MFAS Podiatrists Testify for Creation of Amputation Prevention Task Force
 
Massachusetts Foot and Ankle Society members David B. Alper, DPM, Tyler Silverman, DPM, Hau Pham, DPM, and Sara Rose, DPM testified before the Massachusetts Joint Committee on Public Health in support of creating an amputation prevention task force. Joining with the American Diabetes Association NE chapter, of which Dr. Alper is a Board member, was State Government Affairs Director Steve Habbe and ADA lobbyist Ronna Wallace  bringing this important issue to light and working with impacted stakeholders.
 
MFAS podiatrists testify for creation of Amputation Prevention Task Force.
 
The creation of a Government Amputation Task Force in MA would be the first in the nation. It is hoped that, upon passing, it can be used as a template for other states across the country, using the collaboration between podiatry and the ADA and the task force to infiorm the public of the dangers of vascular disease, urging the medical community to perform further testing and push the insurance carriers to cover testing, annual exams, and other preventative care.

RE: Closure of Residency Program

The Crozer-Chester Medical Center permanently closed last month, effectively ending the residency program which has trained podiatric residents in Philadelphia for decades. Many students still utilize the so-called Crozer Manual in preparation for clerkships and the CASPR/CRIP podiatric residency interview process.  
 
I'd like to recognize and congratulate the faculty of the residency program for their years of educating residents, as well as recognize and thank the residency programs and program directors who accepted the podiatric residents who were orphaned because of this closure:  Christiana Care Health Services (Dr. Raymond DiPretoro), Roxborough Memorial Hospital (Dr. Larry Goss), Boston Medical Center-Brighton (Dr. David Sipala), and Loma Linda University Medical Center (Dr. John Santoro).

I applaud Dr. Lee Rogers and UT Health San Antonio Multispecialty and Research Hospital for being the state’s first to approve use of a new non-opioid medication (suzetrigine) for pain following orthopaedic and podiatric surgeries. For the first time in over two decades, suzetrigine, a novel small molecule and highly selective NaV1.8 inhibitor, offers an effective and safe non-opioid, non-addictive treatment option with broad applicability for managing moderate-to-severe acute pain.  I am fascinated by the medicinal chemistry, biochemistry, and neuro-pharmacology interplay between this therapeutic and the NaV 1.8 sodium channel subtype.  
 
The recommended starting dose of suzetrigine is 100 mg taken orally on an empty stomach. Starting 12 hours after the initial dose, patients should take 50 mg tablets orally every 12 hours. Patients should be advised to swallow suzetrigine tablets whole and not to chew or crush them. Furthermore, suzetrigine should be prescribed for the shortest duration consistent with individual patient treatment goals. A Patient Savings Card is available. Eligible patients with insurance may pay as little as $30 per dispensed prescription.   
 
One study emphasizes that because of the high selectivity of suzetrigine for NaV1.8 (does not bind to other sodium channels/receptors with CNS activity), suzetrigine does not have CNS side-effects or addictive potential associated with opioids. Podiatric physicians, exercising their clinical judgment, may want to review the Journavx® (suzetrigine) drug monograph or suzetrigine complete drug review narratives, and/or audio-videos describing this product and consider this pharmaceutical for appropriate candidates within their podiatry patient population. It is hoped that suzetrigine will become an effective medication that podiatric physicians can incorporate into their non-opioid multimodal toolbox.

I agree with Dr. Sherman. There is nothing wrong with advanced training, and the decision to mandate a 3-year residency was well intended. However, a mandatory 3-year "surgical" residency should be reserved for those who have a desire to specialize in surgery. A number of podiatry students are better suited and more comfortable with practicing as a general podiatrist.  
 
No matter how you slice it (no pun intended), there is something wrong when there are only 400 applicants over 11 accredited podiatric medical schools in the U.S. It's time to face the facts and come up with another answer for this profession. If not, based on the diminished number of applicants, we're headed for a downward spiral into the abyss in the very near future.
