Wear Patterns Can Predict Potential for Injury:  NY Podiatrist
 
“Wear patterns alert us to certain patterns of gait and tell us what position certain joints are in when you are coming into contact with the ground, as well as when you are pushing off,” explains Jacqueline Prevete, DPM, a board-certified surgical podiatrist and runner herself (with an impressive marathon personal record of 2:58). As you walk or run, the friction and pressure between the sole of the shoe and the ground compresses and wears away some of the outsole material, shaving it down.

So the wear pattern basically tells you what areas of your shoes are sustaining the most ground reaction forces when your foot comes into contact with the ground, whether you’re running, walking, or hiking. In other words, they are almost like windows into how balanced your gait is, and can be helpful for identifying potential issues within your stride. “[You] can see whether there is any asymmetry by comparing one side to the other,” says Dr. Prevete. This could alert you to certain patterns that could eventually lead to injury.

NY Podiatrist Discusses How to Choose Shoes if You Have Heel Spurs
 
Heel spurs  are a common foot ailment. "The cause is chronic and repetitive micro-tearing of the plantar fascia or Achilles tendon," says Nelya Lobkova, DPM, podiatrist and owner of Step Up Footcare in New York City. "Over time, the body deposits calcium where the tearing occurs as a reaction to the tearing."

When looking for a shoe for heel spurs, Dr. Lobkova recommends a rigid sole, as that dictates the stability of the shoe. "Someone with heel spurs needs extra rigidity in the sole and cushioning in the midfoot to prevent impact on the heel," she explains. To see whether your sole is rigid enough, Dr. Lobkova recommends a test: "Hold the front of the shoe with one hand and the back of the shoe with another hand. Try to twist the shoe side to side. There should be minimal to no twisting motion possible."

This conversation on the prescribing of urate-lowering medications is not a new one, with many saying yea and others saying nay. The crux of the controversy is whether treating hyperuricemia is addressing a systemic condition or a local manifestation of gout. Those who argue that it’s systemic are quick to equate it with the management of diabetes. There are distinct differences between the two ailments. For example, if a patient presents with acute gout affecting an elbow or has hyperuricemia with no clinical manifestations, treating it or even telling a patient not to have it treated would be out of scope for us.

I think that we all should be able to diagnose and treat a foot/ankle suspected or known acute gout attack to a joint. For example, aspiration of the great toe joint, synovial fluid crystal analysis to establish actual gout is a common occurrence in clinical practice. As we well know, the fluid aspirated is very often actually seen as gout with tophi. Then, injection of local anesthesia/steroid injection into the joint is appropriate. Rx for indocin, 5 day course, or Medrol dose pack, if recurrent attack, is also appropriate and in the realm of our practice, as short-term acute management.  
 
Laboratory orders for uric acid blood level and referral to PCM is also a prudent plan after your initial gout attack diagnosis and in-office treatment. I agree with Dr. Jacobs and those who do not recommend continued medical management of "hyperuricemia". In my clinical experiences in a multi-disciplinary hospital-based, clinical USAF practice, and then in an orthopedic surgery group practice. This approach has been accepted well by PCMs, rheumatologists, nephrologists, internal medicine docs to whom the patient is referred. I can assure you that those specialists are very happy to have a joint aspirate confirming gout rather than just elevated uric acid levels. We all are aware of patients who say, "I have gout," but there is never a definitive joint aspirate confirming that diagnosis!

I recently had cause to delve into and discuss the topic of practice evaluation. A business broker, with decades of experience, shared with me his opinion of practice valuation: "they are junk, I stopped doing them years ago, waste of paper and money." Adding that most buyers need to apply for finances, it is the underwriter who will determine if the banks' criteria can be met. 
 
He opined also that many deals fail when a seller has unrealistic financial expectations or is too emotionally invested. Discussed further, the underwriter explains, "there are a lot of moving parts when considering a loan application."  The practice valuation is not one of those parts.
 
In the final analysis, a good deal is one that both parties are satisfied with. And the bank agrees to make a loan based on their own business analysis. A seller may want to showboat their most profitable year of the business. Some sellers seem to have developed 'COVID denial' and do not consider the impact this has had on business, in the last two years, often preferring to quote 'pre-covid profits'. He adds that most deals can find a solution when the business is realistically priced. How much is a business worth? 30%-50% of the gross annual income will be typical in many successful negotiations.

I would be very interested to find out how much it costs for a valuation of your practice/charts. When I decided to sell my practice in 2015, I contacted a person to give me a valuation of my practice. His services involved my sending my financials and physical make-up of my office. All of this is done via the Internet. They do not come out to your office unless you want to pay extra for that. The valuator's fee was almost $5,000. 
 
My advice would be to do as much research as possible and come up with a price. Let the response you get dictate how accurate your pricing is. Paying someone to give you a valuation of your practice is throwing away money unless the fee is under $1,000.

RE: Additional Member Benefits Might Increase Membership

If NYSPMA and other state societies want to increase membership, why not keep the educational conference online, instead of removing it June 2022? This extended online posting would not have anything to do with granting CME credits. It would provide additional learning opportunities to review educational material. Furthermore, previous years' lectures should be posted (as long as they are still relevant). Other state societies should do the same. 
 
Access to previous conference lectures without CME credits may provide incentive to join. Better yet, why not give on-demand access with CME credits for three years for all members?
