After 40 Years, AZ Podiatrist Takes on a Partner: His Niece 
 
For nearly 40 years, podiatrist Dr. Edward Wiebe has been helping Flagstaff residents put their best foot forward at Flagstaff Foot & Ankle. Now his niece, Dr. Rachel O’Connor, is preparing to follow in his footsteps. O’Connor moved to Flagstaff a year ago to help Wiebe with his practice and learn the ropes of running a small business. She is a graduate of the University of Arizona who would visit her uncle in Flagstaff during the summer. “I saw what my uncle was doing and wanted to get into medicine,” O’Connor said.

“I wanted to be in a profession that was more upbeat and conducive to having a family. Something with a good mix of office and surgery work.” O’Connor found that mix in podiatry, where most injuries and conditions can be fixed without surgery and are not life-threatening  if they can be caught early. “Basically, we deal with everything below the knee, from bones to skin,” Wiebe said.

Leather Shoes Could Cause Problems in School Children: NZ Podiatrist
 
Black leather shoes, mandatory at many New Zealand schools in winter, are causing harm to children's feet, say podiatrists. Most intermediate and secondary schoolchildren around the country switched this week to winter uniforms and the black leather lace-ups that go with them. Auckland podiatrist Caron Orelowitz, of Podiatry NZ, said the shoes could cause major problems for young feet. "The foot is constantly developing and the bones are soft," Orelowitz said. "They need as much cushioning as possible or they could end up with other problems."

She said it was particularly a problem for children running around at lunchtime. "If they're running in leather shoes with rubber soles, there's no shock absorption. They can develop knee, heel, and foot pain." Orelowitz said it would be better for children to be in cheap sneakers than to run around in leather school shoes.

Marta Rendon: There is a new technology called SRT (superficial radiation technology), a low energy radiotherapy that goes no deeper than the thickness of the skin. In reality, this is a 50 year-old technology that has been improved upon to make this available in the office setting. In those patients where the podiatrist has confirmed non-melanoma skin cancer, and who are poor surgical candidates, the cancer can be treated safely by this non-invasive method.

Query: Taking Blood Pressure Readings on Assisted Living Residents and Nursing Home Patients 
 
I work for a family practice physician group which recently implemented an EHR system in their office, and for the physicians and podiatrists who see patients in nursing homes and in assisted living facilities, including patients seen in memory care units.They requested that my assistant or I take a blood pressure reading on every patient at every visit simply for “meaningful use”, so they don’t lose money from Medicare (although they didn’t say this directly).
 
I see at least twenty patients every day (mostly for painful mycotic hypertrophic toenail debridement). I think that this request is ridiculous. From my point of view, you can hurt these 90-year-old patients because they are mostly frail. The pressure from the cuff in taking unnecessary blood pressure readings can cause further skin irritation and pain. I have seen patients with hemorrhages already underneath their skin at this age. These patients usually have their blood pressures taken daily or weekly anyhow. I say that this is “BS”, and I refuse to do this. What do my colleagues think about this just to “be compliant” with this one “meaningful use” point?

Query: Security Risk Analysis
 
How are podiatrists performing the security risk analysis? Who is doing this for us, or are we supposed to do this ourselves? If we do it ourselves, how do we know if we get it right? I have not seen any articles discussing how to perform a security risk analysis.

There are several topical treatments for both basal and squamous cell carcinoma, PROVIDED that the lesions are singular and have not spread to lymph nodes. The poster does not say why the patient is homebound. In any case, before treatment is begun, the patient needs a total body survey and lymph node exam. In my opinion, topical treatment of these cancers is best left to those who do it regularly, or if in this situation, it is possible for the DPM to consult with a dermatologist for advice, I would do so. I would definitely not fly solo, especially since the full evaluation of this may raise scope issues for the DPM. Kudos for the clinician who did the biopsy to get the diagnosis.

1) I normally take out a bit more nail than the next podiatrist to help assure no recurrence. As we all know, that area will come together nicely post-op. 
2) The crucial modification I came up with is to use a curved Kelly to spread open the matrix area to make abundantly sure that the phenol (or whatever you use) gets to the area you want to destroy so you don't get a recurrence. 
3) It is crucial to apply phenol to the nail bed area distal to the matrix because there are matrix cells present in this area at times. 
4) I use an English nail nipper to excise any proud flesh/granuloma and then apply phenol to that area as well. 
5) I am always exceedingly careful to not get phenol anywhere else on the toe.
My success rate is literally 99.99%; meaning, I had one recurrence since 1974.

I switched to using NaOH a few years ago. I find there is a little more discomfort the night after the procedure for the patients, but they are mostly resolved in 2 weeks. I have not had any recurrences yet. The NaOH is applied for 60 seconds, once only and then neutralized with acetic acid. Here is an abstract comparing the two agents:
