NYCPM Students Receive CPR and Defibrillator Training
 
For the first time, students at the New York College of Podiatric Medicine (NYCPM) are learning the critical, potentially life-saving skills of cardio-pulmonary resuscitation (CPR) and use of defibrillators from Training for Life (TFL), an American Heart Association-authorized non-profit organization that focuses on teaching these and similar skills. On May 8, 50 members of the class of 2016 learned these practices from TFL instructors at the College; during the session, the students practiced on life-like dummies.

Pets Can Cause Athlete's Foot: NJ Podiatrist 
 
While you probably know that wearing shoes on wet, germ-filled surfaces will decrease your chances of contracting the infection, you might not be aware that a wide variety of fungi can cause athlete’s foot, says Richard Braver, DPM, a podiatrist at Active Foot & Ankle Care, LLC in Englewood and Fair Lawn, New Jersey. And these fungi can lurk almost anywhere.

Pets shed tiny specks of skin called dander. No biggie, right? For your toes, it is. Fungi easily travels on your pet’s dander throughout your home, Braver says. As you walk barefoot through your rooms, the dander can attach to your soles and cause an infection. To keep your feet fungi-free at home, regularly wash your pet with an anti-dander shampoo like Dander-Out to minimize flakes, suggests Braver. Also, frequently vacuum your floors and wear slippers.

New Zealand Podiatrist Not Surprised by Vibram Settlement
 
Kiwi experts are not surprised that a manufacturer of toe-sock running shoes has revealed that there is no scientific proof that wearing its product has added health benefits. Vibram, the makers of FiveFingers running shoes which are designed to mimic the effect of running barefoot, settled a $4.35 million class action lawsuit in the U.S. over the false claims. As part of the settlement, customers who purchased pairs of the shoes may be eligible for a partial refund, and the firm has promised it will not make any more claims without scientific evidence to back them.

Auckland podiatrist Simon Speight said the legal action did not come as a surprise, as popular theories promoted structured shoes that cushioned the feet for protection. "They are consistent with harm-causing features of footwear," he said. "Running shoes, by their very nature of design, are there to prevent injuries from happening and to improve performance. All of our theories, for the most part, restrict movement in the foot; that's what tends to help."

Revocation of OH Podiatrist's Enrollment Affirmed
 
In Jeffrey Lubell, DPM v. CMS (HHS Departmental Appeals Bd., Civil Remedies Div., No. CR3192, 4/7/14), ALJ Sickendick affirmed the revocation of a podiatrist's Medicare enrollment and billing privileges based on his non-compliance with 42 C.F.R. § 424.57(c)(7)(i)(C) (supplier standard 7), because his facility was unstaffed and inaccessible to the public during his hours of operation. 
 
The ALJ rejected the petitioner's argument that his enrollment should not be revoked because the dates on which he closed his facility fell during "the most Holy Days in the Jewish and Christian calendars." Supplier standard 7 makes no exception for religious holidays, the ALJ said, and the petitioner presented no evidence that whatever signage he did have posted stated that his facility was closed for religious holidays.

UK Podiatrist Receives 15 Months in Jail for Lying About Credentials
 
A man who made 1,000 applications using false details to get a job as a doctor has been jailed for 15 months. Bai Bai Ahmed Kabia, 46, of Hathersage Gardens, South Shields, Tyne, and Wear, was found guilty of 12 counts of fraud. An investigation was launched when it was found that a sample of applications contained misrepresentations about qualifications and employment history. The court was told he trained as a podiatrist and worked for some time on a self-employed basis.

Query: 5th Digit Fracture
 
A 35 year old healthy, active female presented with a painful right 5th digit on 3/18/14, after hitting it on a TV stand on 3/8/14. She knew it was broken when she hit it, but wanted to make sure. Her past medical history is unremarkable. Physical exam indicates normal neurovascular status right foot. A weight-bearing MO view shows a displaced spiral fracture on the proximal phalanx 5th digit.

She was put in a rigid sole post-op shoe because surgery wasn't an option for her after the initial visit. Seven weeks later, x-rays haven't improved, and her toe is still swollen and mildly sore. She is now consenting to surgical intervention. What fixation option would be the best for her?

What is the "G" code that is billed with PQRS Measure 163 for diabetic patients? I tried G9226 and that did not work.

Response: I believe G9226 is now an all-inclusive code for diabetic foot exam, neurological check, and shoe evaluation. If you used any of the old codes too, they will probably get kicked out. I've been using G9226 since March without problems. You need to reference the 2014 CMS rules and regs.

Dr. Markinson writes that a squamous cell CA should not be treated topically without the patient first having had a full body scan and studies to determine if there is spread to the lymph nodes. In light of the fact that the wife of a patient of mine died from a squamous call carcinoma that metastasized, it is easy to see where Dr. Markinson is coming from. On the other hand, I biopsied a suspicious lesion on a patient's foot last spring, which turned out to be a squamous cell. The pathologist at the lab just recommended full resection. The patient was referred to a dermatological surgeon who specializes in skin cancer and eventually went for Mohs surgery, but was never advised to have nodal or full body exams.
 
I suspect that  the doctors who saw my patient and Dr. Markinson are all correct. Perhaps not every squamous cell carcinoma requires a general body work-up. Does size and depth of the lesion play into this decision? If so, perhaps Dr Markinson and some of the pedal oncology experts among us can comment on what those parameters are.

For Meaningful Use, you must record the vital signs ONCE per reporting period, not necessarily every visit. For 2014, the reporting period is a calendar quarter. Find out which calendar quarter you will be reporting for and take the vitals for each patient once during the reporting period.
 
It is important to realize that if you collect this information from the chart in the nursing facility, this is a valid method for meaningful use. If you are looking to meet the PQRS measures, you must measure the vital signs yourself.
