If the eyes are the windows to the soul, the feet can offer a view of overall health, say experts. “Your feet reveal many health conditions,” says Oliver Zong, DPM, a New York-based podiatrist. From thyroid issues to heart problems, your feet can reflect the inner workings of your body.

Here’s how: Cold feet: If your feet are always cold, it may be a sign of an underactive thyroid, especially if you also have dry skin and hair, and fatigue. Another possibility: Raynaud’s disease, which can signal lupus or rheumatoid arthritis as an underlying cause, says Dr. Zong. Unexplained numbness: In rare cases, numb or tingling feet can be a sign of multiple sclerosis, nerve damage, fibromyalgia, or diabetes, Zong tells Newsmax Health.

Deadline Nears for Meaningful Use Hardship Exemption
Eligible professionals who did not attest to meaningful use of electronic health records under the Medicare EHR Incentive program during 2014 must soon take steps to avoid paying a financial penalty on their Medicare reimbursement during 2016. The Centers for Medicare and Medicaid Services is advising that eligible professionals meeting certain criteria apply for a hardship exemption by July 1. Here is more information from a CMS notice:
The hardship exception applications and instructions for an individual and for multiple Medicare eligible professionals are available on the EHR Incentive Program website, and outline the specific types of circumstances that CMS considers to be barriers to achieving meaningful use, and how to apply.

Query: Coding Cheilectomy with Akin Osteotomy
What would be the correct way to code the following procedures, when all performed on the same foot on the same date of service? Cheillectomy 1st MTPJ for hallux rigidus, Akin osteotomy for deviated hallux (valgus), Tenotomy and capsulotomy of the 2nd MTPJ.

Response: I suggest CPT 28298, which is bunionectomy by phalanx osteotomy. In the CPT professional edition book, there is an example/picture of a phalanx osteotomy with resection at the metatarsal head.
That said, you have to document if the work done was significantly more complicated to warrant other procedures. The second MPJ release could be CPT 28270 depending on what was done.

While a pan met head seems the more radical procedure, in the long run, you will best serve this patient by doing what is needed. More limited procedures now only make it likely that you will need to do more later. If she is healing ulcers with A-1Cs in the 8s, then she is quite likely to heal the pan met procedure, assuming careful dissection by you. By all means, speak with her PCP/diabetologist regarding control of her sugars, but you'll have a happier patient sooner, with one surgery, than you will several years down the road with multiples.

Simply based upon the history and data supplied, I tend to go the opposite way and be more aggressive with a pan met head. I would add that especially if the patient is obese. Of course this is said without seeing the patient's gait, and actual foot exam (which is very important in making individual patient decisions).
I have had the opportunity to do isolated met head resections on diabetics, and for some, it works well, especially in the acute stages. I have also been able to follow patients over a long period and have seen my share of transfer ulcerations from this approach -even with...

Unfortunately, Dr. Schaffer’s experience is becoming more and more common. You will need to look at your contract very carefully. If you bought your software before the first collapse of the company, you have one contract with terms of use. After the takeover, the company issued a new terms of use. Read them very carefully.
You may want to have an attorney, one with IT litigation experience read it. What you are experiencing is known as a “Kill Switch.” It was slipped into the software without the knowledge of the users, and it effectively changed the terms of use without your knowledge or permission. This is where a skilled attorney comes in. The requirement to maintain a support agreement to continue to access the software after you cease doing business with the company may not be spelled out in the new terms of use, and you will have a cause of legal action. This, however, can get expensive.

This finding is unfortunate because when examining the risk/benefit ratio, the evidence is clear that statin use has a greater benefit than risk. Statins reduce the incidence of myocardial infarctions and CVAs. I attended a lecture this past weekend given by an interventional radiologist who said that when he does balloon angioplasties to treat lower extremity arterial blockages, he places patients on statins to reduce the incidence of recurrences.
What is the relationship between the use of statins and incidence of diabetes? Are the pharmaceuticals the cause or is it the fact that people who are placed on statins already have risk factors for diabetes such as hyperlipidemia caused by overeating and lack of exercise? Are all statins implicated or only a select few?
