NV Podiatrist Appointed CAVHCS Chief of Staff
The Central Alabama Veterans Health Care System (CAVHCS) has announced that Kittra Owens, DPM has been appointed Chief of Staff effective June 19, 2022. Dr. Owens earned her podiatric medical degree at the Temple School of Podiatric Medicine. She is a Navy veteran who served in Afganistan, Cuba, and Japan.

Considered the father of minimally invasive foot surgery, Albert R. Brown, DPM will celebrate his 103rd birthday on Wednesday, May 25. Ever eager to care for patients, Dr. Brown did not retire from active surgical practice until he turned 99.

CA Podiatrist Convicted of Insurance Fraud, 1 Year Prison Sentence
A Tustin podiatrist found guilty in February of billing insurance companies for medical services that were never actually done was sentenced to almost a year in jail, officials said Monday, May 23. Renae Louise Witt, 42, was sentenced to 364 days in jail followed by two years of probation, Orange County District Attorney’s officials said. The podiatrist was also ordered to pay restitution to two insurance companies she billed for care she didn’t actually perform.

Appropriate Instrumentation, Technique Keys to Successful Ankle Arthroscopy
In his discussion on performing a successful ankle arthroscopy, James W. Stone, MD, said surgeons should use ankle joint distraction when performing the procedure. “The problem with the ankle joint, of course, is it’s a complex curved articular surface, [has] tightly held ligaments and, certainly, using the older-fashioned instrumentation of 4-mm scopes is tough to get into the joint,”
Stone said in his presentation at the Arthroscopy Association of North America Annual Meeting. “The combination of using ankle distraction and smaller joint arthroscopes has been, I think, revolutionary.” In addition to using smaller scopes, he said it is also important to use appropriate hand instruments and appropriate-sized motorized instruments, including shavers.

Kudos to Martin Girling, DPM for exercising sensible independent thinking regarding patient care for systemic conditions when urgent attention seems to be unavailable. It's a touchy subject. I have been faced with this dilemma many times in my career and have always been uncomfortable with my response to patients as to why I wouldn't comply with their request. He has summed up my frustration for 45 years in practice with one sentence - "Our job is clear, our license is not."

I have suffered with gouty arthritis for years. I have been able to keep it under control with allopurinol for most of the time. The last time I had a gouty attack, I had my PCP put me on oral steroids. Of course, you must be weaned off of them gradually. I believe I was on the tabs a total of about 10 days or so. I believe other podiatrists have used this method for acute gouty attacks and let the PCP manage the uric acid situation. For me, oral steroids worked like a charm.

I agree with Dr. Girling that “Our job is clear, our license is not” and I do whatever it is I can, within the scope of my license and my education, but there are times when other PCPs and rheumatologists do not treat a patient properly or aggressively enough to take care of their initial symptoms. I just had a patient this morning that came in and told me that their doctor told them that they have gout and s/he gave them 100mg of allopurinol. No blood test, no x-ray, no previous history; just that their toe was painful, and this happens all of the time. This patient needed colchicine and steroids initially.
As an aside, I have also had many patients over the years who were told that they have psoriatic arthritis. No skin lesions, no nail changes, no negative RF or x-rays changes (joint swelling, dactylyitis), and no family history of psoriasis. Yet they are told they have...

Oftentimes, common parlance used to describe procedures does not align with the exact definition or even intent of the procedure. It is in that vein that I feel compelled to make a statement regarding what my colleagues routinely call a “nail biopsy.” The procurement of a sample of nail plate and subungual tissue by a clinician for analysis under a microscope is NOT A BIOPSY. On the laboratory end, although they may call it a biopsy or mention the word biopsy in the report, the pathology billing is under the domain of the laboratory procedures and is in no way connected to the procedure done in the podiatry office.
There has been discussion (and passionate disagreement) about this for a few years now, with many a word to the wise about calling the submission of nail tissue samples a biopsy. It is not. Even though the CPT descriptor of the nail biopsy, CPT 11755, would seem to indicate that...
