PA Podiatrist and Temple Students Become First Responders at Boston Marathon
For 28 years, Dr. Howard Palamarchuk has taken his Temple University School of Podiatric Medicine students to the Boston Marathon. "This is a field trip for them, kind of a graduation trip to come up and take care of the runners," he said just a few hours after the deadly explosions. "Usually, we take care of problems -- ankle problems, blister control. That's about as bad as it ever gets. Until today."

"And suddenly, there were just two -- one large explosion followed by another. You could feel the concussion of it. And then it smelled like gunpowder. Then, all of the sudden, the medical tent suddenly became a mass casualty tent." Although no one expected to go from blister patrol to triage, Dr. Palamarchuk said he was proud of his students. "Everyone treated. It's just quick, grab whatever you can. They reacted quickly and took the lead of the medical doctors. It was a good effort. I was proud of them."

TX Podiatrist Discusses Many Benefits of Epsom Salt Foot Soaks
As the weather warms, Epsom salt soaks can help your feet feel better, look better, even smell better, according to Dr. Jeffrey Bowman, a foot doctor who serves on several committees with the American Podiatric Medical Association. Soaking in Epsom salt can help with a range of foot issues – from athletes who have been exercising aggressively to people with injuries, infections, inflammation or arthritis, said Bowman.

The Epsom salt – actually magnesium sulfate – can be absorbed through the skin, Bowman said. That helps ease muscle and joint pain, swollen and inflamed feet, foot and toe fungal infections, odor and skin problems. "Epsom salt helps reduce swelling and increase circulation," said Bowman, past president of the Texas Podiatric Medical Association and an avid runner who soaks in Epsom salt while training.

NYCPM Student Receives ACFAS Award
Robin Lenz was given the Michael Stone Outstanding Professional Conduct Award at the recent American College of Foot and Ankle Surgery conference in Las Vegas, NV. The American Board of Podiatric Surgery (ABPS) presents this exceptional award annually to one fourth-year student from each of the nine podiatric schools. These students are selected due to great service to their respective college, professional conduct throughout all four years, and a focused interest in foot and ankle surgery.

Throughout his years at NYCPM, Robin has participated in numerous volunteer activities as well as conducted and presented various research projects. In 2011, Robin’s first research project at NYCPM was awarded runner-up for the Ralph Ger Student Platform Presentation award for an oral presentation on the Anatomic Study of the Transverse Occipital Ligament with Clinical Relevance. Impressively, this research went on to be published in The Spine Journal, a prominent orthopedic journal.

Query: Unresponsive Vesicular Dermatitis
I have been treating a 54-year-old man for an erythematous, vesicular dermatitis involving the plantar medial aspects of both feet, left worse than right and the lateral distal margin of the right foot. He has no other dermatosis elsewhere body wide. He has type II diabetes, hyperlipidemia, and hypertension. This condition has been present for about 7 to 8 months. Prior episodes responded to OTC moisturizing. He has had very thick toenails for many years and also thick nails on one hand. Exam resulted in a positive PAS for the toenails and negative for the skin. Skin biopsy came up with a differential diagnosis of: chronic allergic contact dermatitis, chronic nummular eczematous dermatitis, or chronic dyshidrotic dermatitis (pompholyx)).
I've been treating him with fluocinonide ointment, oral Lamisil, topical clotrimazole, and desiccant measures. While he has had some small improvement, the dermatitis persists. The dermatitis causes very little itching, but the skin gets dry and cracks, causing pain. He notes that his fingernails have been clearing but the toenails are status quo after about 2 months of oral Lamisil. Any suggestions?

RE: EMR Compatibility with Dragon Voice Recognition (John Scheffel, DPM)

A suggestion would be to dictate with Dragon Voice Recognition into a standard word processing program (e.g., Microsoft Word), then copy/paste that transcription into the EMR. You did not mention if your EMR is onsite or in the cloud; this could make the difference. Sometimes using Dragon across the internet into a cloud EMR is a very slow process. If your EMR is onsite, the problem may be with the amount of available RAM of your server. An IT specialist could help you determine this. Personally, I use my old Medinotes program to transcribe, then copy/paste into Practice Fusion. This allows me to have a copy available onsite as well as the EMR in the cloud.

