PODIATRISTS IN THE NEWS
Salon Visits Increase Risk of Infections: DC Podiatrist

If you’re a regular at nail salons, you may be dismayed to learn that your pedicure may be more of a pedi-curse. “Getting pedicures, especially at a nail salon, can increase potential risks of bacterial, fungal, and viral infections — nail salons are hot spots for toenail fungus,” podiatrist Saylee Tulpule, DPM told The Post. Thankfully, the doc has pinpointed some smart choices you can make to avoid those gross problems and other health issues.

Dr. Saylee Tulpule

According to Dr. Tulpule, you should be especially cautious if you get the sense that your technician is…not altogether competent. “Improper technique by a nail technician can also lead to ingrown toenail infections or skin breakdown, especially if using cuticle nippers or graters to file thick, callused skin,” she explained. If you decide to go the salon route, it’s best to BYONP — that is, bring your own nail polish. “Bringing your own nail polish can reduce chances of fungal nail transmission — fungal hyphae can live in between the bristles of the brush,” she said.

PODIATRISTS IN THE COMMUNITY
Podiatry is the Right Choice: MA Podiatrist

Podiatry is really treating anything that affects the foot and ankle, which then affects the way we walk and the way our body functions, particularly your knees and back,” said Don Pelto, DPM, podiatric physician and co-founder of Central Massachusetts Podiatry. “This work is so fulfilling because we make people feel better,” says Pelto.

Dr. Don Pelto

Dr. Pelto and Neil Feldman, DPM partnered to create Central Massachusetts Podiatry in 2009. Benjamin Saviet, DPM is also a partner, and Matthew Monette, DPM is an associate at the practice. All four are surgeons with years of experience treating patients of all ages, and Pelto is fluent in Spanish and Portuguese, which allows him to see patients who are more comfortable speaking those languages.

RELEVANT RESEARCH
Trial Tests Carbon Fiber Inserts for Osteoarthritis Relief

A commonly used method by podiatrists and physiotherapists to manage osteoarthritis (OA) of the first metatarsophalangeal joint (MTPJ) is undergoing formal testing in a randomized clinical trial, a UK researcher said on the first day of The Primary Care Show 2025. The Big Toe OstEoarthritis (BigTOE) Inserts Trial compares the use of a carbon fiber insert to stiffen shoes against a sham insole. The goal is to determine whether this approach can alleviate the pain associated with toe OA, said the trial’s chief investigator, Michael Backhouse

Michael Backhouse

Backhouse, who is a podiatrist and associate professor of clinical trials at Warwick University, told Medscape News UK, “The truth is, we don’t know if this approach works, which is why we are doing the trial.” Although carbon fiber inserts have been available and used within the NHS for years, there is very little evidence to show that these, or indeed any other insoles, actually do anything to help people with foot OA, according to Backhouse.

RELEVANT RESEARCH - PART 2
Research Study Shows Barbie’s Feet Have Gotten Flatter Over Time

There’s a scene from the 2023 movie “Barbie” in which arched feet step out of high heels onto the ground. And stay arched. It took the actress Margot Robbie eight takes to perfect the trick and it set off a social media frenzy almost akin to a collective gasp. For a group of podiatrists in Melbourne, Australia, that scene became fodder for a group chat — the angle of that arch, the weight on the balls of her feet. When Barbie’s heels eventually touched the ground, those podiatrists saw the beginnings of a research paper. “We were talking about how nice it was to finally see Barbie in flat shoes and to see Barbie making all of these different shoe choices based on what she had to do in her day,” said Cylie Williams, an academic podiatrist at Monash University.

Dr. Cylie Williams

Dr. Williams and two other podiatrists, including a male colleague, Ian Griffiths examined the feet of 2,750 dolls that were produced from 1959 to 2024. The researchers found that, over time, Barbie’s feet did indeed go through a revolution. In the early decades of Barbie’s life, 100 percent of the dolls had arched feet. In the last four years, only 40 percent did. “Employed” dolls were far more likely to have flat feet, while fashion-focused ones were more likely to have the extreme arch.

PRACTICE MANAGEMENT TIP OF THE DAY
6 Signs Your Practice Needs to Hire More Staff

Here are six red-flag trends, drawn from recent industry studies and management guidance, that signal it’s time to post a job ad or bring in contracted help.

3. Labor cost per visit rises faster than revenue.

A national Flash Report found labor now accounts for 84 percent of medical group expenses, with subsidies topping $300,000 per employed physician—evidence the current cost curve is unsustainable.

Action: Divide total compensation by total visits each quarter. If the ratio climbs while visit volume is flat, off-load clerical tasks to specialized staff or software so clinicians can see more patients.

RESPONSES/COMMENTS (PODIATRIC EDUCATION)
RE: Podiatry and Cardiac Escape Beats
From: Rod Tomczak, DPM, MD, EdD

It should be no secret, but most podiatrists won’t admit podiatry is dying. We are podiatry and many of us see the white light. The guys out here in the field are podiatry. If somehow all the podiatry alphabet organizations existed with no podiatrists seeing patients every day, there would be no such thing as podiatry. For some reason though, the alphabet people are trying to keep us from seeing that ECG that reveals prominent escape beats that often herald cardiac arrest.

APMA is at the top of the podiatry flow chart and it doesn’t want us to see the profession is deteriorating around us. The why they don’t want us to see is because the alphabet organizations think they have some miracle medication that will fix the inevitable cardiac arrest. We wish podiatry wasn’t crashing, but it is.

There are two important indicators that have surfaced of late. One was the PM News survey that revealed only 13% of the 690 respondents were happy with...

Editor's note: Dr. Tomczak's extended-length letter can be read here.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Jack Reingold, DPM

After reading dozens of viewpoints on the subject, I thought I would add mine. First of all, we are doctors. I’ve never had a patient in my 40 years of practice question whether or not I was a doctor or even realize what degree I hold. I graduated in 1979 and had the privilege of being residency trained. I had the ability to change the guidelines at three hospitals and added ankle privileges to all. At the last hospital, I sat on the orthopedic committee and said that I thought we should be judged by the same standards as any surgeon and there were no complaints. I noticed that in the orthopedic guidelines, they had one check off box for ankle, so I did the same (note, the applicant had to show competency.). The guidelines were passed without discussion!

Most of our residents are very well trained in foot and ankle pathology. Whether they choose to practice in the surgical arena or not is their choice. There are many orthopedists who have completed surgical residencies but choose areas where they are not in the OR. This is true of other specialties also. I believe that our residencies should train us all to be surgeons, but there should be fellowships for those who wish to excel in one specific area.

Those of us who had less than three years of residency will disappear with time. The three-year residency will be the standard, plus fellowships for many. In summary, we should all have training in all areas of pathology, and all of our residents should have the ability to practice in the direction they wish to go.

Jack Reingold, DPM, Solana Beach, CA

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Jeffrey Trantalis, DPM

It is difficult to have a profession when you can go to your local store and get orthotics. Seeing ads for Good Feet orthotics is not going to strengthen our profession.
