed. At the end of March, there were still no monies received; therefore, we contacted CMS and...

RE: Ideal Feet
From: Mark Robson, DPM

I am concerned about a company named Ideal Feet. They are the first commercial business that is openly anti-podiatry. Because their shtick is to sell cheap OTC orthotics as expensive custom devices, we are competition. They have ads telling people to come in for treatment of heel pain. Basically, they are diagnosing and treating medical conditions of the feet without a medical license. Ads and salespeople tell patients to avoid the podiatrist who can only offer injections, surgery, or expensive orthotics that don't work. Each store has a barrel of "ineffective" old custom orthotics clients have supposedly turned in. Money wasted on the podiatrist, chiropractor, or pedorthist is the message.

People spend over $400 for a basic rubber catalogue-type device. Allen Selner's Walk Fit devices have a better design for less than $20. I'm not sure what would be appropriate to do, but this company needs to be watched as an anti-podiatry franchise.

RE: HI Podiatrist Invents "Energy Positive" Military Boots
From: Steven King, DPM

I would like to thank PM News for posting this story. Perhaps it will start a positive and constructive conversation.

I know the term "energy positive" may sound a bit far-fetched. I would plead with most podiatrists to re-read chapter 7 on Work and Energy in Dr. Nigg's book Biomechanics of Sports Shoes. If carbon fiber prosthetics have over 90 percent energy return, just think what we can do with orthotics.

RE: Scholl Professor Sues Associate Dean for Defamation (Lloyd S. Smith, DPM)
From:  Jon Purdy, DPM

I have nothing but respect for Dr. Smith and his vast contributions to podiatry. The last thing I want to be known for is drive-by blaming. I don’t believe I blamed anyone or any entity, but simply stated the schools could do more to prepare students for the real world of medicine.

Years ago, I interacted with the CPME to help form new residency positions and did go through the system, so I’m certainly not an “outsider.” Maybe Dr. Smith is not aware that...

RESPONSES / COMMENTS (NEWS STORIES)- PART 2B

RE: Scholl Professor Sues Associate Dean for Defamation
From: Brian Kiel, DPM

I feel I must respond regarding the recent lawsuit filed by a professor at the Scholl College. It seems that in our profession, nothing has changed in the past 35 years. In 1970, we, the students at ICPM (Scholl) went on strike to protest the inadequate education and the dictatorial behavior of the administration at that time. The dean, Donald Anderson and his minions used multiple methods of punishment to maintain their control over the school. They defamed students, they expelled students, and threatened anyone who challenged their opinions.

Led by Martin Stoller and several professors, including Lowell Weil and Steve Smith, we left the school, formed our own classes, and demanded...

RE: Scholl Professor Sues Associate Dean for Defamation
From: Hartley Miltchin, DPM

Healthcare will never be the same in the future due to fiscal restraints. The changes will affect every practitioner in every specialty, but podiatry even more so. The initiatives that Scholl College has made make no sense and only prove that they want censorship and not relevant practice information for the students. Assets of a great school lie in the faculty, not the administration.

The administration is responsible to maintain a top notch faculty. In releasing part-time faculty while they are in the middle of a course only...