I use Dragon Medical 10. There is also a Dragon Medical 11. It is important to also use the Nuance PowerMic. These are all divisions of the original Dictaphone company. You can say “Open Dictation Box” almost anywhere in any program. Dictate to the box, then hit the transfer button or say “transfer”. However, the problem can probably be resolved with settings. Go to TOOLS>OPTIONS>and review all tabs. If this gets confusing or doesn’t work, call the people who sold you the program, or better yet, call your EMR directly, and they should be able to resolve the problem.

RE: Bullying By Stomping on Teens' Feet

Recently, I saw a young male teenager of small stature with bad ingrown toenails who has been wearing steel toed boots at school for a few months. I find that such a shoe can cause toenail problems, as with our shipyard workers nearby; but in this case, I pursued the reason why he was wearing this type of boot during school. After some hesitation, he responded that it is for protecting his feet. This is the second kid in 6 months who has other boys bullying him by stomping on his feet and the feet of "weaker" kids. This is an important area of social youth consequence that needs to be addressed within our educational system. Our profession needs to alert all podiatrists and other specialists to be aware of this situation.

RE: APMA Pinterest Page (Crystal Holmes, DPM)

Honestly, I do not believe Dr. Holmes understands social media any more than the "kids" in the APMA's PR department. Why is it okay with you that the APMA Pinterest site contains random unscientific material? You are mistaken if you think the majority of users understand the difference between what a DPM posts, and what the general public posts.
Why is it that you, or anyone on your committee ever posts on the APMA LinkedIn page? Why is it that the page is used for companies who do not support the APMA to "sell"? Why is it the page has more post from non-DPMs than DPMs?
I know the process. The APMA staff writes the stuff, and then the committee is supposed to review it. I only have to point to last Spring's Foot Prints, stating we make custom insoles (no mention of prescription orthotics), as to the failure of the committee to review and guarantee that we sound like we know what we are - the experts, when it comes to the foot and ankle

I recently wrote a letter on this forum excoriating the APMA for its incompetence and irresponsibility in the development of the Pinterest site Beat-Bunion-Blues. A response to my post was published on this site several days ago and it is even more ridiculous and irresponsible than the site itself.
Crystal Holmes DPM, the chairperson of the APMA communications committee stated, "images (not the source of the images) are the content users are interested in)." So, according to this web wizard, people looking for advice about bunion treatment are only looking for pictures, not...

I am personally and professionally offended by the inane and misinformed comment by Dr. Jacobs about Dr. Gastwirth. There is no individual within our profession who since 1970 has contributed to its progress more than Glenn Gastwirth, DPM. We were classmates at NYCPM, close working colleagues during my tenure on the APMA Board of Trustees, and my confidante and guide during my tenure as APMA president. We continue to communicate on issues of mutual concern. Despite my professional respect for Dr. Jacobs, his comments should be retracted and an appropriate apology offered.

Dr. Kornfeld understands the 104 crisis, but I suggest it to be the iceberg tip of the DPM titanic crisis that podiatry is currently sailing on. Podiatrists, residents, and the “lost 104” need short, cheap, intense workshops and training programs to, as Robert has so aptly stated, establish “thoughtful, patient-specific diagnostic and treatment paradigms” in practice. These courses must also create a profitable core to practice that will ensure a respectable income commensurate with the investments we are making every day to be healers.
With that in mind, I am offering 1-2 or 3 day workshop/practice shadowing at The Foot Typing Centers of LifeStyle Podiatry in NYC to any lost 104 or current 2nd or 3rd year residents. These workshops will focus on Foot Centering Biomechanics®, Closed Chain Dermatology®, and “uninsured/uncovered” programs like lasers and in-offfice dispensing. There will be no associated fees.
I will allow 2 participants a week, starting in two weeks and lasting throughout the summer. Participants must dress, act, and behave professionally.

I would like to clarify my offer since many of the 104 wanted to know how to "apply" to the Pre-Residency Fellowship in Integrative Podiatric Medicine. This offering is open to all of "The 104." There is no application necessary, only the desire to become part of medicine’s fastest growing sub-specialty of integrative medicine and the desire to become part of the residency shortage solution on a team, in community with collaboration.
We are looking to create a population of podiatrists who will enter their professional practices with an expanded focus on foot and ankle medicine. The fellowship will include medical training, education, and research through the lens of integrative medicine.
