CA Podiatrist Discusses Nail Care for Aging Feet
 
Taking care of toenails also becomes more important as we age. If you cannot care for your toenails properly, “it can lead to problems down the line,” says Michael Coyer, DPM, a foot and ankle surgeon in Orange County, CA. Neglect, for instance, can lead to painful ingrown toenails and damage to too-long toenails that have been bumping up against the front of your shoes.

When cutting nails, take care not to cut too far down. “Overaggressive trimming and cutting the toenails too short can lead to ingrown toenails,” Dr. Coyer says. “It’s best to cut the nails fairly straight across, ensuring that the corners of the nail do not cut into the skin folds of the toe,” Coyer says.

NY Podiatrist Coaches HS Hockey Team to Another Division Championship
 
On March 7th, 2023 the Xaverian High School Varsity Ice Hockey Team, coached by podiatrist Anthony J. Esposito, DPM, won the City Ice Hockey Varsity Championship in the CHSHL. This was their third time in four seasons as champions and the second consecutive year. This year they had to do it the hard way, just barely making the playoffs as the fourth-place team. They eliminated the first and second place teams both in a best of three-game series, bringing the cup back home.

Four Common Pitfalls Your First Year in Practice
 
Congratulations for making it through medical school, and residency, and perhaps even a fellowship. Now — finally! — you’re ready for your first or second job. You may be clinically prepared, ready to see patients and begin procedures, but are you ready from a practice management perspective? There are four areas that trip up new doctors the most.
 
4. Compliance - You don’t want to be the office tattle, but what if you start to see something that sounds an alarm? How do you handle it? If you’re working for a small group, make sure they have a compliance plan. Make sure there are some basic billing and coding audits. Hiring your own auditor is a good thing! So many doctors think they know how to bill and code but end up making mistakes, and that can really cost you. Remember that if your partners are messing things up, it can redound on you as a member of the company. If your APPs are billing under your national provider identifier and not following the incident-to rules, that will affect you. You don’t have the luxury of burying your head in the sand.

QUERIES (CLINICAL)
Query: Unidentified Lesion on Dorsum of Foot
 
An 88 year old male was seen a week ago with a painful hard raised red nodule on the top of his foot. There was no break in the skin or indication of a foreign body. It looked like possible irritation from the worn out slip-on shoes that were very tight due to his pedal edema or irritated blood vessel. Doppler revealed normal pedal arterial sounds. He is not diabetic, but on BP, cholesterol meds, and ASA 81mg.

A foam aperture pad was applied over the area and he was fit with a trauma shoe to reduce pressure. He returned today (1 week later) with the area looking like this. There was a large amount of mushy material and this picture is after debridement (without anesthetic). I took a culture, placed him on Bactrim DS for 10 days, and dispensed Telfa pads for qd application. Any ideas on what this might be?

It is totally okay for Dr. Spinner or anyone else in our profession to have professional disagreements. In the four decades that I have been a podiatrist, I have lived through and weathered many controversies and have participated in debates over many issues, with many of our esteemed colleagues. 
 
What is quite disconcerting is the way Dr. Spinner characterized one of the leading scholars in our profession. Referring to Dr. Rogers as a "cancer" or a selfish person crosses a dangerous line. I have worked with Dr. Rogers in academia for many years and he has never turned me or any of my colleagues down whenever we either needed him to lecture at a seminar or to help with the management of a wound care patient. 
 
Service to the community is what Lee Rogers is all about. Having been the president of an APMA affiliate and served on the board of directors of other podiatric organizations, I know how much work goes into this type of service and referring to someone who devotes his life to serving our profession, often without monetary compensation, in such disparaging terms is less than professional.
