PODIATRISTS IN THE NEWS |
PA Podiatrist Describes Two Types of Compression Socks
You’re not imagining it — your legs start to feel heavy, swollen, and tired after sitting or standing for a while. If you don’t move around every 60 to 90 minutes, fluid starts to build up in your lower body, which creates that uncomfortable feeling, says Khurram Khan, DPM, a professor and chairman at Temple University’s School of Podiatric Medicine. Walking around may help, but to really alleviate aches and pains (or prevent them in the first place), try wearing a pair of compression socks.
|
Dr. Khurram Khan |
There are two types of compression socks: Graduated compression socks: These compression socks, which are best for most casual wearers, are tighter around the foot and ankle and gradually get looser higher up the leg, hence their name, says Dr. Khan. Anti-embolism compression socks: These compression socks are specifically designed to prevent blood clots by applying a uniform amount of pressure across the foot, ankle and calf, says Khan.

PODIATRISTS AND DIABETES |
Foot Care Especially Important for Diabetics: MS Podiatrist
People who are healthy and take care of their bodies typically see a podiatrist when they have a problem. But for some people — diabetics especially — taking care of their feet is especially important. "With diabetes, it's important that people get their eyes and feet checked on a regular basis, about every six to 12 months depending on their overall health. That’s also recommended by the American Medical Association," said Vern Christensen, DPM, a podiatric surgery specialist with over 25 years of experience. He is affiliated with North Mississippi Medical Center Gilmore-Amory.
|
Dr. Vern Christensen |
"With diabetes, we can see some complications," Dr. Christensen said. "On the far end of the spectrum, unfortunately, we tend to see people not do what they're supposed to do, and we end up seeing those move on to amputation.”

PRACTICE MANAGEMENT TIP OF THE DAY |
13 Tips for Firing Practice Employees
Firing an employee is rarely pleasant, but it can be especially sensitive in a medical practice, where patient care and privacy are top priorities. When handled properly, a termination can protect the practice’s reputation and maintain a professional environment. Here are 13 tips for managing the process:
9. Have a Witness Present
An additional manager or human resources representative can serve as a witness and help ensure the conversation remains professional and factual. This person can also provide support if the conversation becomes tense.

RESPONSES/COMMENTS (CODING & BILLING Q & A's FROM THE DOCTORLINE.COM) |
From: David J. Freedman, DPM
In response to the thedoctorline.com posting on 4/15/25, CMS released the following statement on 4/11/25: "As part of the transition to a new Administration, CMS is reviewing its coverage policies for skin substitute products. CMS believes it is important to maintain patient access to skin substitute products with high quality evidence of effectiveness. Because of this review, the Skin Substitute Grafts/Cellular and Tissue-Based Products for the Treatment of Diabetic Foot Ulcers and Venous Leg Ulcers Final Local Coverage Determinations effective date will be delayed until January 1, 2026.
The Agency requests that any peer-reviewed publications and high-quality findings from other public sources of skin substitute study results be submitted to CMS at CAGInquiries@cms.hhs.gov by November 1, 2025. CMS will ensure all evidence received will be sent to the MACs to review to determine if revisions to the LCD are appropriate. For more information on the LCD process, see Local Coverage Determination Process and Timeline CMS. For the policies, click this link:

RESPONSES/COMMENTS (CODING & BILLING) |
From: Kristin Happel
A couple of things jump out at me here: 1. Was an orthotic benefit verification made by your staff before the patient was cast? 2. Why was a -GA modifier appended to both lines of the L3000? These two questions are intertwined, as it were.
If an orthotic benefit verification were done, staff would know if orthotics were covered. If they were covered, there should be no reason for the -GA modifiers on the claim (Check with your billing company on why this modifier was added). What the -GA modifier is telling the insurance company is that you expect the orthotics to be denied for not being reasonable and necessary for Medicare. Was this a Medicare claim?
In terms of documentation, Optum is very...

RESPONSES/COMMENTS (MEDICARE ADVANTAGE) - PART 1 |
From: Ivar E. Roth, DPM, MPH. Jim Shipley, DPM
I see so much abuse with grafts and now revascularization procedures. I wish our profession would just do what is in the best interest of the patient and not the doctor’s pockets. I know it is tough out there taking insurance and that is why I advocate for podiatrists to take up the direct pay practice. Get off the insurance drugs and live clean and free again.
Ivar E. Roth, DPM, MPH, Newport Beach, CA
I just wanted to respond to the posts regarding LE revascularization procedures performed by interventional cardiologists and radiologists. Half of the practices I'm privileged to be associated with reside in rural communities. Every time that we needed to refer to vascular for a consult, and hopefully revascularization, our patients were put on a 6+ month waitlist that almost always seemed to end up in BK amputations with no revascularization even attempted.
This is their profession and not mine and so I don't desire to...

RESPONSES/COMMENTS (MEDICARE ADVANTAGE) - PART 2 |
From: Paul Kesselman, DPM
I couldn't agree more that it is absolutely ludicrous that the big business entities of medicine continue to get fat increases and patted on the back (even though many of their executives are guilty of participating in fraud in the billions of dollars...data mining). But this fight is not for just DPMs; it's about the survival of every healthcare provider, DPM, MD, DO, and all others who bill third-party payers. Dr. Brisbee and APMA cannot afford and should not fight this alone. In fact, this is not even organizational medicine's fight. We need a grassroots effort by all healthcare providers to fix a broken system from the ground up.
Until that happens we will be stuck with an antiquated system which simply does not work for patients and for which all healthcare providers will continue to be disenfranchised and not properly compensated!
Paul Kesselman, DPM, Oceanside, NY
