Melanoma of the Feet Least Often Diagnosed: NC Podiatrist
 
From the beach to the backyard, taking care of your feet and ankles in summer is essential. “Nothing ruins summer fun faster than a problem with your feet. However, a few smart precautions can help keep you healthy and safe,” says Gretchen Lawrence, DPM, a board-certified foot and ankle surgeon and an associate member of the American College of Foot and Ankle Surgeons. Don’t overlook your feet during your sun protection routine. Feet get sunburned too, and melanoma on the foot or ankle is more likely to be misdiagnosed than on any other part of the body.

A study published in The Journal of Foot & Ankle Surgery reported the overall survival rate for melanoma of the foot or ankle is just 52%, in sharp contrast to the 85% survival rate for melanomas on other areas of the body. Apply sunscreen to the tops and bottoms of feet and limit sun exposure. Dr. Lawrence notes, “If you spot abnormal moles or pigmented skin, including under toenails, visit a foot and ankle surgeon. Early detection and treatment could save your life.”

NY Podiatrist Discusses Plantar Fasciitis
 
“There are two types of plantar fasciitis: acute, which was triggered by a specific activity, and chronic, when the condition becomes worse over time,” says Hillary Brenner, DPM, a New York-based podiatrist and foot surgeon. You may also experience knee, hip, or back pain due to the changes in your gait because of the compensation from plantar fasciitis pain, adds Dr. Brenner. If this happens, or if you have worsening pain that is a 5 or greater out of a pain scale of 1 to 10, your quality of life is interrupted, or over-the-counter pain medications are no longer helping, it’s time to see a professional, she says.

“A general rule is to try to ice, stretch, and rest [your foot], wear supportive shoes for plantar fasciitis, and [take] over-the-counter pain medication for a few days,” she says. “If none of this improves plantar fasciitis symptoms after several days and the pain is a 5 or higher, call the doctor.

If you’re planning a trip to a warm location that involves any amount of walking, you need a pair of walking shoes that can stand up to the task — and your hot, sweaty hiking boots just aren’t going to cut it. “Arch support is important to look for when choosing sandals for walking and activity,” Anne Sharkey, DPM advises.

Shoes with cushioning can also make walking feel more comfortable, but it’s really the combination of cushioning and arch support that you need for all-day wear. “Cushioning can be helpful in increasing comfort, but arch support will go farther in providing long-term comfort than cushion alone." Dr. Sharkey suggests leaving “sandals that are flat without any arch support or cushioning for wearing poolside, or for when you expect to do very little walking.”

Four Common Pitfalls Your First Year in Practice
 
Congratulations for making it through medical school and residency, and perhaps even a fellowship. Now — finally! — you’re ready for your first or second job. You may be clinically prepared, ready to see patients and begin procedures, but are you ready from a practice management perspective? There are four areas that trip up new doctors the most.
 
3. AdvocatesNever act on the say-so of someone else’s lawyer, another doctor, or another partner without doing your own homework. Trust, but verify. You don’t always have to tell a potential new partner or employer that you’re calling your attorney for advice. Your communication with him or her is privileged and confidential, and they can be a sounding board for you. Many times, I end up writing the emails my clients send or raise red flags that my clients raise, and no one else knows that I exist. If you don’t have a good relationship with your lawyer, find a another one. But think about it when someone says, “This has been vetted by legal.” Who does that lawyer work for? If it’s not you, get your own opinion.

Query; Hallux Rigidus Surgery
 
The hospital is bugging me for my surgical codes for a patient whom I treated with severe degenerative joint disease. The patient suffers from hallux limitus/rigidus. I am considering the following codes: CPT 28291, CPT 28289 and CPT 28899. I found an osteochondral defect along the articular surface of the 1st metatarsal head, I did some subchondral drilling, and placed ProChondrix allograft in deficit. Any thoughts about the appropriate code to use in this scenario? (HCPCS L8699 to ASC/Hospital)

Response: From what you describe in this scenario, it sounds like you have surgically addressed hallux rigidus and the only question comes into play whether or not you used an implant. CPT 28291 (Hallux rigidus correction with cheilectomy, debridement and capsular release of the first metatarsophalangeal joint; with implant) could be a problem if the insurance company believes that the ProChondrix allograft is not an implant. I would consider CPT 28289 as there is no question that you have surgically addressed hallux rigidus.
 
I will be curious to see what other’s thoughts are on this scenario.

Congratulations to the ABPM Board. I would like to send my personal vote of gratitude and appreciation to those ABPM board members who recently resigned their positions voluntarily. I am sure that it was not a decision that was arrived at with light hearts, because I am sure that they have a love for their profession and their organization. However, after the disruption and inappropriate behavior of the president, Dr. Lee Rogers, I am sure that in their conscience they had no other choice.
 
It is disappointing, however, that the board could not unanimously decide to remove him as the president. It continues to amaze me how after every major organization within our profession, including most recently, the American Podiatric Medical Students Association has come out in no uncertain terms against policies recently enacted by...

RE: Follow the Rules and Invest for Your Retirement

Recently, there has been a spate of DPMs who have been prosecuted for taking part or originating illegal billing scams. They will pay an enormous price for participating in these after they are caught, which is inevitable. It has been reported that podiatrists are in the top 15th salary level in the U.S. I know you have very expensive college debts to pay off. But with judicious and controlled living, it is still possible to pay those off and still invest for retirement.
 
In 2006, I wrote an article for Podiatry Management entitled: “How to Invest in the Coming Real Estate Bust.” I just re-read it and was astonished at how well it relates to these exact times, today! My point is that being a DPM is still very much in the running for not only a successful career, but also successful retirement planning. I was born poor in Detroit. Nothing I could get came from any corporation or family inheritance would benefit me.
 
My point is that it is ABSOLUTELY UNNECESSARY to attempt any nefarious plot to plan and succeed doing in this profession. I hope you look up my article, which is completely germane to today’s times. I could not depend on any outside “largess” for my retirement. But it was very much “there” and remains so even in my 80th year.
