OH Podiatrist Discusses How to Spot and Treat DFUs Early
 
“In people with long-standing or poorly-controlled diabetes, the nerves in the feet often lose their ability to sense pain,” explains Windy Cole, DPM, a Cleveland-based podiatrist and certified wound care specialist. “That means someone can step on a sharp object or wear an ill-fitting shoe and not even know there’s been an injury until it’s already infected.” Anyone with diabetes is at risk, but some people are more vulnerable to foot ulcers than others. That includes people with poorly-controlled blood sugar levels, says Dr. Cole, who teaches at Kent State University College of Podiatric Medicine.

“Daily foot checks should be the norm for patients with diabetes,” says Dr. Cole. “Look for any areas of redness, blistering, callus tissue, or even breaks in the skin. It’s a serious problem that could mean something dangerous is forming underneath. If you can’t see the bottoms of your feet easily, place a mirror on the floor,” she adds. “Or have someone you trust act as a ‘foot check buddy.’” This includes checking between toes, where infections can hide.

The Professional Society for Health Economics and Outcomes Research (ISPOR) announced the results of its recent elections. Re-elected as a Director was Lucinda Orsini, DPM, MPH. Dr. Orsini is vice president for value and outcomes research at COMPASS Pathways in Skillman, New Jersey. Dr. Orsini earned her podiatric medical degree at the Temple Univeristy School of Podiatric Medicine and her MPH degree in Health Policy and Management at the Harvard T.H. Chan School of Public Health.

The 11th Shijiazhuang International Diabetes Conference, held at The Second Hospital of Shijiazhuang, convened more than 1000 clinicians, scientists, and policy leaders from across China and around the world. The conference spotlighted urgent challenges and bold innovations in diabetes care, with a focus on prevention, technology, and limb preservation.

RE: The Passing of Hugh Bryant, DPM
 
Recently, we lost one of our very own who served as Clinical Instructor in General Podiatry, now called Medicine Clinic at NYCPM. A son of Savannah, GA, he earned his DPM degree in 1975 and began teaching at The College (1979—2003), while running a private practice in Harlem and later relocating his practice to Brooklyn, NY. He was an avid lover of golf, photography, fixing cars, and most importantly, the practice of podiatry. Dr. Bryant devoted his life to bringing out the best in every student while coordinating extra-curricular basketball competition games between students and faculty.

He leaves behind his wife (Grace) of 56 years, son (Hugh Jason), daughter (Kiera) and grandson, Elijah. Dr. Bryant will surely be missed by many in the profession who remember his stern teaching style and serious demeanor in clinic. His dedication to the success of every student and our profession will never be forgotten.

People are wondering why podiatry is fading as a profession. Below is a brochure from a retail store promoting custom foot orthotics for painful feet, knees, and hips. Besides this being inappropriate to be available for the general public, why don't I see the APMA being more aggressive in advertising and promoting of podiatry with biomechanics for treating the feet to the lower back?

Why do we forget the knee pain patients, especially the female athlete with femerol anteversion? The people who work in these retail stores have limited education and training, and thus should not be able to dispense “custom” orthotics.

Some years ago, I was serving on the credentials committee at a large hospital in St. Louis when a DO orthopedic surgeon applied for staff. The discussion, led by the MD chairperson, was that "we" (i.e.-medicine) did not recognize his DO residency nor his DO board certification. He was denied staff access. Prior to that, while in high school, college, and podiatry school, I worked as a surgical retractor holder and then as a scrub nurse in several major Philadelphia hospitals. Never did I see a DO in these hospitals, nor any positive references to the DO degree.
 
Today, 11% of medical school graduates hold a DO degree, and schools of osteopathic medicine are rapidly multiplying. With no reference points other than common sense, I suspect the majority, not all, but the majority of students entering osteopathic medicine colleges do so as a second choice to obtaining an MD degree. Similarly, I suspect...

I agree with my good friend Dr. Reingold that, certainly, we are regarded as doctors by our patients and the medical community but disagree with him strongly on one point that he makes. Putting all podiatry students through a 3-year surgical residency is resulting in 3 major problems for us: (1) it is directly reducing the application pool to podiatry schools, and (2) it is wasting the time of the majority of podiatrists in training who are not suited to be advanced surgeons, and (3) it is resulting in those who will be foot and ankle surgeons having less cases to train on. I am not advocating for shortening residency training for any podiatrists. We must never reduce residency training to less than 3 years. 
 
What I am advocating for is to stratify residency training, to have a "sorting" process after the first year, when it's clear who is suited to be an advanced surgeon, and who is better suited to do wound care and general podiatry. Over time, we will get more applications to podiatry schools by the many who know that they don't wish to be surgeons, but do want to be great general practice foot doctors.

Maybe I am overly simplistic, or maybe I’m just old! But, if you open up a foot, move the bones around as needed, put it all back together again, that sounds like a doctor to me; and also to everyone that I know, including other medical professionals. If you heal a nasty wound that’s been around for a long time and possibly save a limb, that sounds like a doctor to me; and also to everybody that I know, Including other medical professionals.
 
On the flipside, if you have a busy office full of patients that love you, because you make them feel better, that sure sounds like a doctor to me; and also, to everyone I know, including other medical  professionals. I can understand the fixation on the degree, but it’s time to either get over it, or to somehow try to fix it. I hope that...
