NY Podiatrist Discusses Treatment of Foot Blisters

Foot blisters are small bubbles of liquid that develop on the outer layer of the skin and are typically caused by friction from your shoes or socks rubbing against the skin. Most blisters don't require special attention and should go away within a few days.

Dr. Suzanne Levine

“If you do develop a blister, it’s best to soak your foot and not pop the blister. I’ve seen so many people with infections,” Dr. Suzanne Levine a podiatrist in New York City told FoxNews.com. “Soak your feet in Epsom salt solution rinse and dry gently.” If a blister pops on its own, you should clean the area with mild soap and water, dry gently and then apply a topical antibiotic, Levine said.

CODINGLINE CORNER
Query: Meaningful Use Measure 10

Our specialty registry is telling us that we have to upload information on EVERY patient that we treat in 2016. This seems a bit excessive. Is anyone else hearing this?

Michael Giordano, DPM, Mt Kisco, NY

Response: It depends upon what information the registry is collecting. Off the top of my head, I cannot think of any single measure that would require data on every patient. Typically, a registry is collecting data on a specific clinical condition. In that case, they only should be requesting information on patients who fit the specific cohort.

Your registry SHOULD NOT be requesting patient data that is NOT related to the specific measure that they are collecting data for. To request that you send in more information than is needed has a number of ramifications in relation to HIPAA and Patient Privacy.

Michael L. Brody, DPM, Commack, NY

RESPONSES/COMMENTS (CLINICAL) - PART !
RE: Verrucae in Patient with Myasthenia Gravis
From: Sam Bell, DPM, Elliot Udell, DPM

I have had satisfactory results with Cantharone and having the patient apply 3% diclofenec gel at home.

Sam Bell, DPM, Schenectady, NY

If the patient is taking modulators to suppress his or her immune system, treatments aimed at generating an immune response might not be that successful. One question that you might ask is whether the verrucae are painful. If they are not causing the patient discomfort but are being treated just because "they are there," you might opt to not treat them at all. On the other hand, if the warts are causing the patient pain, then go down the list of multiple treatments that are available for resistant verrucae and hope for the best.

Elliot Udell, DPM, Hicksville, NY

RESPONSES/COMMENTS (CLINICAL) - PART 2
From: Steven J. Kaniadakis, DPM

Any wound should remain viable with the use of materials and topical treatments, and by essentially maintaining a moist environment. It has been my experience that using topicals, including topical antibiotic ointment, is a key way to help promote the advantages of healing. I think reports about such ointments preventing damaging enzymes and other proteins and other wound byproducts from leaving the wound are wrong. The benefits outweigh these reported risks.

I would use copious amounts of topical ointment and Adaptic. The Adaptic is placed first in intimate contact with all wound boundaries. Then the material is weighed down with the application of the ointment. It is unlikely that the ointment is going to last that long anyway. Any products should be allowed to wick off the wound. Ointments last longer than cremes or lotions. This maintains a longer viability of growing healthy tissues and red granulation from the margins and borders of the wound, rather than the growth of yellow fatty fibrosis, which turns into scar tissue.

Eschar is better than this type of tissue formation, and the ointment will help any developing eschar more easily slough off instead of adhering to the wound longer. Remember to bevel all edges of the wound. The eschar (dried blood) has valuable proteins and products of the platelets.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Neil Scheffler, DPM, Patrick J Nunan, DPM

I am not an attorney. In fact, I don't practice podiatry anymore, but for many years, I have believed that if a salesman in a store makes a diagnosis of a foot problem ("You have plantar fasciitis.") and/or treats a condition with, for example, orthoses, he/she is practicing medicine without a license. Podiatric medical boards have investigators who can be dispatched to substantiate these claims. The board can then ask the attorney general who works with the board to prosecute. I believe that a few prosecutions would shake up the industry sufficiently to minimize the problem.

Neil Scheffler, DPM (Ret), Estero, FL

People now have the ability to use a tablet to take a picture of their feet to make a 3-D printed orthotic. Several prominent podiatrists endorse the product that sells for $99. In this computer and social media age, along with vet high deductibles, patients will try these first.

Patrick J Nunan, DPM, Savannah, GA

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Robert Scott Steinberg, DPM

First off, if someone is naive enough to believe the shoe salespersons, even if they are CPeds, then it's buyer beware. But, if the person then presents to your office with the same or worsening pain, you have the opportunity to show off your expertise. If the patient feels the store ripped them off, you can write them a letter stating why the device was improperly made and sold (dispensed). Your patient can then try to get a refund and, failing that, forward their complaint, with your letter, to their state's attorney general's office. Your state law may also be of help, so contact your state society.

One other thing, be a secret shopper, and find out exactly what the store is claiming and what they are doing. Record the encounter.

Robert Scott Steinberg, DPM

YOU CAN'T MAKE THESE THINGS UP
Airline Launches Vibrating Shoes to Help People Navigate Towns and Cities

A 'smart-shoe' has been unveiled which claims to help guide people around unfamiliar places. Sensors in the sneaker shoe vibrate to tell people which way to turn when walking the streets. The sensors are connected to a user's smartphone via Bluetooth, which in turn relays GPS data back to the shoes.

easyJet "smart" sneakers

European budget airline, easyJet, is behind the venture and says it hopes to sell the vibrating sneakers direct to passengers onboard flights. The airline unveiled the 'Sneakairs' at an event in Barcelona where testers roamed around the city unaided by maps or looking at their phones.
