APHA Podiatric Health Section Charts Decades of Group’s History

As podiatric health deals with foot health, it is only natural that a major part of The American Public Health Association's (APHA) Podiatric Health Section history is a huge traveling foot. The oversized papier-mâché foot, filled with shoes and giveaways, made its way around Washington, D.C.-area schools in the 1970s to teach children about foot health, a push to educate the nation about podiatry documented in a 40-year history written by former Section Chair Arthur Helfand, DPM.

The history is an update of a historical section previously published in 2006, said Helfand, who served as the Section’s first chair when it officially formed in 1972 at the APHA Annual Meeting. "It’s a wonderful thing to see this Section survive and, hopefully, it will survive to 50,” Helfand told The Nation’s Health. Helfand said he compiled the history using his own personal files and documents from the American Podiatric Medical Association.

Obesity Puts Feet at Risk: Aussie Podiatrist

Research has shown that excess weight can raise the risk of not-so-fun conditions like tendon inflammation and osteoarthritis. “Simply by being overweight, you can put your feet at risk,” says podiatrist Phil Vasyli. “Just as intensifying activity can increase force on your feet, so can adding a large amount of weight, which puts additional stress on the foot with every step you take.”

If you’re overweight, but concerned about getting moving, try starting with low-impact activities like swimming, water aerobics, the rowing machine, or yoga. When beginning or intensifying a fitness routine, start from the ground up and make sure your sneakers offer enough support to keep your feet healthy as you “jumping jack” your way towards your wellness goals, says Vasyli.

Shire Partners with AAWC to Provide 100K for Fellowships for 2014-5 

Shire has announced that it is providing the Association for the Advancement of Wound Care (AAWC) with an unrestricted grant to be used for fellowships that will advance the science  of wound care. This unrestricted grant from Shire of $100,000 will be split to sponsor two individual fellowships for the 2014-2015 academic year. The fellow selection process will be administered by the AAWC, and each fellowship will be open to DPM, MD, and DO candidates to support a post-residency or fellowship program focused on multi-disciplinary wound healing and tissue preservation.

Roy Lidtke: What our research has shown is that a very flexible shoe will allow more motion to occur and thus decrease forces at the knee. Research that I have been involved with at Rush University has lead to the OA shoe. This specifically-designed flexible shoe has shown to decrease knee forces up to 20%.

This patient presented with a pedal skin reaction to Doxil, which is a chemotherapy drug. Skin reactions are a known side-effect of this pharmaceutical. The skin reaction developed within days of taking the drug.

Initially, the redness was far more severe and  her oncologist controlled it with topical steroids. The scaling and cracking remains, and we are managing it with a 30% urea-based cream in addition to the topical steroid.

Judging by the lateral x-ray, it appears that she may have a functional hallux limitless. It does appear that she has diminished first metatarsal declination angle due to hypermobility of the first ray. It would be interesting to know what her range of motion is with the first metatarsal loaded. Her deformity does not appear to be radiographically or clinically applicable for a joint implant. This appears to be more of a functional stage I hallux limitus. Your original concept of performing a decompression osteotomy would make more sense. This can be performed utilizing minimally invasive procedures.

Pre- and Post-op X-Rays

I recently had a similar case that I corrected  by performing an 1.5 cm. incision medially and performing a decompression osteotomy on the first metatarsal phalangeal joint using a guide pin. This small incision also allowed me to reduce the dorsal exostosis, utilizing a power rasp. I was able to perform the surgery without the need for fixation. The patient is completely pain-free. Above are the pre-op and post-op x-rays. I angled the osteotomy to get plantarflexion of the first metatarsal and also addressed her equinus deformity by endoscopic gastroc recession. Addressing the equinus deformity will give you improved plantarflexion of the first metatarsal. Most of these deformities are associated with gastroc equinus, which should be addressed.

As our editor pointed out, this issue is between the anesthesiologist and your patient, not you. I don’t think you can refuse to perform additional surgery because your patient stiffed the anesthesiologist, but you can warn your patient that when you book a second surgery, it might be impossible to get an anesthesiologist to agree to involve themselves with your patient under the circumstances. While it might be entirely possible to perform your subsequent surgery with only local anesthesia, it would be your call to decide whether that’s in the patient’s best interest. If he’s gonna squirm around on the table or work himself into a compromised state of health because he’s not sedated, you can certainly insist on doing your surgery with an anesthesiologist present. If you can’t get one, then you’re unable to do the surgery.
 
You may want to share a story I had with that anesthesiologist.  Several years ago, the same thing happened to me where the patient pocketed insurance reimbursement and wasn’t paying me. I called the patient and told him at 4:30 PM today, I’m making a call to the IRS to inform them of this unreported taxable income he received which will almost certainly result in an audit. I received the entire amount in cash that very day.

RE: Purchasing a Digital X-Ray System (Michael Brody, DPM)

In the interest of getting closer to the bottom of this debate, I have a few questions. Dr. Brody states, “Every system I have seen at podiatry shows will provide you with quality diagnostic images.” If this is the case, why wouldn’t you just find the one with the lowest price and purchase that? I have difficulty accepting that “image enhancing” after the fact is the “power” in digital x-ray. A poor image cannot be enhanced to become a great image.
 
And, why would anyone choose a more cumbersome CR system to “process” films, which takes more time and effort than a DR system? I have never understood the appeal of CR systems.

There is no level 3 or higher study to support that Achilles tendon ruptures are associated with having ankle equinus. Quoting one level 4 study is not very assuring. Furthermore, there is also no evidence that stretching and other rehabilitation techniques are helpful. In fact, the mainstay of treatment for Achilles tendinopathy (but not ruptures) is eccentric strengthening. All NBA teams have a rigorous conditioning and rehabilitation team that utilize these techniques. Unfortunately, ruptures are just part of the sport. If stretching the Achilles is so helpful, why do virtually all patients who present with Achilles tendinopathy state they already have been stretching when they come in for treatment?
