NY Podiatrist Discusses Conservative Treatment of Hammertoes
 
Years of trekking can give you pain, and leave your feet in no shape to be seen. There are options to alleviate the discomfort of bearing your not-quite-ready-for-the-beach feet. Common complaints are bunions, hammertoes, corns, and calluses, according to New York City podiatrist, Krista Archer, DPM. She restores the cushions to the bottom of feet to make wearing heels so much more comfortable. "Scultpra® Aesthetic is placed primarily in the balls of the feet where fat has been lost to restore padding and reduce pain. Sculptra induces collagen to grow, and results can last for more than one year. Sometimes, I also use Juvederm® for corns on the tops of the toes."

Hammertoes can cause your smaller toes to bend inward and downward, due to a muscle or tendon imbalance. This also increases your likelihood of developing corns because the toes don't sit flat, and may rub against the roof of your shoes. Shoes that tend to rub against or place pressure on skin can cause corns or calluses. Corns show up on your toes, while calluses form on your heels. Corticosteroid injections can be administered to ease inflammation and pain.

RIPMA  Presents Centennial Year Myron Keller Memorial Award
 
Dr. Robert Crausman of Rehoboth received the Centennial Year Myron Keller Memorial Award from the Rhode Island Podiatric Medical Association (RIPMA) at its Centennial Year luncheon. Crausman was selected the recipient for his outstanding service to podiatry in Rhode Island.

Dr. Douglas Glod, chief of podiatric medicine and surgery at Memorial Hospital of Rhode Island, said, "The Myron Keller Memorial Award is given to Dr. Crausman because he exemplifies the high ideals and dedicated spirit so nobly portrayed in the life of RIPMA's past Treasurer and editor, Dr. Myron Keller." Crausman is a clinical professor of Medicine at Brown University School of Medicine and is President of Trumed in Fall River.

Joel Wilner, DPM has been appointed a Board Member at New Mexico Board of Podiatric Medicine. Dr. Wilner is a foot and ankle surgeon at NM Foot and Ankle Associates in Santa Fe, New Mexico and is currently a Board Member at Christus St. Vincent Regional Medical Center, where he was past Chairman of the the Foot and Ankle Department. He is a graduate of the California College Of Podiatric Medicine and is Board Certified in Foot and Ankle Surgery.

I have been going to see my favorite podiatrist, Paul Poshoglian, for more years than I can remember. He is an absolute whiz with the corn-slicing blade, not to mention an amazing storyteller. Paul began his career tending feet with Elizabeth Arden back in the 1960s and is still going strong. I visit him regularly – every four weeks – and am always at my happiest propped up on the reclining podiatry chair.

Paul is never judgmental about my sky-high shoes; in fact, he is often in awe, and the reason I am still tottering around in these killers is largely due to his expertise. Healthy feet contribute to general well-being, so even if your shoe cupboard is full of flats, pop a visit to a podiatrist into your health calendar.

Medicare has released new DME codes for night splints and BK pneumatic walking boots and taken the old definitions for the new codes and left the old codes with new definitions. For example L4396 has a new definition, and L4397 has the old definition that was once L4396 – What’s going on?

Paul Kesselman: Medicare has changed the codes and kept the payment the same. Maybe this starts the beginning process of competitive bidding for these codes in the future. For now, everyone should clearly make sure they are aware of the changes and what the codes L4360 and L4396 are today, and verify they are billing the correct code.

STATE HEALTHCARE LEGISLATION
Bill Would Allow OH State Medical Board to Issue Fines for Rules Violations
 
At the request of officials from the State Medical Board of Ohio, Rep. Anne Gonzales is pushing for the passage of a bill that would permit the board to fine medical professionals for failing to comply with continuing medical education requirements without suspending the individual’s certificate to practice. House Bill 531 also would authorize OSMB to fine a certificate holder for violating laws administered by the board and impose additional terms and conditions for certain certificate restorations. The fines could be levied against physicians and podiatrists, among others.
 
Current Ohio law authorizes the board to limit, revoke, or suspend an individual’s certificate to practice, refuse to register the individual, refuse to reinstate a certificate, or reprimand, or put a certificate holder on probation for a rules violation but does not authorize a fine. If the bill is enacted, says Gonzalez, Ohio would join 38 states which have permitted their medical licensing bodies to impose fines on physicians and other medical professionals who are found to have violated state law.

It seems that there are specific requirements which I understand, but are there specific forms that they would approve if filled out and kept in patients' charts, so when they request documents, they will have forms that cover all their requests and rules?

On paper, DPM Preferred may sound great; however, many hospitals in NYC do not accept them as they are a 'Risk Retention Group'. You may want to check with any institution you have privileges with as to whether they allow RRGs.

Many may have taken my words way out if context. I am neither advocating mass exodus from medical practice nor retiring anytime soon. I too suffer from the grass is greener syndrome, thinking this or that vocation or profession is better with less govermentt oversight. The truth is just about anywhere you turn, there is goverment oversight. Ask any factory owner about dealing with OSHA, drug companies, about the FDA, etc. Many of these companies also must deal with a complex set of local, state, and municipal regulations. Learning how to live and succeed with the government in your back pocket is truly the key. 
 
I have one simple set of requests from any level of goverment: write clear and concise regulations which are realistic to practice and are the reality of medical practice. Be sure they don't conflict with those of other government agencies, and be sure your own agency's employees understand their own regulations. And before any of you think consulting is so glamorous, think again. Dealing with owners, corporate boards, etc. is no easy picnic. So, forgive me if I used this space to do some personal venting. We all need to!
