PODIATRISTS IN THE COMMUNITY - PART 1 |
Two Podiatrists Join NH Practice
Core Podiatry welcomes two new physicians, Edward Carver, DPM and Vasilios Lirofonis, DPM, to provide services in Plaistow and Exeter. Dr. Carver completed a surgical residency at the Maryland Podiatric Residency Program, Bon Secours Hospital in Baltimore, MD. He is also board-certified in the prevention and treatment of diabetic foot wounds (Cert/DFW).

PODIATRISTS IN THE COMMUNITY - PART 2 |
TX Podiatrist Appears as "Fan Guest" on New Zealand Radio Show
Dr. Gary Prant is a podiatrist at the Arbor Foot Health Center. The good foot doc also is an amateur radio operator. He loves to contact faraway folks. Prant likes to make contact with people from other lands.

For about five years, Prant also has pursued his one-man effort at international understanding by playing a New Zealand radio show, by way of the Internet, throughout his office. The Two Robbies airs weekdays 6-10 a.m., Auckland time. Prant’s podiatric assistant Paula Allen recently emailed the Robbies to tell them they have fans at the Arbor Foot Health Center. So thrilled were one or the other or both of the Robbies that they called Prant’s office on April 27 and aired the chat on April 29.

OUTSIDE INTERESTS |
MI Podiatrist is Hooked on Needlepoint
Typically, one wouldn’t associate a medical convention with learning to needlepoint, but for 86-year-old Dr. Donald Ketai, a retired podiatrist, such an assembly held in Boston over 30 years ago is exactly what led to his longtime passion for doing embroidery work over canvas. His wife was in bed at that time with a back problem and he had nothing to do. "Our daughter-in-law was doing Bargello (flat-stitch embroidery) on a canvas to make a pillow. I asked her to take me to the yarn store and get me started on one. They started me out with a canvas and instructions of what to do.” Ketai finished his project and found himself hooked on the age-old form of needleart.

Inspired by the thought of having a hobby that would help keep him “away from the refrigerator,” Ketai has done numerous needlepoint projects, many of which he gifted to family members, including the one he presented to his daughter at her engagement party.

SUCCESS TIPS FROM THE MASTERS |
Bret Ribotsky: How do you treat turf toe?
Jeff Ross: First, you x-ray and look for a separation of the sesmoids. If no acute injury is present, order an MRI. Non-weight-bearing is most important if there are any sesmoid injuries; then a bone stimulator (dual wave).
|
Dr. Jeffrey Ross |
Bret Ribotsky: How do you convince patients that all this is needed for what they assume is a small injury.
Jeff Ross: I tell them, that it will be a big deal when you can't wear high heels, can't get on the treadmill, and can't run 3-5 miles.
|
Dr. Luke Cicchinelli |
Meet the Masters is broadcast each Tuesday night at 9 PM (EST). This week's guest is Dr. Luke Cicchinelli. APMA President Dr. Phill Ward will be the guest on May 19th. You can register for future events by clicking here

HEALTHCARE LEGISLATION |
Diabetes Prevention Act Introduced
The American Diabetes Association (ADA), American Medical Association (AMA), and the YMCA have introduced the Medicare Diabetes Prevention Act (S. 1131/H.R. 2102). Sponsored by Senator Al Franken (D-MN), Senator Susan Collins (R-ME), Representative Susan Davis (D-CA) and Representative Peter King (R-NY), the legislation would provide coverage for the National Diabetes Prevention Program (National DPP) to Medicare beneficiaries at high risk of developing type 2 diabetes. Avalere Health estimates the legislation will save $1.3 billion over 10 years.
The AMA is partnering with the YMCA of the USA to increase the number of physicians who screen and test patients for pre-diabetes and refer them to diabetes prevention programs offered by local YMCAs that are part of the Centers for Disease Control and Prevention's National DPP recognition program.

CODINGLINE CORNER |
Query: Medicare Denial of CPT 29515
Why is Medicare denying CPT 29515? The EOB says, CO-4, "The procedure code is inconsistent with modifier used." We are using the appropriate right or left foot modifier. We are not bundling this service with anything else.
Lina Diaz, DPM, Englewood, NJ
Response: There may be other codes on the EOB that are prompting this to appear. CO-4 is a reason code. Please look at the EOB for the Remark codes (to the left under the patient's name) and see if there is another comment.
You are correct - "RT" or "LT" - is appropriate for this CPT code, but I believe something else on the EOB has more information.

RESPONSES/COMMENTS (CLINICAL) |
From: Eric M. Hart, DPM
I have had very good success with extensor tendon lengthening, dorsal fifth metatarsophalangeal joint capsulotomy, and proximal partial syndactylization of the fourth webspace. The cosmetic appearance is excellent and the syndactylization isn't visible unless spreading the toes. Ask them about family members afflicted with the same deformity. I've done several from the same families.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1 |
From: Jack Ressler, DPM
The answer is simple, John. The introduction of ICD-10 codes would save Medicare a substantial amount of money, so they think. Once the system gets going, they can audit Drs' notes to see if the coding matches the content of the chart. If they discover an error such as the patient's infection on the medial side of the right hallux was actually caused by the bite of a sparrow instead of a crow, boom, REFUND demand letter.
Although ICD-10 codes are being used and touted as the world-wide accepted diagnosis coding system, I treat patients who live in many different parts of the world, and they almost all say that our medical system is by far the best. I find in my practice, I'm doing several procedures on these patients for cash because the wait period to have "elective" procedures done is ridiculously long. Forget about an MRI! That would never be allowed.
The most important information I can add to this tongue-in-cheek query is that I have heard from a couple of sources that there is no way the ICD-10 codes will be approved before the 2016 Election!
Jack Ressler, DPM, Tamarac, FL

RESPONSES/COMMENTS (NEWS STORIES ) - PART 2 |
From: Daniel Chaskin, DPM
The prosecutors in this case incorrectly argued that the material gain needed in the element of fraud was the economic difference between an office visit and a nursing home visit. Since this was performed in a room in the nursing home, Place of Service 99 would have been a proper place of service with no economic difference between the allowance for an office visit vs a place of Service code 99.
Daniel Chaskin, DPM, Ridgewood, NY
