Almost All Diabetic Foot Problems Can Be Prevented: CA Podiatrist
 
Diabetes involves the feet by causing problems with nerves, circulation, muscles, joints, and infections. Diabetes can also cause a frightening condition: numbness that masks the detection of important symptoms. “Many diabetic patients have peripheral neuropathy in their feet,” said Kenneth B. Rehm, DPM, Diplomate of the American Board of Multiple Specialties in Podiatry. Rehm is ABMSP-certified in diabetic foot wounds and limb preservation and salvage.

“Neuropathy shows up in many different ways, as dry skin, toenail problems, or loss of sensation. But the problem is that a lot of people with diabetes don’t realize their feet have gone numb. Patients with diabetes must be vigilant,” Rehm said. “If they take care of their feet, they don’t have to lose their feet or legs. Trouble doesn’t have to happen. Almost all diabetic foot problems can be prevented, if they’re caught early enough,” Rehm said.

Two MI Podiatrist Join McLaren Flint
 
Two new podiatrists, Drs. Karl Dunn and Rebecca Jolissaint, have joined the medical staff at McLaren Flint. They are both seeing patients at Flint Foot and Ankle. Dr. Dunn completed an Intensive Lower Extremity Fellowship at Pennsylvania Orthopaedic Center and completed his residency at Henry Ford Macomb Hospital in Clinton Township.

Dr. Jolissaint completed her residency at University Hospitals Richmond Medical Center, and Kent State College of Podiatric Medicine. Both Dr. Dunn and Dr. Jolissaint earned their medical degrees at Ohio College of Podiatric Medicine.

INTERNATIONAL PODIATRISTS IN THE NEWS
Aussie Podiatrist is Anxious to Combine Practice and Academia 
 
Podiatrist Jade Tan says treating her patients typically means assessing them, educating them about their diagnoses, discussing treatment plans, and engaging in follow-up consultations. Tan is a podiatrist at Melbourne-based In Stride Health Clinic. She's also a casual academic and PhD candidate researching the potential of orthotics to reduce pain experienced by people who have arthritis behind the knee cap.

In Australia, podiatrists are required to complete a bachelor of podiatry. Tan, who completed a bachelor of podiatry (honors) at La Trobe in 2011, says one of her main reasons for combining clinical practice with academia and post-graduate study is to extend her longevity in podiatry. "A lot of the reason why is definitely to increase career prospects... I also like variety and translating research into clinical practice is important," she says.

Citing the unprecedented worldwide rise in diabetes and its complications, Dr. David G. Armstrong, Professor of Surgery and Director of the University of Arizona’s Southern Arizona Limb Salvage Alliance (SALSA) compared it to another disease. “No one says that ‘I’ve got a touch of cancer’ and yet this is the pervasive notion about diabetes and its complications. Very few realize that a wound immediately doubles your risk for sudden death, and an amputation cuts your chances of surviving three years to 50%.”

Query: Evaluation & Treatment of the Same Condition on the Same Day
 
An established patient presents with a new complaint. I do a thorough evaluation, including history of present illness (NLDOCAT) and physical exam (4 systems). The patient is educated on her diagnosis, and the condition is treated the same day with a steroid injection. My understanding has been that I bill for either E/M or injection. My office manager is not in agreement, since a separately identifiable evaluation and management service was provided, even though it was for the same condition. Please advise.

Response: Your office manager is correct. If the E/M service is separately identifiable from the procedure performed, you may bill for that E/M service in addition to the procedure. This is true if the E/M and procedure are for the same problem and also true for an established patient. 
 
It is important that the documentation for this encounter supports the fact that a separately identifiable E/M service was performed and that it was necessary.

“Dr. Robert Baldauf, transitioned home to a magnificent party and celebration of his life on Thursday, April 28, 2016,” his family wrote in a tribute. “He will be remembered by his friends, family, many colleagues, and patients, and all who knew him as a person of generosity, integrity, wisdom, and an open heart.”

In 1984, Dr. Baldauf graduated as salutatorian at the then Pennsylvania College of Podiatric Medicine. He was an active and well respected president of the Northeastern division of the New York State Podiatric Medical Association during the 1990s. In 1987, He, started his own podiatric practice, which flourished over the years. “Dr. Baldauf was well respected by many of his colleagues and patients for his excellent skills in the office and in surgery as well as for his listening ear and wisdom,” his family wrote.

RE: Call the Cops! We’re Getting Robbed!

I wrote a one week Rx for Erythromycin 250mg generic this week. My patient reported that the cash price was …wait for it ... $450! I couldn’t believe it so I checked the local pharmacies and it ranged from $350 on up. We called one pharmacy for an explanation and received a half-hearted explanation about the unintended consequences of Obamacare. Being in practice 30 years, I believe I seem to remember E-mycin prescriptions being around $10. Be careful about what you write these days. By the way, I checked the online Canadian pricing, and it was around 81 cents a pill.

I don't believe solo practice is doomed. If in solo practice, you can join a clinically integrated network and there will be virtual groups that you may use to your advantage. This will allow for an individual or small group to survive. I also know that a solo practitioner can certainly make 500K plus yearly. This can be done with ancillary services suggested by Dr. Kiel and making a niche practice for yourself. I have done this successfully for many years. There will always be people willing to pay for cash services. As plans discourage surgery and start taking away all of the expensive medical devices used in surgery, you will need to adjust.
 
The problem as mentioned by Dr. Fellner with regard to student debt is, in my opinion, one of the biggest failures to plague the profession. What is the leadership doing to help new practitioners with crazy debt? They are just being thrown out there without help. What are the colleges doing to help? We need the profession to step up to the plate.
 
I am happy to help new doctors with advice as I feel fortunate to have had success myself and would be happy to help and mentor new grads. Feel free to contact me.

I just read the 2016 Residency Placement Status Update from AACPM published by PM News. From the numbers, I conclude that the residency shortage crisis is now over. I would like to congratulate the profession and the individuals directly involved in solving the crisis. The problem, which began in 2010, was a critical one which threatened to limit applications to the colleges, and severely delay the completion of training by many dozens of podiatrist each year. The issue was first covered by PRESENT in Practice Perfect #220 “The Podiatric Residency Shortage: What YOU Can Do?” and dealt with in many subsequent issues. 
 
There are still a few carryovers, 24 podiatrists who graduated in prior years yet to be placed, and 12 (2.2%) from this past graduating class. But the response from the profession has been tremendous. I am very proud of my profession for taking care of this serious problem as well as it did. I won’t mention any individuals by name for fear of leaving important contributors out, but we know who you are, and if any other readers wish to recognize those who made major contributions, that would be a nice addition to the PM News newsfeed.

Kudos to AAPPM for partnering with Practice Management Institute to certify podiatric medical office managers. As our reimbursement system transitions from fee-for-service to value-based care, a well trained office manager who can help accomplish this transition will be worth his or her weight in gold. A certified podiatric office manager can not only keep the business of podiatry running, but in addition can also be trained to understand how to integrate the practice with ACO and Medical Home networks, how to market the practice to the public and managed care entities, how to compliantly maximize what is left of fee-for-service podiatric care to its legal limit, how to collect outcome data to verify care value, and how to keep up with the change from PQRS to more complex systems of value verification.
 
A well trained and certified office manager working in tandem with his or her employers can enable podiatrists to come to work in the morning to practice the medicine in podiatric medicine, instead of having to trudge through the quagmire of increasingly complex practice management responsibilities.
