Dry, Flaky Feet Could Indicate Thyroid Disease: IL Podiatrist
If you have dry, flaky feet, It could be thyroid problems, especially if moisturizer doesn’t help. When the thyroid gland goes on the fritz, it doesn’t properly produce thyroid hormones, which control metabolic rate, blood pressure, tissue growth, and skeletal and nervous system development. “Thyroid problems cause severe dryness of the skin,” says Marlene Reid, DPM, a foot specialist in Naperville, IL.
|
Dr. Marlene Reid |
“When we see cracking on the feet, or if moisturizer doesn’t improve dryness over a few days, we usually refer patients to their primary doctor to make sure their thyroids are okay.” Brittle toenails can also signal thyroid complications.

Flip-Flops Expose Feet to Harmful Bacteria: NY Podiatrist
Flip-flops are more a matter of fashion sense—or lack thereof. But here is where things get kind of dicey: “I can remember walking up a steep mountain in California and seeing other climbers wearing flip-flops, and falling out of them,” said Dr. Corinne Kauderer, a Brooklyn-based board-certified podiatrist and former president of the American Association for Women Podiatrists (AAWP).
|
Dr. Corinne Kauderer |
“Flip-flops may be better than going barefoot,” Kauderer said, “but even so, much of your foot is exposed to dirt and bacteria, which increases your chances of getting an infection.” In fact, a 2009 Today Show report found that more than 18,000 types of bacteria were discovered on one pair of flip-flops, including bacteria traces from fecal matter. “For those with diabetes, it’s especially dangerous, since they are at an even higher risk of infection,” Kauderer said.

STATE HEALTHCARE LEGISLATION |
IN Podiatrists to be Protected by Health Shield Law
Health professionals who volunteer their services in Indiana are about to be protected from being sued. Governor Mike Pence was at the Shepherd Community Center in Indianapolis Tuesday morning where he signed the bill into law. The law protects physicians, physician assistants, advanced nurses, podiatrists, and optometrists who volunteer their time to give basic treatments to those who would otherwise not have access to healthcare.
The new law includes protection for volunteers who provide services such as sports physicals, routine medical, eye, and dental exams, and suturing of minor lacerations. The law takes effect on July 1st.

MEDICAL RESEARCH |
New Optical Device to Diagnose Basal Cell Carcinoma
A new optical technology called multiphoton microscopy (MPM) could allow for non-invasive, painless, and rapid diagnosis of basal cell carcinomas (BCCs) in the clinic, according to a small pilot study published online April 24 in JAMA Dermatology. The new technology could potentially reduce wait time from diagnosis to treatment. "This study demonstrates, in a limited patient population, that non-invasive in vivo MPM imaging can provide label-free contrast that reveals several characteristic features of BCC lesions," write first author Mihaela Balu, PhD, and colleagues from the University California, Irvine, and Saarland University, in Saarbrucken, Germany.
MPM takes advantage of chemical compounds in the skin, such as keratin, collagen, NADH, FAD, elastin, and melanin. When excited by a laser, these compounds emit light that can be used to create high-definition, 3-D images of the skin. In particular, NADH and FAD concentrations provide an indication of metabolic activity, which differs in normal cells compared with tumor cells.

QUERIES (MEDICAL-LEGAL) |
Query: Wound Clinic Conundrum
I work at a hospital-based wound clinic. I am not an employee. I am an independent contractor. I bill for my services to the patient's insurance and get paid that way. I recently found out that the wound clinic has also been billing for my services, essentially double billing the insurance. I'm not sure if this was done purposely or in error. Most of the wound clinic's billings were denied payment because I submitted my claims first. Apparently, the wound clinic then bills the patients for these denied claims, sometimes totaling thousands of dollars. What should I do about this?

RE: Legitimacy of Distinguished Master Consultant in Podiatric Surgery
Has anyone heard of a certificate as a Distinguished Master Consultant in Podiatric Surgery? I received a letter from the National Institute of Medicine in Washington, DC saying that I've been selected. Is this legitimate? The letter was signed by an Arthur Windsor, MD.
Bret Ribotsky, DPM, Boca Raton, FL
Editor's comment: PM News does not provide legal advice. According to the website ripoffreport.com, a firm operating from this office has been sending similar letters to physicians. A Google search finds no record of an Arthur Windsor, MD. Readers are advised to scrutinize any unsoliticed honors received in the mail, particularly if they require a "registration fee."

QUERIES (EMR) |
Query: EHR Integrity Questions
How many times should patient EHR and billing data be checked to ensure its integrity? What issues could arise that could corrupt data and hence their respective backup? If the EHR and billing data cannot be restored for some reason, then what is the next best recourse? What issues are there with respect to HIPAA regarding possible irreparable data loss?
Joseph Borreggine, DPM, Charelston, IL
Response: "How many times should patient EHR and billing data be checked to ensure its integrity?"
This is a very interesting question and there is no direct answer to it. A loss of data integrity can be difficult to detect. The corruption of a single patient entry may not cause the system to crash, so you may look at a patient chart and not even realize that the data has been damaged in some manner.
My recommended solution is a very exhaustive back-up plan. The following back-up plan assumes your office is...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A |
RE: EPAT
From: John Moglia, DPM
I would like to add to the ESWT discussion. EPAT is a less expensive alternative To ESWT. While it may take up to 6 treatments to be effective, I have seen dramatic improvement and "cures" in many recalcitrant cases (over 6 months ) treated elsewhere with conventional therapy. I put "cures" in quotation marks as there is no cure for those patients who return to the repetitive stess of running or an occupation that causes re-injury. Patients are willing to pay a reasonable fee as an alternative to surgical treatment or painful cortisone injections, with no guarantee of "cure" either. I have no financial interest in an EPAT company and won't even mention the name of the company I use.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B |
RE: EPAT
From: Dale Feinberg, DPM, CPed
I Just got off the phone with a past patient who is a dancer. When asked how her feet were doing, she stated that they were great and that she is sorry she didn't have EPAT on her second visit two years ago. We tried the plethora of conservative treatments, and EPAT worked the best. I am not sure why there are negative reviews for this treatment that is a mainstay of Olympic caliber athletes. I have used this treatment modality for well over five years; in today's medical marketplace, you should too.
