Hyperbaric Chamber Hood Saves NY Podiatrist's Life During Pandemic
 
During the first wave of the pandemic in 2020, Warren Altwerger, DPM fell ill with COVID-19. He struggled with severe symptoms, his breathing became difficult, and he ended up at Phelps Hospital emergency room. Knowing the mortality risks of being ventilated, he opted for a hyperbaric chamber oxygen hood like the one routinely used at the Department of Undersea & Hyperbaric Medicine at Phelps Hospital Northwell Health.

Working with the Feinstein Institutes for Medical Research, his physician, Owen J. O’Neill MD, MPH, medical director, Division of Undersea and Hyperbaric Medicine at Phelps Hospital, recommended that Dr. Altwerger take part in his team's clinical trial, which he believed would help patients whose oxygen levels had dropped too low and would require mechanical ventilation. It worked. After spending 13 days in the hospital, with four of those days spent in the oxygen hood – without the use of a ventilator, Dr. Altwerger went home where his oxygen levels gradually improved. One year later, he has made a full recovery and is back to seeing patients and practicing podiatry.

OH Podiatrist Uses Fish Graft to Save Patient's Foot
 
Jeff File was suffering from a non-healing wound that was infected and while staring down amputation, he met James LaPolla, DPM, who offered a relatively new procedure. It uses fish skin to initiate tissue regeneration, and a Vienna Township man says it saved his foot from being amputated.

"It's good for all types of wound care," Dr. Lapolla said. "The benefit of micro-3s [is that] it basically mimics the human skin if you take a slice of it in terms of the multiple layers of the fish skin and layers of human skin. It allows for revascularzation much easier than some other types of grafts, other than their own human grafts obviously, that would be the best. It also keeps a bacterial-free, infection-free wound bed that then allows the recipient, the patient's natural skin, to incorporate over that wound so you don't get a lot of scar tissue."

My Feet are Killing Me Kicks Off 4th Season
 
The new season of TLC’s hit show, My Feet are Killing Me, recently premiered. Season four of “My Feet Are Killing Me” will have present severe medical issues. Drs. Brad Schaeffer, Ebonie Vincent, and Sarah Haller are met with the most challenging cases, such as a foot that “looks like a big volcano, bubbling over about ready to erupt.”

The pilot episode introduced cases such as a woman walking on a broken tibia for 10 years, bunions that prevent another woman from running again, and a man who experienced severe foot problems after having his big toe transplanted to his hand. Other unexpected issues will arise throughout the series – the doctors face a woman whose feet grew horns, sisters that suffer from different foot fungi, and a man whose foot is shaped like a hobbit’s foot.

RELEVANT RESEARCH
Patient Perception of the Diagnosis and Its Impact on Patients With  Melanoma or BCC
 
A questionnaire survey was administered to 355 patients with melanoma and 320 patients with basal cell carcinoma (BCC). At the time of diagnosis, 66.8% of patients with melanoma and 36.1% of patients with BCC reported that they were "shocked." Almost half of the patients with BCC were fearful of metastatic progression or the risk of dying because of the word "cancer." Patients with melanoma isolated themselves, felt discouraged, tired, tense, depressed, and alone, and they had difficulties falling asleep more often than patients with BCC.
 
Dermatologists should be mindful of the psychological impact of a diagnosis of melanoma or BCC when delivering the diagnosis and offer empathy and reassurance.

Query: Source for Dehydrated Alcohol for Injection
 
Is anyone still able to obtain dehydrated alcohol for alcohol injections? BPI has Ablysinol available which is the only FDA-approved dehydrated alcohol product available but is marketed for symptomatic hypertrophic obstructive cardiomyopathy and is prohibitively expensive. Is another similar product available anywhere that can be used for alcohol injections?

Unfortunately, without an organized response to the insurance companies, they will continue to push the limits of our tolerance. Years ago, Carnegie Steel was one of the biggest companies in the world and it made Andrew Carnegie the wealthiest man in the world. His workers, however, were forced to work 12 hour shifts 7 days a week. He told them that if other local employers lowered their work days and hours [and Carnegie didn't], then eventually Carnegie Steel and its employees would be put out of business.
 
The money is there, the need for doctors is there; unfortunately, the organized response is not. The insurance companies, hospitals, drug companies, and medical product companies all rely on doctors to stay in business. I have been charging $250 to cash patients for a P&A since 1991. Currently, one of the larger private insurance companies in Philadelphia reimburses less than $120. Patients are willing to pay for our services... pain is a very strong motivator. I realize that going on strike is considered unethical but unless something is done, nothing will change, and the best and brightest will certainly not be considering podiatry as a career.

Dr. Block asks, “Who wants to be a podiatrist?” Dr. Fellner discusses how non-professionals appear to charge more for routine foot care than podiatrists. Dr. Davis shows how logic plays into this ongoing scenario for DPMs, if we just take action toward parity at all levels including plenary licensure. Past APMA President Jeff DeSantis, DPM, communicated well the need to have our profession at the level of core competencies that could entitle equivalent licensure opportunities to entering classes to podiatric medical schools.
 
The osteopathic profession can’t open schools fast enough with entering classes of around 150 students in each. There are currently 38 accredited colleges of osteopathic medicine in the United States. These colleges are accredited to deliver instruction at 60...

Kim  Clayton asked about using 64455 RT and 64455LT, and was given two solutions. Neither of the solutions is recommended. The physician should bill a corrected claim and put both codes on one line, using the 50 bilateral modifier,  64455 - 50 , and double the price.,  It will get paid.

Medicare MUE limits 64455 to 1 unit per DOS, however allows this service to be submitted with modifier 50 with 1 unit. A recommendation to correct the claim from 64455-LT x1 & 64455-RT x1 to 64455-50 x 1 unit and bilateral diagnosis may be the easiest option.  Appealing the claim is an option as well, but we all know how long that can take. If the opportunity to correct a claim exists, that above option is recommended before appealing.
