INTERNATIONAL PODIATRISTS AND DIABETES
Awareness Reduces Diabetic Leg Amputations: Aussie Podiatrist
 
Better access to health services and more awareness of diabetes are being hailed for a 70 per cent decrease in the risk of type 2 diabetics requiring a leg amputation. The finding comes against a backdrop of rising diabetes rates worldwide, study author and podiatric medicine researcher Dr. Mendel Baba says. "Although the burden of diabetes is increasing… there have been some changes in the way we care for diabetic foot disease or management strategies that have been successful," she says.

The research compared people who took part in the Fremantle Diabetes Study from 1993 to 1996 (Phase I) with those who participated from 2008 to 2011 (Phase II). Dr Baba says greater awareness of diabetes and diabetic complications, and GPs checking for complications have also likely contributed to the drop in amputations. Dr. Baba, who conducted the research at Fremantle Hospital, says amputation is only a last resort for diabetics. "Someone with diabetes who's diagnosed today won't necessarily have an amputation tomorrow," she says.

He completed a three-year residency in adult and pediatric foot and ankle surgery at Christiana Care Health System in Wilmington, Delaware, as chief resident.

QUERIES (CLINICAL)
Query: Odd Reaction to Sclerosing Therapy
 
The other day, I gave a patient an injection for a Morton's neuroma using the dehydrated alcohol solution that I have had much success with for the past 15+ years. The patient called my office one hour later informing me that her entire leg from the knee down was numb! This eventually wore off, but left her with some knee pain which also eventually subsided. Has anyone ever had this happen to a patient?

Query: Accessory Navicular ICD-10 Code
 
I cannot find a code close enough to the accessory navicular or the os tibiale externum. Can someone make some suggestions? The closest I can find is M21.0 (valgus deformity not elsewhere classfied) or Q66.6 (other congenital valgus deformity of the foot).

Response: If you look up "accessory navicular" in the ICD-10 Tabular Index, you won't specifically find it, but you will find Accessory bone NEC Q79.8, Accessory tarsal bones Q74.2. When you check the Tabular List, you will find that Q79.8 is described as "other congenital malformations of musculoskeletal system." 74.2 is described as "other congenital malformations of lower limb(s), including pelvic girdle." 
 
Both present as congenital malformations. Of the two, obviously, Q74.2 is more specific. However, if you go to the CMS site and look at the 2016 GEMs (General Equivalent Mappings) crosswalks from ICD-9 to ICD-10, ICD-9 755.67 (accessory bone foot) crosswalks to ICD-10 Q66.89 - other specified deformities of foot. If you go to the APMA Coding Resource Center ICD-10 Quick Index, the AAPC crosswalk site, ICD10Data.com, etc., all point you to Q66.89, which is my recommendation.

I was at a lecture years ago by Gary Gibbons, MD of New England Deaconess/Harvard who said "eschar-goes". Having also listened to a PhD nurse giving a lecture at an APWCA meeting on leaving stable eschar, I asked about the difference between what we docs are taught and what the nurses are taught.
 
The docs and the nurses are trained differently in how to treat stable eschar. The nurses are taught to leave it alone. We are taught to remove it. The fact is that you don't know what's going on underneath. You need to use your clinical judgment. If you think it's infected underneath or that this necrotic tissue is interfering with healing, remove the eschar. Dead tissue doesn't get better.

I believe the APMA letter from Dan Davis in response to the AAOS letter was well written and honest and a fair representation as to who we are and why we should be on par with MD, DO and DDS/DMD in the VA. Although the AAOS letter did impart some statements of truth, it also included inaccuracies. It was framed in such a manner as to be disrespectful to us as a profession, and as a whole is dishonest. The orthopedic community should be outraged by their society's initiative to hurt us in such a manner. In spite of this threat from AAOS, it amuses me that they are so paranoid of us as to behave this way.

I think the real issue is that AAOS is getting involved in a problem that they cannot solve. There are no full-time orthopedic foot and ankle surgeons at the VA vs ~ 400 full-time DPMs. There are 16 part-time foot and ankle orthopedic surgeons versus over 1,000 part-time DPMs in the VA system. Since the VA (in reality) pays much less than private practice, adjusting DPM salaries has nothing to do with preventing foot and ankle orthopedic surgeons from doing cases. I’m almost tempted to say, let the AAOS fill all the 400 DPM positions with foot and ankle orthopedists so there will be a lot of great jobs available for DPMs in ortho groups.
