PM News is saddened to report the passing of David Shofler, DPM, MSHS, 40, of an apparent heart attack. Dr. Shofler received his podiatric medical degree from the Ohio College of Podiatric Medicine and a Master of Science in Health Sciences degree from the Western University of Health Sciences.

For those of you who have taken the time to read the white paper as well as the Resolution, I think you will find that many of the comments that have been made by some PM News readers are incorrect. Let me give you some of the tenets of the Resolution:
 
1. The document has been developed to allow GRADUATES of CPME-approved podiatric medical schools to have access to take the USMLE. Remember that the NBME controls who can sit for the USMLE, and at this time, podiatric medical students are prohibited from sitting for the exam. The resolution asks for the AMA to evaluate if the CPME is COMPARABLE to the LCME, not to replace the CPME. It says GRADUATES, without specifying present students and residents. So, potentially, anyone who is a graduate from a CPME-accredited podiatric medical college, would be eligible to...

Since the backslapping, and kudos, and misinterpretation, and queries about existing DPMs taking the USMLE have begun, I feel compelled to state my take on it. Dr. Allen Jacobs' very accurate, but hidden assessment is one that I 100% agree with. The AAOS and AOFAS seem to have put APMA and ACFAS into a full Nelson without any resistance whatsoever and forced them to cry "uncle." Any DPM with an iota of self-respect must condemn this statement in its entirety. For those who support it, ask yourselves the following:
 
1) What do you think will happen if the AMA takes up this resolution to "study" our education from the top down to see how it compares to that of an MD/DO? I'll tell you....it will be disemboweled pure and simple. You will see how the retrofit about...

As a younger member of this profession, I empathize and understand the frustrations of my mentors and professional leaders that lived through the "civil rights era" of podiatry in the '80s and '90s. If I have seen further, it is by standing on the shoulders of giants. But as a generation X member of our profession, I am truly excited and grateful for APMA, ACFAS, and others involved regarding this resolution. My generation has numerous opportunities in practice now, not limited to only private podiatry practices, and the majority of the next generation will be practicing alongside allopathic colleagues, shoulder to shoulder.
 
Dr. Lowell Weil, Sr. (my fellowship director) used to sneak into AOFAS/AAOS meetings due to his degree, but now, he consistently has fellows lecture and win manuscript/poster competitions at these meetings. Podiatrists are lecturing, teaching, and learning alongside allopathic and osteopathic colleagues, and this resolution is another step forward in our natural progression as a profession.

I agree with Dr. Smith. In fact, as insurances tighten, I am doing more and more office surgeries for self-pay rates and I am doing better financially with less paperwork. It has benefited my practice and my income to practice without the total reliance on insurance. I perform 90% of my procedures in the office now with more and more patients opting for self-pay. 
 
All the moves to obtain parity to MDs will cost us dearly. If we want success, we should follow the dentist path. They get paid and are paid very well. The more self-pay and inadequately insured patients I see, the better I am doing. Podiatrists will do better financially the less they rely on insurance and suspicious buzzwords like parity.

David Armstrong is spot on with his analysis concerning the USMLE. My son has now chosen the path to become a podiatrist in the near future and will graduate from UCLA as a bioengineer. No question when the time comes to pass this exam, he will, and all of us can be proud that the profession is moving forward. To those naysayers, don’t get lost in the words of a piece of paper; the hard facts are that we are getting there in parity with the MDs and it is only a matter of time.
