PODIATRISTS AND DIABETES
FL Podiatrist Stressses Amputation Prevention for Diabetics

More than 65,000 lower limbs are amputated annually due to complications from diabetes including diabetic foot ulcers, according to the American Podiatric Medical Association. Even worse, a huge percentage of those amputations could have been avoided if diabetic patients had stuck to their treatment plans and made at least one visit a year to a podiatrist like Amberly Paradoa, DPM at Cleveland Clinic Indian River Hospital’s ambulatory care center. Dr. Paradoa is also a wound healing and hyperbaric medicine expert as well as a foot and ankle surgeon who is keenly aware that after one amputation, the chance of having another one within three to five years can be as high as 50 percent.

“I always tell my patients,” Paradoa states flatly, “that [with peripheral neuropathy] the foot and the brain are no longer wired correctly. And that neuropathy gradually gets worse and worse without them realizing it.” She advises checking the bottoms of the feet and in between the toes looking for any red spots, any blisters, or any open sores. ”That, she says, is vitally important because “some of the statistics show that for patients with diabetic foot ulcers with diabetic neuropathy and peripheral vascular disease, the mortality rate is [just] five years.”

PODIATRISTS AND POLITICS
NJ Podiatrist Running for Seat on Borough Council

Two three-year terms on the Borough Council will be on the ballot in Freehold Borough on Election Day, Nov. 5. Michael Lichardi, DPM, on the GOP ticket, is seeking a council seat. Lichardi is a physician assistant with more than 30 years of experience and a doctor of podiatric medicine. He has completed a fellowship with the New York State Department of Health, which he said provided training and experience in governmental and political issues of medically disenfranchised and special needs populations.

Dr. Michael Lichardi

Making his second bid for Borough Council after running in 2008, Lichardi is a member of the Freehold Borough K-8 School District Board of Education; first serving from 2009-18 and joining the board for a one-year term this year. Since 2015, he has served as board president. Lichardi has been a volunteer firefighter with the Freehold Fire Department for 19 years, a Member of the Medical Reserve Corps of the Monmouth County Health Department, a former board member of Freehold Little League baseball, and a former Cub Scout assistant den leader with the Monmouth Council Boy Scouts of America.

INTERNATIONAL PODIATRY IN THE NEWS
UK Podiatrist Discusses Onychomycosis

Crusty, thickened yellow toenails are a telltale sign of a fungal nail infection – and it’s important to get them checked out and treated quickly, as the infection can spread and be notoriously stubborn. “Fungal nails cause dramatic visual changes, which can be unsightly,” says podiatrist Tony Gavin, founder of Osgo. “They can make the nail change color, thicken, and break more easily.” They can also cause more pressure on the end of the toe and become painful. “As the fungal infection progresses, they can produce a very distinct odor too,” adds Gavin.

Tony Gavin

The good news is that fungal nail infections are treatable – although more severe infections may take a while to disappear completely. “There are a variety of treatments, but success depends upon an appropriate plan. Medication needs to act on the affected areas, which can be difficult with topical preparations. A podiatrist may thin down a fungal nail or introduce very small holes into the nail and apply an anti-fungal medication,” says Gavin.

PODIATRISTS AND FOOTWEAR
Stay Away From Platform Sandals: NY Podiatrist

’90s-era platform sandals are back — and foot doctors are horrified. Last week, Steve Madden reintroduced his iconic foam platform sandals as part of a capsule collection for Urban Outfitters. On Monday, model Suki Waterhouse posted a photo of herself wearing a similar 4-inch-high pair from Simon Miller.

Dr. Hillary Brenner

“Definitely stay away from this one,” Dr. Hillary Brenner, a podiatrist based in the Financial District, tells The Post of the throwback trend. She says that platform sandals’ rigid soles can throw off your gait and stability — leaving you prone to tweaked ankles or worse. That’s opposed to shoes with more flexible soles, like sneakers. Those allow you to shift your weight from your heel to the ball of your foot as you walk. A good rule of thumb is to make sure you can bend a shoe slightly with your hands, she says.

PODIATRISTS IN THE COMMUNITY
FL Podiatrist Honored as Lightning Community Hero

The Tampa Bay Lightning hockey team honored Marc Colaluce, DPM as a Lightning Community Hero this season during a recent game. Dr. Colaluce received a $50,000 donation from the Lightning Foundation and the Lightning Community Heroes program. This donation will be given to Special Olympics of Florida for the Healthy Athletes Fit Feet Program to create increased volunteerism and stronger health screenings for those with intellectual disabilities.

