MI Podiatrist Explains Why We are the Leaders in Chronic Wound Care
 
According to Conway McLean, DPM, "Approximately 75% of all chronic wounds are venous ulcers, nearly always occurring below the knee. Another sizable category of non-healing skin ulcers occur to people with diabetes. These are typically found on the bottom or sides of the foot. Wounds that won’t heal because of inadequate arterial blood flow are called ischemic ulcers and are primarily found on the toes, foot, or lower leg. Consequently, since the great majority of chronic ulcers occur below the knee, podiatric physicians have become leading providers of wound care."

"An important part of the training for a podiatric residency program is devoted to their care. There are numerous seminars devoted to the topic. Podiatry has long had a concentration on diabetic foot care, a population frequently experiencing these challenging sores. Wound care is an important part of many podiatric practices. Because a large part of the practice of podiatric medicine is surgical, we are comfortable wielding a scalpel. This is important to wound care since the removal of dead tissue is often necessary to heal these wounds. Studies reveal the more often an ulcer is debrided, the faster it heals," says Dr. McLean.

CA Podiatrist Retires as Residency Director of LRMC After 37 Years
 
Lawrence Hodor, DPM has retired after serving for 37 years as residency director of Lakewood Regional Medical Center (LRMC). Dr. Hodor graduated from the California College of Podiatric Medicine in 1982 and completed a podiatric residency at Lakewood Regional Medical Center in Lakewood, CA, and was selected to head up their podiatric residency program four years later.

NY Podiatrist Discusses Financially Egalitarian Dating and Marriages
 
In 1972, husbands were the primary or sole breadwinners in 85% of U.S. married households, while 5% of wives made all or most of the money, and 11% of married couples had equal salaries. According to the Pew Research Center, things have changed quite a bit in 50 years. According to new Pew data, 55% of husbands are now the primary or sole financial supporters (a 35% drop). Financially egalitarian marriages have risen to 29% (more than a 160% increase), and 16% of married women provide the lioness’ share of family finances (a 220% increase).

Rabbi Jack Cohen, DPM, who mentors Jewish couples in dating and marriage, has seen dramatic changes in the dating landscape, which are connected to the data Pew is tracking. Especially among older Jewish singles, women often earn more than men, in Dr. Cohen’s experience. Cohen, who is also a podiatrist, says dual incomes come in handy during inflationary times, and a dual-income couple can be a “fantastic dynamic.” But he cautioned that working couples can often be very busy and lose focus on their commonality: “They drift as human beings.”

Regarding the unidentified lesion on the dorsal aspect of the foot, this lesion needs to be biopsied as soon as possible to rule out carcinoma. With its rolled border, I would guess that it is a basal cell carcinoma.

We also have been subject to Ciox Health's harassment. We fax back an invoice requesting $30 per chart, citing California Health & Safety Code section 123110 - allowing the fees for processing and preparing records.
 
We instruct staff to not answer any questions until payment is received. We fax the same letter each time we receive the same request. Payment is received each time. It won't stop the requests but at least we're being fairly compensated for it.

Ciox requests can take 10 minutes to respond to. Ciox presents a portal to upload chart notes. Your EHR should be able to save a chart note in PDF format. Make a folder on your desktop, and save all the .pdfs to the folder. Log onto the portal and upload all. It is a very quick and efficient way. 
 
After you are done, delete the folder from your desktop to comply with HIPAA rules.

I find the debate by Dr. Spinner and Dr. Udell significant, especially when combined with the recent APMA letter and the Summit at the House of Delegates. Issues of this sort are multifaceted and quickly become emotional. There are no easy solutions. Blog forums are not helpful in resolving an issue of this magnitude. 
 
All parties involved must commit personal time and organizational funds to meet and present their issues and possible solutions. APMA has done the groundwork at the Summit. I participated and chaired many of these forums during my political career. They are successful if everyone commits to the process. 
 
The next steps include the meeting(s) noted above with an appropriate facilitator in order to create compromise and agreement. The task may not be easy but the profession needs a solution.

Dr. Spinner’s letter regarding the resignation of ABPM directors was so inaccurate I don’t know where to begin. Observing the behavior of all the “players” recently that revolved around CAQs, the APMA summit, to the Oregon ruling regarding surgical privileges, I find Dr. Rogers agenda to be one of the few that I would call admirable and without being self-serving. This is the exact opposite of “selfish”.
 
Dr. Spinner writes, ”I believe board certification should not be a gatekeeper to hospital staffs.” That’s exactly what Oregon tried to do. And ABPM fought. The Oregon Board of Medicine wanted board certification to dictate privileges, not individual hospitals. This would limit the right to do procedures to a certain few with certain letters behind their name. How is that protecting the public? Isn’t that each individual hospital’s job? Not a certifying board in another state.
