Feet Benefit from Shock Reducing Surfaces: MN Podiatrist

According to Rochester, Minnesota-based Mayo Clinic podiatrist Martin Ellman, DPM, “Structural changes in the feet, systemic diseases that affect the feet, and environmental factors can all lead to foot pain, and often these three factors overlap. Included in environmental factors is prolonged time on your feet. “Foot problems such as plantar fasciitis and metatarsalgia can develop from these environmental factors. Our bodies are designed to absorb shock. We have natural padding on the bottoms of the feet for cushioning."

Dr. Martin Ellman

But our feet are potentially exposed to a lot of stressors each day.” Aging is another factor, he points out. “As we age, the natural padding on the bottom of the feet may break down, so depending on an individual’s foot structure, health status, age, ability to absorb shock, and activity level, the type of flooring in the home can be a source of comfort or pain for the feet.” Dr. Ellman’s patients often tell him that they can walk on carpet without pain, but not their tiled kitchen or bathroom floors. Stone, ceramic and porcelain tile are highly durable and low maintenance, but also extremely hard.

Flip-Flops Have Come a Long Way: IL Podiatrist

Flip-flops have a bad reputation for messing up your feet. But, it turns out, there are plenty of flip-flops out there that can be supportive and comfortable. "Flip-flops have come a long way," Melissa Lockwood, DPM, a podiatrist at Heartland Foot and Ankle Associates in Bloomington, IL, tells Yahoo Life. "There are some really good ones out there now that don't look orthopedic, but still give you good arch support."

Dr. Melissa Lockwood

If you're looking for a new pair of flip-flops, Dr. Lockwood recommends steering clear of ones that don't have a strong bottom. Meaning, if you can easily bend the base of your flip-flop, that's an issue. "That will be almost like you're walking barefoot," she says. Instead, look for flip flops with at least a quarter to a half inch of arch support on the bottom. "They should not be completely flat," Lockwood says. Ideally, you also want something with a thick, sturdy thong, too, to help your toes get a comfortable grip.

PRACTICE MANAGEMENT TIP OF THE DAY
Why Social Media is Important to Your Practice - Part 4

Social media gives you insight into the needs of your patients. Don't ignore the opportunity to market your practice to specific patient populations.

4. Social media is the new "search engine."

Many people go to social media to find the places and services they are looking for; by looking for a business directly or by polling friends, family, and community members on social media.

QUERIES (NON-CLINICAL)
Query: Alcohol Injections for Neuroma

I would like some guidance from those practitioners who use alcohol injections for neuroma treatments. The answers to the following questions would be appreciated:

1. What is the cocktail injection using alcohol and the amounts per substance?
2. What is the amount of the cocktail injected per treatment?
3 When do you increase the % of alcohol from 4% to a higher amount—and what is that higher amount?
4. What, if any, are the side-effects with alcohol injections and how are they treated?
5. What are the total number of injections given and over how long a period?
6. What results are expected and when?
7. Is the series of injections repeated and if so when?
8. Where in relationship to the neuroma mass is the injection given?

RESPONSES/COMMENTS (NON-CLINICAL)
RE: APMA Virtual Advocacy Day
From: Alan L. Bass, DPM

Last week, on behalf of New Jersey, I had the opportunity to participate in the APMA Virtual Advocacy Day. I have to begin by thanking Ben Wallner, Director of Legislative Advocacy for APMA. He prepared a PowerPoint with all the speaking points regarding the HELLPP Act, ARC Act, VA Mission Act, and the Medicare Physician Fee Schedule, along with information on the Student Loan Reform Bill for the members of APMA to use when speaking to the members of Congress.

In addition to me, Larry Price, DPM and Laurie Clarke, our Legislative Liaison, represented New Jersey proudly and we met with our 2 senators and 4 different representatives to discuss these issues. They were all appreciative of the information we provided and many of them agreed to co-sponsor the bills that would benefit...

RESPONSES/COMMENTS (CODINGLINE CORNER)
From: Ron Werter, DPM

I suggest that the rejection is solely due to the -25 modifier. The insurance companies have decided to find another way to not pay for our services.

Question 1 - How can an E/M code which is supposedly included in a service pay more by itself than the service it is part of?

Question 2 - Recently an established patient returned with a new problem. I did my work-up including a diagnostic ultrasound (76882) and determined it was an inflamed 1st MP joint. She was in great pain, so I gave her an injection (20600), Not with US guidance. Insurance did not pay for the diagnostic US saying it was...