Ryan Patrick McDonagh, Dr. Marc Colaluce, and Elizabeth Frazier (Executive Director, Lightning Foundation)

Dr. Colaluce runs his own podiatry practice, All Florida Podiatry, in St. Petersburg, Florida where up to thirty percent of the patients have an intellectual or developmental disability. Dr. Colaluce wanted to give back to the community so he started working with the Special Olympics over 12 years ago and now he has led the Fit Feet program in Local, Regional, State and World events. In 2012 he became the Florida Clinical Director of Fit Feet. He has since been named Regional Clinical Director of the Fit Feet Program.

PODIATRIC PRODUCTS AND SERVICES IN THE NEWS
New Study Points to SFDI as Promising Technology for PAD Assessment

Modulim, a recognized leader in SFDI-based optical imaging solutions for the non-invasive assessment of tissue health, has announced the findings of a recent study published in the Journal of Vascular Surgery. The study describes Modulim’s technology as a promising method for rapidly assessing compromised circulation in tissue in the lower extremity of patients with and without diabetes. The study examined the ability of SFDI, a new method that uses structured illumination combined with visible and non-infrared light to characterize subsurface tissue beneath the skin to evaluate lower extremity circulation in 94 limbs (47 patients).

Dr. David Armstrong

In the first-of-use study in PAD, lead researchers Craig Weinkauf, MD, PhD, an assistant professor of surgery at the University of Arizona, and David Armstrong, DPM, MD, PhD, a professor of surgery at University of Southern California’s (USC’s) Keck School of Medicine, compared SFDI outputs to current non-invasive testing standards, including the ankle-brachial index (ABI), toe-brachial index (TBI), pedal pulses, and Doppler waveforms. The results showed that SFDI revealed a distinct perfusion profile of the diabetic foot.

PRACTICE MANAGEMENT TIP OF THE DAY
Get Paid for Procedures Insurance Won’t Cover - Part 4

Getting paid for procedures continues to become more complicated. Many doctors are adding new procedures to their offerings that patients want and can benefit from, such as Botox to treat migraine headaches, dermal fillers for aesthetic purposes, laser therapies for scar reduction, stem cell and platelet rich plasma (PRP) injections for musculoskeletal disorders, genetic testing, and many more. But insurance companies often won’t pay for them. Here are some ways to get paid:

4. Participate in Care Credit or other similar programs. Physicians can sign up for a program that allows patients to make payment plans rather than paying the whole amount at once. While this may not make sense for a procedure that is less costly, it may be the only way a patient can afford more expensive ones. This is often used by orthodontists when the cost for children’s braces is in the thousands of dollars.

QUERIES (MEDICAL-LEGAL)
Query: HIPAA and Death

How long after a patient's death does the privacy protection continue? I'm afraid to even admit someone was a patient.

Larry Aronberg, DPM, Lake Worth, FL

Editor's comment: PM News does not provide legal advice. "The HIPAA Privacy Rule protects the individually identifiable health information about a decedent for 50 years following the date of death of the individual. This period of protection for decedent health information balances the privacy interests of surviving relatives and other individuals with a relationship to the decedent, with the need for archivists, biographers, historians, and others to access old or ancient records on deceased individuals for historical purposes. During the 50-year period of protection, the personal representative of the decedent (i.e., the person under applicable law with authority to act on behalf of the decedent or the decedent’s estate) has the ability to exercise the rights under the Privacy Rule with regard to the decedent’s health information, such as authorizing certain uses and disclosures of, and gaining access to, the information.

With respect to family members or other persons involved in the individual’s health care or payment for care prior to the individual’s death, but who are not personal representatives, the Privacy Rule permits a covered entity to disclose the relevant protected health information of the decedent to such persons, unless doing so is inconsistent with any prior expressed preference of the deceased individual that is known to the covered entity."

RESPONSES/COMMENTS (MEDICAL-LEGAL) - PART 1
From: Kim Antol

What makes an image storing App HIPAA-compliant is the requirement of an additional password or PIN to open and view images. (This would be in addition to your normal log-in procedure to your device). The one I use and recommend is called “Vaulty”. Once a medical image is received, you can transfer it in from your normal photo App…as well as the ability to delete or transfer it out.
