People Need to Take Lawn Mowing Dangers Seriously: VA Podiatist
 
“Lawnmower blades whirl at 3,000 revolutions per minute. Yet we see patients who’ve been hurt operating a mower barefoot,” said Matthew Dairman, DPM, a board-certified foot and ankle surgeon and a Fellow Member of the American College of Foot and Ankle Surgeons. “Foot injuries range from dirty, infection-prone lacerations to severed tendons to amputated toes. People need to take lawn mowing safety seriously because the injuries can, indeed, be very serious.”

To avoid becoming a statistic, follow these sensible guidelines: Don’t mow a wet lawn. Losing control from slipping on rain-soaked grass is the leading cause of foot injuries caused by power mowers. Wear heavy shoes or work boots—no sneakers or sandals. Never pull a running mower backward.

People Should Not Have to Endure Pain from Calluses: MI Podiatrist
 
According to Conway McLean, DPM, "Removing callus tissue is a time honored approach, although the methods for doing so can sometimes be brutal. The use of a razor blade is not recommended due to the high propensity for self-inflicted injury. Reducing the callus is an effective means of reducing pain, but the amount of relief will depend on the specifics of the tissues affected. Again, debriding the callus reduces the pressure to the living tissues and should lead to less pain. But the relief is only temporary since the situation producing the pressure remains."

10 Suggestions to be an On-time Physician
 
Many of these ideas can easily be implemented in any office, with minimal or no expenses and without adding additional staff. 
 
7. Cushion your schedule to allow the doctor to take a short break or catch up if there are delays or patients take longer than expected. It helps if you have a 5-10-minute cushion each hour. It can be used to see an urgency or emergency, return phone calls, or complete paperwork. Doing so makes you more likely to remain on schedule for the subsequent hour.

RE: Functional Hallux Limitis

Howard Bonenberger, DPM hinted about a very important and prevalent condition that is seen in many, if not all, podiatry offices. This is hallux limitus. In the early 1980s, I had a professional NFL receiver come to me with hallux limitus. It was obvious that conservative treatment was the only option. At that time, orthotics with a hallux extension was the recommended treatment. So I tried this very treatment, knowing it would probably fail. Well it did fail. 
 
At that time, I dispensed an orthotic where I increased the ability to plantarflex the first metatarsal by supporting the 1st metatarsal-cuneiform joint. This allowed for a more normal function of the first MPJ. This was very successful treatment that allowed the receiver to make his route maneuvers.

RE: Functional Hallux Limitis

Howard Bonenberger mentioned in a post last week that one of my lectures on functional hallux limitus (FHL) inspired him to treat patients with chronic postural complaints. I appreciate his comments and am glad he was able to find these concepts valuable. I have written extensively on FHL over my career, and have come to the realization that this concept remains misunderstood. The reason why it can impact postural form lies in the following principle. Once the hallux makes ground contact during any step, IT DOES NOT MOVE AGAIN UNTIL TOE-OFF. ln any form of hallux limitus (structural or functional), what is restricted is not the hallux, but the remainder of the foot and proximal structures all the way to the neck.   
 
As the body adjusts for these motions, various sites are stressed repeatedly during each step cycle. And since these motions are repeated thousands of cycles per day, the stress becomes a chronic irritant. In particular, loss of MTP joint motion restricts heel lift, shortens stride length, and causes the ensuing swing phase to be altered in such a way as to make toe-off mechanically inefficient. Since the iliopsoas is the primary hip flexor at toe-off, and originates directly from the lumbar spine, it becomes the site of pain. The references for this are below.

"Profit first decision making" in medicine has nothing to do with direct pay medical practice. Many plastic surgeons, dentists, veterinarians, and concierge practices are but a few examples of those who engage in direct pay medical practice. Dr. Kornfeld has presented reasonable and cogent arguments to endorse the direct pay model of practice. There is no more greed inherent to the direct pay model than exists in the traditional billing of third parties for medical care rendered. Your misinterpretation of the commentary which I made on this subject is the suggestion of a need for you to re-examine your own motivation for the practice of podiatry. The concept of avarice appeared in your mind, not mine. Dr. Kornfeld's thoughts and philosophy on direct pay are well taken and acknowledged as such.
 
Regardless of the payment model, ethical behavior and unethical behavior are what they are. Unnecessary surgery, the irrational use of...

I was surprised when I read that only about 1/3 of our profession is in favor of a 6 year college/podiatry school option. In an ideal world, a 4 year undergraduate degree, a 4 year podiatry degree, followed by a 3 year residency would still be preferred. However, it is not an ideal world any longer. The enrollment in our podiatry colleges is down, which means the selection pool for good candidates is not good and some of the schools may have to consider closure, if the current situation does not reverse itself. 
 
I have 4 children, all of whom had the bedside manner and the work ethics to become outstanding physicians. However, they all chose different career paths, which is more than fine, but it is the reasoning behind their decisions which still bothers me. The two main reasons that they told me for not considering medicine/podiatry were the cost and the amount of extra years of schooling.
 
I think that we need to still investigate the 6-year college/podiatry school option. We may be losing some good potential students to carry on in our noble profession.

Our loss of Dr. Michael Forman leaves our profession without an exemplary teacher, me without a lifelong friend and colleague, and our students with the loss of a dedicated professor. His White Coat is unlikely to be soon, if ever, filled. I knew him in 73-74 when I was privileged to be Co-Director of Surgery at then OCPM.
 
He made me feel welcome and helped me through the maze of my first teaching appointment. We became lifelong friends. That unholy illness has taken a wonderful man that I will deeply miss the remainder of my days. Rest well Michael, you are much loved.
