APWH Holds First Student/Resident Conference
The Academy of Physicians in Wound Healing (APWH) has spent the last 6 to 12 months developing a "student/resident mentoring initiative" multifaceted program. This project recently provided the APWH first student/resident conference. This conference was held with students from Temple University School of Podiatric Medicine and Western University College of Podiatric Medicine of Pomona, CA.

The event was entirely put together by students and residents. It was remarkably successful; everything ran on time without a hitch and the lectures were of quality that reflected professional academic wound healing lectures around the country. APWH President Steven Kravitz, DPM explains, "COVID-19 has affected medical education significantly and this provides a method to help students and residents develop academic interesting programs and interactive activity to learn more from each other". Brandon Ryan Mojica, TUSPM '22 was the conference student chairperson. Harry Penny, DPM and Randolph Fish, DPM serve as chairs of the initiative.

Brits Have Developed ‘Lockdown Feet’ After Pandemic
With many Brits spending a lot more time at home during the pandemic, being comfortable and wearing slippers all day has been one of the small benefits. But it looks like our comfy footwear has actually been causing health problems, with people developing 'lockdown feet'. If you've been experiencing pain in the arch or heel of your foot, it's possible that you've got what is officially called plantar fasciitis. The College of Podiatry has warned that one of the causes is floppy soles or poor quality footwear - much like those fluffy socks you've been wearing while you work from home.

Emma McConnachie, a spokesperson for the foot health specialists, told The Sun: "The ligaments in the foot can stretch out, arches are unsupported, and putting on anything with a heel becomes painful. "If your feet go unsupported, they can start to feel the strain. Wearing floppy slippers all the time eventually takes its toll. "Many patients are struggling with foot issues and are trying to fix the problems themselves. They assume we are closed, but podiatrists are very much open and there to help."

Improper Heel Height Can Cause Back Pain: NY Podiatrist
If you’ve had back pain for more than a month, your pain is worsening over time, or you’re developing new neurologic symptoms such as numbness or weakness in any part of your body, it is best to see your healthcare professional. But if you’ve cleared your symptoms with your doctor, or you want to give some commonsense guidelines a try first, it’s worth considering whether your footwear choices may be contributing to your back pain.

According to Pauline Fu, DPM, a doctor of podiatric medicine and an assistant clinical professor of orthopedics at Mount Sinai Hospital in NYC, shoes with a slight heel are best. The fact that your heels are slightly elevated allows your ankle to turn a little bit and rotate differently. A heel that’s the same height as or lower than the toe of the shoe, on the other hand, affects the way your leg — and, in turn, your pelvis — turns. That, then, will affect your spine and lower back and cause back pain, Dr. Fu says.

TX Podiatrist Sentenced to 30 days for Repeated Sexual Assault of Child
A Katy podiatrist is currently serving a 30-day jail sentence for the continual sexual assault of a child, often under the guise of giving the child an athletic massage. On March 30, Dr. Hanna Elias Francis, 62, from Katy, pleaded guilty of the charge of repeated sexual assault of a child and waived his right to trial. A judge granted Francis 30 days in jail and 10 years deferred adjudication in exchange for the plea, court documents show.

RE: Abnormal Vascular Physiology in the Lower Extremities a Risk Factor for Ischemic Stroke and Mortality- A Potential Role for Podiatric Physicians

Peripheral artery disease (PAD is pervasive among the general population, affecting up to 25% of patients 55 years or older. There is a known association with acute ischemic stroke but limited large-cohort studies exist pertaining to the relationship between PAD severity and incident ischemic stroke. This study evaluates the risk of incident ischemic stroke and mortality along the spectrum of low and elevated ankle brachial index (ABI) measurement. The authors performed a retrospective extraction of ABI data of all adult patients who underwent a lower extremity physiology study for any indication from January 1, 1996, to June 30, 2018, in the Mayo Clinic health system. PAD was categorized into severe, moderate, mild, and borderline based on ABI measurements and poorly compressible arteries (PCA). These were compared with normal ABI measurements. Associations of PAD/PCA with new ischemic stroke events and all-cause mortality were analyzed. A total of 39,834 unique patients were included with a median follow up duration of 4.59 years.
All abnormal ABI groups, except borderline PAD, were associated with increased risk of incident ischemic stroke after multivariate regression compared to normal ABI. Patients with PCA carried the greatest ischemic stroke risk (HR, 1.30 [95% CI, 1.15–1.46). Similarly, abnormal ABI groups were associated with a significant risk for all-cause mortality in a severity-dependent manner, with severe PAD conferring the greatest risk (HR, 3.07 [95% CI, 2.88–3.27). This study adds to the growing body of evidence that both PAD and PCA are independent risk factors for incident ischemic stroke and all-cause mortality. The association of PAD severity and PCA with risk of ischemic stroke may help clinicians with risk stratification and determining treatment intensity. The ankle brachial index (ABI) is a reliable and noninvasive method to assess lower extremity physiology that can highly predict recurrent ischemic stroke outcomes in patients with prior ischemic stroke. The problem with this type of study is the small number of people tested. In the backdrop of where the U.S. put a pause on the J&J coronavaccine because one out of a million people got sick from it, trying to prove that a medicine is good for warts when only 68% out of a total of only 45 people got good results, does not sit well. We all know that any stimulus including needling without medication added, could make a wart disappear. We also know that sal acid therapy has been studied on a far greater number and has been shown to be 75% effective. Granted, any study including an anecdotal study of even one patient, has value. It would, however, be prudent to wait until far greater numbers of people are studied before injecting our patients with HPV vaccine.

BioLab Sciences is the manufacturer of “Fluid Flow”. BioLab Science applied for a Q code and was issued a HCPCS code of Q4206 to be used when injecting Fluid Flow. Their intended use verbiage in that application, includes musculoskeletal inflammatory conditions like fasciitis and tendonitis. Coverage is dependent on an NCD (national coverage determination) or LCD (local coverage determination) handed down by CMS. If neither an NCD or LCD exists, then coverage falls to “medical necessity”. To date, there is no current NCD concerning injections of amniotic fluid.
It is important to look at the existing LCDs in your MAC region to better understand coverage in your particular area.

RE: Informed Consent and the COVID-19 Vaccine

Podiatrists have been prescribing drugs with systemic actions for treating foot and ankle problems for years. Every time you give a patient a prescription for an antibiotic or a pain medicine, you are doing that. I do not think that podiatrists who are administering coronavirus vaccines are really doing anything that they really have not done before. However, we must remember that the coronavirus vaccine is considered experimental by the governmental agency that approved it. Remember that it would not be approved otherwise.
We also must remember the real impetus for obtaining the informed consent of a patient before treating them was a result of what took place in Nuremberg, Germany after World War II. I have seen this concept used successfully in medical malpractice trials. Are all of the podiatric vaccinators carefully explaining this to the patients that they are vaccinating? That is, are they truly obtaining a patient's informed consent in each case?
