Diabetes an Issue for American Indian Population: MT Podiatrist

Diabetes, in particular type 2 diabetes, can be a stealthy and aggressive grim reaper of American Indians. In an attempt to keep on top of a folks with type 2 diabetes, the Tribal Health and Human Services Department’s Diabetes Prevention program contracted the services of G. Gregg Neibauer, DPM in 2009. The Missoula-based podiatrist sees patients twice a month.

“What I am here for is to treat patients with diabetes that have problems with their feet,” Dr. Neibauer said, adding that diabetes can negatively affect nerves causing decreased blood flow, especially to the areas far from the heart like feet and lower legs. “Sores in the lower extremities don’t heal well because of decreased blood flow. That can eventually lead to amputations."

CA Podiatrist Chosen as Resident of the Year

Chief podiatric resident, Visoth Chan, DPM was chosen as "Resident of the Year" at White Memorial Medical Center, Los Angeles, California. This GME award is given to only one graduating resident from the hospital's residency training programs (internal medicine, pediatrics, OB/GYN, family medicine, or podiatric medicine and surgery) who receives the most votes from all the teaching faculty at the Loma Linda University affiliate hospital.

CT Podiatrist Facing Years In Prison For Fraud

Brookfield podiatrist Dr. Samir Zaky was found guilty Friday on 14 counts of healthcare fraud and 14 counts of making false statements relating to healthcare matters for illegally submitting Medicare claims for procedures he did not perform. According to the evidence presented at trial, from August 2010 to July 2011, Zaky submitted numerous Medicare claims for nail avulsions — the surgical removal of the border of the toenail, a procedure that requires injectable anesthetic. Zaky faces a maximum of 10 years in prison on each count of fraud and five years on each count of making false statements

AAPPM Holds Inaugural West Coast Workshop

The American Academy of Podiatric Practice Management (AAPPM) made its inaugural visit to the West Coast Last weekend. Podiatrists and staff from Alaska to Arizona and from throughout the northwest flocked to Seattle for the first AAPPM workshop of its kind on the West Coast. In all, over 50 podiatric physicians and staff attended the event to learn more about practice management general principles, billing and coding concepts, and marketing plans for practices.

Hal Ornstein, DPM, AAPPM Chairman, stated, “This was a resounding success.  I heard so many positive comments and so many attendees were grateful that AAPPM would come across the country to provide instruction.”  Ornstein also noted that the AAPPM speakers enjoyed the new locale and new faces as well. AAPPM President Jeffrey Frederick, DPM added, “the AAAPM looks forward to its next West Coast adventure.  The AAPPM will host its California Dreaming Workshop this December in Santa Monica, California.”

The manufacturer's specifications are for one-time use. There are companies that will recertify hardware, like getting a used program car, and certify that it meets the specifications as well as repackage the hardware, sterile if required. This is why once a screw is put in and then one decides a different size is necessary, there is a charge for the extra hardware.

What is not seen are microscopic changes occur to the screw. The threads could be damaged "slightly (minimally)", perhaps not observable to me, but...

RE: Fissuring Under 4th and 5th Digits

From the brief history given, the congenital issue is likely a coincidence for my initial thought is infectious etiology, not mechanical/strutural. Dermatophytes and Candida are notorious for causing interdigital fissuring, especially between 4th toe webs, toe creases, especially in moist environments, i.e. the patient's swimming history. Included in your patient's history is a 2-3 time/year recurrence, likely representing next generation spore germination and re-start of the infectious process. If it were a mechanical etiology, it would likely be constant all year round.

Consider empirically starting your patient on OTC bid topical fungicides, then switching to a broad spectrum Rx if no response in 3 weeks. Ensure that Rx covers Candida. If still no response, take a shave biopsy and submit for analysis as erythrasma and white psoriasis can also present this way. Pending biopsy result, if infectious, yet unresponsive to appropriate topical agents, you may also need to consider an oral agent, particularly for recurrent/resistant cases. If you want to initially avoid the minor surgery, a Wood's light may shed light on the etiology (no pun intended). Once infectious etiology is ruled in, then also follow through with patient education on the benefit of consistent shoe sanitizing, as well as the various hygiene adjustments your patient can undertake that protects toe creases from additional exposure.

A similar thing used to happen to my daughter after swimming in concrete swimming pools. The base of her toes would crack open and peel. We finally figured out that it was the concrete causing abrasions in this area of the foot from pushing off the cement floors of the pools.

I’d say that this is tinea pedis that should clear up in a few days with the use of Loprox b.i.d. (I find it to be more effective than Lamisil cream). Fissuring under the base of the 5th toe is common. It sometimes can occur after wearing shoes with no socks after one day.

I can’t say that I blame the 19  year old girl for not wanting surgical treatment of her syndactylized toes. A lot of women have this and aren’t concerned. For example, my wife and 26 year old daughter both have partial syndactylism of two of their toes and have never been the least bit bothered by it.

RE: Time for EBM Study on Custom Orthotics

I appreciate the ongoing discourse about biomechanics theory and who’s right/who’s wrong. But I’m still waiting to see ANY decent controlled study in our literature.
 
When will one of our “experts” please take a couple hundred patients, and divide them into two groups? Have one group use “custom” orthotics and the other use OTC inserts for some condition (heel pain, metatarsalgia, whatever). After some period of time, maybe a year, look back and see whether there’s a statistical difference in outcomes. How about the same study (but longer than a year) with custom orthotics vs. no orthotics on patients with early hallux valgus to see if there's REALLY any effect upon progression of the deformity.
 
Until that’s done, we are at the mercy of insurance companies who (rightfully) say our “custom” orthotics are unsubstantiated. Worse yet, we are denied the ability to refute the ads that are now popping up for commercial orthotic stores who claim that theirs are “300% better” than podiatrists’ custom orthotics!

Some clarification is needed. Does Dr. Kaniadakis think I'm encouraging more foreign graduates? I'm not saying that at all. I was suggesting a possible alternative pathway FOR PODIATRISTS to be allowed to take the exams that foreign-trained applicants to the U.S. are allowed to take. I had hoped this might demonstrate educational equivalency and thus the right to a general licensure pathway.

It is obvious to me that continued bribery of politicians to increase scope is not the appropriate way to gain recognition. Tested and proved educational equivalence is. At present, there is NO mechanism available in any venue. Podiatry is sure considered FOREIGN in some viewpoints (tongue in cheek!)

RE: Scholl Professor Sues Associate Dean for Defamation

I too am saddened to see this kind of activity occurring at Scholl.  Both Dr. Bareither and Dr. Hrywnak were excellent professors when I was there.  The students need more professors like them, not less.  Hopefully, they will realize the futility of legal matters for something like this. It's time to put egos and hurt feelings aside, apologize to one another and move on.  The only thing that can arise out of this is further animosity and fattening the attorney's pockets. I hope the school will realize the value that the part-time professors bring, by exposing the students to some real world podiatry and business experience, and consider keeping on Dr. Hrywnak and others. They provide a great service.
