MO Podiatrist Uses HBOT to Save Patient's Limb
At age 51, Sherri Barnes was facing the amputation of her right big toe and a portion of her foot. The trouble began more than a year ago, when Barnes, a diabetic, lost her toenail. “After several rounds of the nail growing back and then falling off again, it progressed to a really bad infection and the bone was exposed,” Barnes says. “It was incredibly painful.” After a trip to the emergency department in October 2013, Barnes was referred to foot and ankle specialist Cody Fox, DPM, at Lake Regional Podiatry.

“For nine of 10 people in Sherri’s condition, amputation would be the only option,” says Fox, who commonly treats diabetic wounds. “Chronic wounds can lead to deep infections that involve bone and can eventually lead to nonviable bone. Fortunately, Sherri acted soon enough that treatment was still possible.” Barnes visited Lake Regional’s multi-services department for six weeks for IV antibiotics, as well as undergoing 20 daily visits to the Wound Healing Center, Monday through Friday, for two-hour HBO treatments. Now, after the specialized treatment, Barnes is looking forward to the future wound-free.

REMEMBERING THE BOSTON MARATHON BOMBING |
NYCPM Student Among The Medical Volunteers at Boston Marathon
Warm weather sent hundreds of overheated and dehydrated marathoners to the finish line medical tent yesterday and kept doctors and nurses busy for hours. But the fatigued runners were a welcome change for the medical volunteers, who last year found themselves treating scores of patients with a wide range of traumatic injuries after the marathon bombings.

The bombing drew some first-time volunteers. “That’s the biggest reason I came,” said Mohammad Rimawi, a third year podiatry student from New York. “When things like that happen, the best thing we can do is not to give up.” He tended to sore feet, while others checked blood pressure, gave runners fluids, and wrapped them in blankets.

VA Podiatrist Gets Jail Time in Orthofix Probe
A Virginia podiatrist is sentenced to jail time for lying to a grand jury about her involvement in a scheme to defraud Medicare by collecting reimbursement for procedures using Orthofix's bone growth stimulator. Virginia podiatrist Dr. Ilene Terrell drew a jail sentence yesterday for lying to federal investigators probing Orthofix International and its bone-growth stimulators. Terrell was sentenced to 5 months in jail, 5 months of home confinement, 2 years of supervised release and a $15,000 fine, according to the office of the Massachusetts U.S. district attorney.

Alleged Lack of Informed Consent and Negligent Supervision (FL)
Facts: An adult female patient, reportedly suffered osteonecrosis on her left foot resulting in permanent deformity to her great toe, an abnormal gait, and mental anguish after defendant podiatrist performed a modified Mcbride bunionectomy with hammertoe correction and plantar plate release. The plaintiff contended Wellens fell below the standard of care by failing to obtain consent to perform the bunionectomy, failing to advise the plaintiff of the risks, overtightening the medial capsule, placing the plaintiff's great toe in a varus malposition, failing to perform the surgery with neutral to slight valgus natural balance positioning of the toe and failing to prescribe a Dyna-Splint or similar device.
The plaintiff additionally contended that defendant was vicariously liable. The defense witness opined that the plaintiff's injuries were the result of a reduced blood supply to the area and the patient's inability to heal the bone. The defendants denied liability. The plaintiff's spouse made a claim for lost consortium but received no award.
Result: Plaintiff Verdict Total: $109,922 Judge Reduced Award To: $0

Hospitals Get Into Doctor Rating Business
After some doctors at University of Utah Health Care noticed scathing online reviews about themselves in 2012, the hospital system decided the best way to respond was by posting its patients’ ratings of physicians on the hospital’s own website. The hospital was already randomly surveying patients about their experiences with physicians. Now, when potential patients use online search engines to look for a University of Utah Health Care doctor, the hospital’s reviews pop up first. Many hospitals have surveyed patients about doctors for decades, but the data was usually only used internally.
In the past few years, some hospitals including the Cleveland Clinic, have started showing doctors how they compared to each other on the ratings. Online ratings of physicians haven’t caught on as much as ratings of books and movies, but medical information sites, such as Healthgrades and Vitals, and broader consumer sites including Yelp and Consumer Reports offer physician reviews. Some score doctors based on patient surveys, and some allow patients to post comments. A research letter in the Journal of the American Medical Association found that one in four patients consulted them when picking a primary care doctor in 2012.

Whenever patients tell you “I need a discount; your fees are too high,” look them square in the eye, count silently to three and then offer one of these responses:
• “Thank you for sharing that. How much too high are we?”
• “I appreciate your telling me that. Have you found a less expensive doctor?”
• “You are right; we are more expensive than some other doctors. How much did you hope to pay?”
• “I agree.We are not the cheapest. Is price your main consideration?”
• “Thank you for letting me know. I’m curious: How much were you expecting to invest in your health?”

I too share the frustrations of many other podiatrists and suppliers, when it comes to dealing with therapeutic shoes and the nightmare the Medicare contractors have caused. However, the reality is that I hear from far many more DPMs (and others) who are successful when it comes to pre-payment and post-payment audits than I do from those with negative outcomes..
Having all the paperwork in order and taking no short cuts, does usually vindicate the provider at the contractor level. The biggest problem most suppliers have is getting the required...

I appreciated Dr. Steinberg’s comments about the use of orthoses versus wedges in cycling shoes. Dr. Shier’s comment that cycling is a non-weight-bearing sport is a little misleading. Cycling is a partial weight-bearing sport in which the forefoot is used to apply force to the pedals of the bike in order to create rotation of the bike’s sprocket.
As a result of the pressure exerted on the ball of the foot, the lower extremity, which includes the subtalar and midtarsal joints, is functioning in a closed chain environment. It is similar to the...

For those high performance road shoes, with little extra space, I designed a special cycling orthotic, with the help of the owner of JSB Orthotics Lab. I figured that since there is no heel strike, there is no need for the plastic shell under the heel. The rear portion is cut off, leaving a convex shape to fit just proximal to the calcaneus. As I said previously, most often I add a metatarsal pad. I also order external forefoot posts, whenever forefoot posting is necessary. I do use a full length, thin top cover. The shell and topcover take up no more space than the shoe's sockliner that covers screws for the cleat. Without the shell under the heel, the calcaneus stays snug in the heel counter.
