Better Patient Conversations Lead to Better Care: PA Podiatrist
When patients are empowered to take an active role as a partner in their treatment and feel they are being treated with kindness and empathy, it can build stronger relationships and lead to better treatment outcomes. Sham Persaud, DPM, is a podiatric surgeon at Advanced Surgical Hospital, which Healthgrades ranked as the top hospital in Pennsylvania for joint replacement in 2025.
|
Dr. Sham Persaud |
"The biggest mistake I see is rushing through difficult conversations. Because they’re uncomfortable, people tend to try to get out of the room as fast as possible and move on to the next patient. One of my great mentors, Alan Catanzariti, DPM, my residency director, helped me learn to have difficult conversations with patients. Whether it was explaining a diagnosis, discussing complications, or anything in between, he would lean back in his chair—always sitting, always relaxed—and just take time with the patient. And that’s something I think patients really appreciated," says Dr Persaud.

PODIATRIC PODCASTS |
VA Podiatrist Reveals The Silos in Specialist Medicine
Ben Pearl, DPM collaborated with healthcare advocate Craig Thomson to discuss the silos that exist in our healthcare model. The lack of communication is, in part, a product of the hyperfocus on each specialist’s own lane. Here is an segment from the podcast:
|
Craig Thomson and Dr, Ben Pearl |
Thomson gave an example of the work-up for his peroneal nerve entrapment at the fibular head of his knee. His primary care doctor referred him to a neurologist who ordered an EMG test which was negative. He got a recommendation from an orthopedic surgeon to have the nerve decompressed. After following up with a neurologist, he decided to go to a sports medicine specialist and had successful nerve hydrodissection. The previous surgeon never reviewed complications or alternatives.

INTERNATIONAL PODIATRISTS IN THE NEWS |
Why You Should Take off Your Shoes at Home: Spanish Podiatrist
In some families, it’s not very common to take off your shoes when entering a home. However, this practice is quite typical in other countries, especially in Asian nations. For many people, this can be uncomfortable, particularly if they feel obliged to do so in the homes of family or friends. Yet, this simple action can bring significant health benefits.
|
Héctor Alonso |
Héctor Alonso, a podiatrist and surgeon, creates social media content on various topics such as the most suitable types of footwear. “Did you know that not taking off your shoes when you come home is a hazard?” This is how the podiatrist begins his reel. He bases his statement on a scientific study from New York, where samples were taken from streets that appeared clean to the naked eye. This research identified more than 30,000 fecal bacteria, which were later also found in the carpets of many homes."

PRACTICE MANAGEMENT TIP OF THE DAY |
Zero-Budget Staff Motivators that Beat a Pay Raise
These cost-effective strategies can boost employee morale and retention in medical practices, addressing turnover without breaking the budget.
3. Swap rigid shifts for micro-flexibility
Eighty-two percent of clinicians say flexible hours would ease burnout, yet only 29 percent receive them, according to a survey on curbing staff turnover. You can close that gap without blowing up the schedule:
-
Let billers log in from home during storms.
-
Approve a lunch-hour swap so a nurse can make the daycare pickup.
-
Offer Friday half-days when patient volume is light.
Generational staffing research shows that many employees will trade modest raises for autonomy nearly every time.

RESPONSES/COMMENTS (MEDICARE FRAUD) |
RE: Medicare Fraud Hurts Everyone
From: Paul Kesselman, DPM
Fortunately this was not a DPM, but an almost octogenarian formulated a $1B DME fraud scheme via multiple marketing companies who managed along with cooperating patients, physicians and other suppliers to perpetrate one of the largest takedowns in the history of the CMS Fraud investigations strike force. If you wonder why the application process to obtain a DME PTAN or retain the one you have via revalidation, look no further than this report $1B Strike Force Bust.
My only question to all involved in this and previous large takedowns is WHAT TAKES YOU SO LONG and why are these investigations so expensive! If any of these people would...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A |
From: Paul Kessleman, DPM
I hope Dr. Pressman can answer a question or two about the reference AMA article. I agree that the three-year 4/4/3 model may have helped elevate us to the point where podiatry is no longer negatively noted in this interesting article. Unless I am missing something, however, I failed to see in the reference article any mention of podiatrists at all. The physicians who were listed were MDs, DOs, even optometrists and naturopathic physicians.
I guess the good news is that nothing was noted negatively about DPMs, but where does that leave us if we are not even listed? I am sure those of us who read the AMA article would appreciate some interpretative explanation for this. Or if I missed something, please let me know.
Paul Kesselman, DPM, Oceanside, NY

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B |
From: Rod Tomczak. DPM, MD, EdD
I would like to comment on the letter by Martin Pressman, DPM. In the spirit of true transparency which I insist upon, Dr. Pressman was one of my trainers at Metropolitan Hospital in Philadelphia where I completed a two-year residency beginning in 1977. I am neither afraid to bury or praise podiatrists who have influenced my life.
When I arrived at Ohio State University from Des Moines CPM in 1995, there were two foot and ankle care givers for the 35,000 faculty and staff plus their families who were self-insured by the University’s prime care, 55,000 students and outside patients. The two foot and ankle physicians were the chair of the department, Sheldon Simon, MD and myself. I asked him if I could start a surgical residency program shortly after arriving. He told me...
